Background: Mycophenolate mofetil (MMF) is a mainstay for the treatment of systemic sclerosis (SSc). The occurrence and implications of MMF-related adverse events on drug retention rates in real life remain poorly defined. We aimed to determine the MMF retention rate and to investigate the causes and patterns of discontinuation, adverse events (AEs) and treatment options used after discontinuation.

Reducing levels of low-density lipoprotein cholesterol (LDL-C) below recommended thresholds is a core component of cardiovascular prevention strategies. We hypothesized that the addition of bempedoic acid to patients already on statin-ezetimibe therapy was more effective than titrating the statin dose in reducing LDL-C. The study enrolled 120 patients at high cardiovascular risk and with LDL-C above 70 mg/dL.

Cerebrovascular events (CE) are one of the most common and severe events in antiphospholipid syndrome (APS), a condition characterized by thrombosis and circulating anti-phospholipid antibodies (aPL). Seronegative APS (SN-APS) refers to a group of patients with clinical features of APS but persistently negative tests for "criteria aPL": anti-cardiolipin antibodies (aCL) and anti-β2glycoprotein I antibodies detected by enzyme-linked immunosorbent assay (ELISA), and the lupus anticoagulant detected by clotting assays. We report a series of five cases of SN-APS in young or middle-aged patients who tested positive for "non-criteria" aPL.

  • Atrial fibrillation (AF) in older patients is linked to cognitive decline and dementia, with ongoing debate about the benefits of anticoagulants on cognitive function.
  • This study involved 983 older patients comparing the cognitive decline between those on non-vitamin K oral anticoagulants (NOACs) and vitamin K antagonists (VKAs) using the Mini Mental State Examination (MMSE) score over an average of 7.2 years.
  • Results showed that patients on NOACs had a significantly lesser decline in cognitive function compared to VKAs, indicating that NOACs may be more beneficial for preserving cognitive health in older AF patients despite their higher age and comorbidities.

Chronic Thromboembolic Pulmonary Hypertension (CTEPH) presents a significant diagnostic challenge due to its complex and often nonspecific clinical manifestations. This review outlines a comprehensive approach to the diagnostic assessment of CTEPH, emphasizing the importance of a high index of suspicion in patients with unexplained dyspnea or persistent symptoms post-acute pulmonary embolism. We discuss the pivotal role of multimodal imaging, including echocardiography, ventilation/perfusion scans, CT pulmonary angiography, and magnetic resonance imaging, in the identification and confirmation of CTEPH.

  • Excessive heart rates in septic shock patients can negatively impact their health, prompting a study on the use of the ultra-short-acting beta-blocker landiolol to control heart rate without raising the need for vasopressors.
  • Conducted across 20 sites in Europe from 2018 to 2022, the study compared landiolol combined with standard treatment to standard treatment alone in adults with septic shock and high heart rates, focusing on maintaining a target heart rate (80-94 bpm) without increasing vasopressor requirements.
  • Results showed that a higher percentage of patients receiving landiolol achieved the target heart rate compared to those receiving standard treatment (39.8% vs. 23.5%), but there were no

  • Diagnosing solitary pink skin lesions is challenging due to the overlap of clinical features among both benign (like cherry angiomas) and malignant lesions (such as basal cell carcinoma).
  • Reflectance confocal microscopy (RCM) improves diagnostic accuracy beyond traditional dermoscopy, offering better sensitivity and specificity for identifying these lesions.
  • Despite advancements, a definitive diagnosis still requires surgical excision and histopathological analysis to ensure correct treatment.

Article Synopsis
  • The study aimed to understand physicians' preferences regarding diagnostic pathways and treatment priorities for systemic lupus erythematosus (SLE) through a discrete choice experiment (DCE).
  • A total of 95 clinicians, mainly rheumatologists, participated in the DCE, revealing that "referral time to a rheumatologist" was highly prioritized, especially in mild-moderate and severe SLE cases.
  • Results indicated a strong preference for treatments that prevent organ damage over other factors, with physician awareness about the critical need for timely diagnosis and prevention of damage in SLE management being consistent across different medical specialties.

Objectives: Starting from the unmet need of early diagnosis and treatment in systemic lupus erythematosus (SLE), the study aims to explore patient preferences in diagnostic pathways and treatment modalities. It seeks to integrate clinical priorities with patient perspectives, providing an optimal approach to SLE treatment that remains uncertain.

Methods: A discrete choice experiment (DCE) has been conducted to investigate whether patient preferences align while maintaining consistent attributes and levels, providing a direct assessment of relative preferences and hypothetical treatment approaches in SLE.

Renin angiotensin system inhibitors and outcome in patients with takotsubo syndrome: A propensity score analysis of the GEIST registry.

Am Heart J

December 2024

Institute of Cardiology, Madre Giuseppina Vannini Hospital, Rome, Italy; Department of Clinical Internal, Anesthesiological and Cardiovascular Sciences, Sapienza University, Rome, Italy.

Background: Few data are available on long-term drug therapy and its potential prognostic impact after Takotsubo syndrome (TTS). Aim of the study is to evaluate clinical characteristics and long-term outcome of TTS patients on Renin Angiotensin system inhibitors (RASi).

Methods: TTS patients were enrolled in the international multicenter GEIST (GErman Italian Spanish Takotsubo) registry.

Antiplatelet therapy is crucial for reducing thrombotic events in patients with atherosclerotic disease, but the response vary widely among individuals. The identification of patients at high (HPR), optimal (OPR) or low platelet reactivity (LPR) is dependent on high interlaboratory variability. We report results of a large dataset of patients to assess the gold standard light transmission aggregometry (LTA).

  • IgE-mediated food allergies involve a type 1 immune response triggered by certain foods, resulting in a variety of symptoms that can vary in severity, including severe reactions like anaphylactic shock.
  • This pilot study examined the effects of dupilumab, an antibody treatment approved for atopic dermatitis, on patients with both AD and food allergies, focusing on their allergic sensitization profiles.
  • Results indicated a significant reduction in specific IgE levels and molecular allergen components after 16 weeks of dupilumab treatment, suggesting potential benefits in managing allergic symptoms among patients with multiple sensitivities.

Dementia is an age-related syndrome characterized by the progressive deterioration of cognition and capacity for independent living. Diabetes is often associated with cognitive decline and shares similar pathophysiological mechanisms with dementia, such as systemic inflammation, oxidative stress, insulin resistance, and advanced glycation end-products formation. Therefore, adequate diabetes management may reduce the risk of cognitive decline, especially in patients with other comorbidities and risk factors.

Chronic Thromboembolic Pulmonary Hypertension (CTEPH) is a severe and complex condition that evolves from unresolved pulmonary embolism, leading to fibrotic obstruction of pulmonary arteries, pulmonary hypertension, and potential right heart failure. The cornerstone of CTEPH management lies in a multifaceted therapeutic approach tailored to individual patient profiles, reflecting the disease's heterogeneity. This review delves into the current therapeutic strategies for CTEPH, including surgical pulmonary endarterectomy (PEA), balloon pulmonary angioplasty (BPA), and targeted pharmacological treatments such as PDE5 inhibitors, endothelin receptor antagonists, sGC stimulators, and prostanoids.
