Introduction: Shared decision-making (SDM) is a method of care by which patients and clinicians work together to co-create a plan of care. Electronic health record (EHR) integration of SDM tools may increase adoption of SDM. We conducted a "lightweight" integration of a freely available electronic SDM tool, CV Prevention Choice, within the EHRs of three healthcare systems.

Aging is a major risk factor for most chronic diseases, including osteoporosis, and is characterized by an accumulation of senescent cells in various tissues. MicroRNAs (miRNAs) are critical regulators of bone aging and cellular senescence. Here, we report that -- decreases with age in bone samples from mice as well as in posterior iliac crest bone biopsies of younger versus older healthy women.

Article Synopsis
  • A study investigated the link between lipid level variability and cardiovascular disease (CVD) risk in a large cohort from electronic health records, focusing on individuals over 40 years without prior CVD.
  • The researchers analyzed data from 19,652 patients, finding that those with high variability in total cholesterol levels had a 20% increased risk of developing CVD compared to those with low variability.
  • The findings suggest that lipid variability could be a potential risk marker for CVD, but further research is needed to validate its clinical application.

Article Synopsis
  • The incidence of ST-segment-elevation myocardial infarction (STEMI) in young women (18-55 years) has been increasing, despite an overall decline in the U.S.
  • Analysis of hospital data from 2008 to 2019 showed a decrease in STEMI hospitalizations for women aged 45-55, while those aged 18-34 and 35-44 saw an increase in rates.
  • The risk factors for cardiovascular disease have risen across all age groups, but in-hospital mortality rates have remained stable, highlighting a need for improved risk assessment and management strategies for STEMI in younger women.

A gene fusion in a patient with myeloid/lymphoid neoplasm with eosinophilia and tyrosine kinase gene fusions: a case report and review of the literature.

Cold Spring Harb Mol Case Stud

February 2023

Division of Hematopathology, Department of Laboratory Medicine and Pathology, Rochester, Minnesota 55905, USA;

Article Synopsis
  • Myeloid/lymphoid neoplasms with gene fusions, known as MLN-TK, are a rare type of blood cancer recently recognized by health organizations.
  • A 33-year-old man was diagnosed with MLN-TK after tests showed he had a specific gene fusion, making him the third known case of this condition.
  • Unlike other cases, this man had milder symptoms, showing that MLN-TK can act differently in different people; doctors suggest genetic testing to help treat patients with similar issues.

Article Synopsis
  • The study compared the effectiveness of percutaneous left atrial appendage occlusion (LAAO) to non-vitamin K antagonist oral anticoagulants in patients with atrial fibrillation, using data from over 562,000 patients between 2015 and 2018.
  • It found no significant difference in the risk of major complications like ischemic stroke or systemic embolism, although LAAO was linked to a higher risk of major bleeding and a notably lower risk of all-cause mortality, especially in patients with a history of intracranial bleeding.
  • The results suggest that LAAO could be a viable treatment option for certain patients, but highlight the necessity for careful management of bleeding risks following the procedure.

Article Synopsis
  • - The POLARIX trial found that pola-R-CHP offers better progression-free survival than R-CHOP for patients with untreated intermediate- to high-risk DLBCL.
  • - A survey of 174 clinicians, primarily from academic centers in the U.S., revealed that 70% would not switch to pola-R-CHP due to its limited PFS benefits and high costs.
  • - There was significant variation in how clinicians interpreted the trial results and their strategies for applying pola-R-CHP in practice.

Our phase III trial reported that autograft-absolute lymphocyte count (A-ALC) improved survival post-autologous peripheral blood hematopoietic stem cell transplantation (APBHSCT) for a short-term follow-up of 2 years. We evaluated retrospectively in our phase III trial patients that the A-ALC still confers survival benefit with a longer follow-up. With a median follow-up of 127.

Background Lactic acidosis is associated with mortality in patients with cardiogenic shock (CS). Elevated lactate levels and systemic acidemia (low blood pH) have both been proposed as drivers of death. We, therefore, analyzed the association of both high lactate concentrations and low blood pH with 30-day mortality in patients with CS.

Background Elevated plasma ceramides are independent predictors of cardiovascular disease and mortality in patients with advanced epicardial coronary artery disease. Our understanding of plasma ceramides in early epicardial coronary artery disease, however, remains limited. We examined the role of plasma ceramides in early coronary atherosclerosis characterized by coronary endothelial dysfunction.

Humidified rapid-insufflation ventilatory exchange (HRIVE) is an option for maintenance of oxygenation. This technique allows for oxygenation while the patient is apnoeic due to continuous positive airway pressure and gas exchange through flow-dependent dead space flushing. There is no study about the usage of HRIVE during rigid bronchoscopy.

Background Age-related heart diseases are significant contributors to increased morbidity and mortality. Emerging evidence indicates that mitochondria within cardiomyocytes contribute to age-related increased reactive oxygen species (ROS) generation that plays an essential role in aging-associated cardiac diseases. Methods and Results The present study investigated differences between ROS production in cardiomyocytes isolated from adult (6 months) and aged (24 months) Fischer 344 rats, and in cardiac tissue of adult (18-65 years) and elderly (>65 years) patients with preserved cardiac function.

Changes in aging bone that lead to osteoporosis are mediated at multiple levels, including hormonal alterations, skeletal unloading, and accumulation of senescent cells. This pathological interplay is superimposed upon medical conditions, potentially bone-wasting medications, modifiable and unmodifiable personal risk factors, and genetic predisposition that accelerate bone loss with aging. In this study, the focus is on bone hemostasis and its dysregulation with aging.
