Introduction: The importance of the appropriate therapeutic interventions in children and adolescents with bipolar disorder (BD) cannot be overstated since treatment choices and their consequences may have effects into adulthood.
Areas Covered: Randomized clinical trials (RCTs) investigating treatment of mania, bipolar depression, and maintenance in adolescents with BD are reviewed. When RCTs are not available or are inadequate, naturalistic data or open studies are also reviewed.

Background: Bipolar disorder type I is a severe psychiatric condition that leads to significant morbidity and mortality and whose treatment remains suboptimal. Its pathophysiology involves disturbance in the control of ionic fluxes so that when patients are either manic or depressed, the resting membrane potential of neurons is more depolarized than normal. Available mood stabilizers have a shared mechanism of normalizing ion flux by compensating for ionic abnormalities, and normalizing membrane potential.

This study used a flanker task with NoGo elements to investigate frontal executive function deficits in 19 cocaine abusers. The executive functions of interest in this study were cortical inhibition or ability to withhold motor response, the ability to select an appropriate response among several competing ones, the ability to inhibit inappropriate responses, and the ability to detect error and exercise corrective control. These processes were evaluated with specific frontal and parietal event-related potentials (ERP) registered during performance on this speeded reaction time task with conflicting motor response demands.

Autism is a condition manifested as abnormalities of relatedness, communication, range of interests, and repetitive behaviors. Despite alarming prevalence estimates and exhortations to research, little is known regarding its pathophysiology. Recent reports of a putative minicolumnopathy explain changes in brain size, gray/white matter ratios, and interareal connectivity.
