Objective: The phase III LION trial found no therapeutic benefit from systematic lymphadenectomy in patients with advanced ovarian cancer with optimal upfront cytoreduction and normal-appearing lymph nodes. Patients were randomized intra-operatively, excluding those who could not be operated on when they were tumor-free or had suspicious/bulky lymph nodes upon inspection or palpation. Since ovarian cancer typically spreads intraperitoneally or via lymphatics, a retroperitoneal duodenal obstruction is a rare presentation of ovarian cancer. Such upper gastrointestinal obstruction in a young patient is diagnostically challenging and surgically difficult to address. In this case report, we describe that in an interdisciplinary approach a Whipple pancreaticoduodenectomy could be safely implemented into the interval debulking surgery to achieve complete cytoreduction.

This paper provides an English summary of a German guideline on prioritization and resource allocation for colorectal and pancreatic cancer in the context of the pandemic.
