Background And Study Aim: Local resection of early rectal cancer is being increasingly used. With invasion of the muscularis propria layer of the rectal wall, the risk of lymph node metastasis becomes too high to consider this the optimal oncological treatment. Therefore, a diagnosis of muscular invasion is important before attempting local resection; however, endoscopic and magnetic resonance imaging (MRI) images have limitations, such as overstaging (26-31%).

Deep learning for segmentation of colorectal carcinomas on endoscopic ultrasound.

Tech Coloproctol

December 2024

Department of Applied Mathematics, Technical Medical Center, University of Twente, Drienerlolaan 5, 7522 NB, Enschede, the Netherlands.

Background: Bowel-preserving local resection of early rectal cancer is less successful if the tumor infiltrates the muscularis propria as opposed to submucosal infiltration only. Magnetic resonance imaging currently lacks the spatial resolution to provide a reliable estimation of the infiltration depth. Endoscopic ultrasound (EUS) has better resolution, but its interpretation is investigator dependent.

Introduction: The muscle-sparing direct anterior approach (DAA) for total hip arthroplasty (THA) has gained popularity for reduced postoperative pain and faster rehabilitation. The aim of this study was to evaluate the potential benefits of transitioning from straight lateral approach (SLA) THA to DAA THA, and investigate whether these benefits outweigh potential disadvantages of the DAA learning curve.

Method: Patients undergoing surgery between 2015 and 2020 ( = 2275) were categorised into three periods: pre-transition period (2015-2016) utilizing SLA exclusively, transition period (2017-2018) incorporating both SLA and DAA, and post-transition period (2019-2020) employing only DAA.

Background: A previous prospective multicenter study revealed the change of the oncologists' chemotherapy advice due to the 70-Gene signature (GS) test result in half of the estrogen receptor-positive (ER+) invasive early-stage breast cancer patients with disputable chemotherapy indication. This resulted in less patients receiving chemotherapy. This study aims to complement these results by the 7-year oncological outcomes according to the 70-GS test result and the oncologists' pre-test advice.

Article Synopsis
  • Early introduction of peanut for high-risk infants may help prevent peanut allergies, but it's unclear how to best diagnose reactions reported at home.
  • In a study of 186 infants who had reactions to peanut, 69% showed sensitization, but 73% of the oral food challenges were negative, allowing safe home introduction of peanut.
  • After 6 months, 96% of infants continued consuming peanut regularly without reactions, indicating that testing can prevent misdiagnosis and support long-term tolerance.*

Aims: Digoxin is the oldest drug in cardiovascular (CV) medicine, and one trial conducted >25 years ago showed a reduction in heart failure (HF) hospitalizations but no effect on mortality. However, later studies suggested that the dose of digoxin used in that trial (and other studies) may have been too high. The DECISION (Digoxin Evaluation in Chronic heart failure: Investigational Study In Outpatients in the Netherlands) trial will examine the efficacy and safety of low-dose digoxin in HF patients with reduced or mildly reduced left ventricular ejection fraction (LVEF) with a background of contemporary HF treatment.

Article Synopsis
  • The study compares the effectiveness of baricitinib and TNF-inhibitors (TNFi) in treating rheumatoid arthritis (RA) patients who did not respond to conventional treatments.
  • A total of 199 patients were randomized, showing baricitinib to be both non-inferior and superior in achieving meaningful clinical responses at 12 weeks, with a higher percentage of patients reaching specified disease activity targets compared to TNFi.
  • The results suggest that baricitinib may be a more effective initial treatment option than TNFi for RA patients who have failed previous therapies, based on the outcomes measured over 48 weeks.
