Department of Epidemiology, Fuwai Hospital, National Center for Cardiovascular Diseases, Chinese Academy of Medical Sciences and Peking Union Medical College, Beijing 100037, China;Key Laboratory of Cardiovascular Epidemiology, Chinese Academy of Medical Sciences, Beijing 100037, China.

Article Synopsis
  • Recent decades have seen China implementing policies aimed at reducing air pollution, specifically targeting health issues related to stroke caused by fine particulate matter (PM).
  • Analysis of data from the Global Burden of Disease study indicates that the rate of disability-adjusted life years (DALY) lost due to stroke from PM increased dramatically from 1990 to 2012, but showed a steady decrease from 2012 to 2019.
  • The research highlights that while rates for ischemic and intracerebral hemorrhage strokes initially rose, they have since declined, demonstrating a positive impact of air quality improvement efforts on public health in China.

Department of Radiology and Research Institute of Radiology, University of Ulsan College of Medicine, Asan Medical Center, Seoul, Republic of Korea.

Article Synopsis
  • The study aimed to investigate whether switching gadolinium-based contrast agents (GBCAs) can help reduce the recurrence of acute adverse drug reactions (ADRs) associated with their use during MRI exams.
  • From nearly 239,000 MRI exams between January 2016 and December 2021, 1,042 cases of acute ADRs were identified, with follow-up evaluations showing a recurrence rate of 31.9% among patients who experienced ADRs.
  • Results indicated that patients who switched to different GBCAs had a significantly lower recurrence of ADRs (14.3%) compared to those who remained on the same agent (36.9%), suggesting that changing

Background: Calcaneal lengthening osteotomy (CLO) is one of the main surgical options for treatment of pediatric idiopathic flexible flatfoot (FFF). Reportedly, calcaneocuboid (CC) joint subluxation occurs after CLO; however, its effect on the midfoot remains unclear. This study aimed to investigate the radiologic midterm results after CLO treatment in pediatric idiopathic FFF.

Background: Levodopa-induced dyskinesia (LID) is a common adverse effect of levodopa, one of the main therapeutics used to treat the motor symptoms of Parkinson's disease (PD). Previous evidence suggests a connection between LID and a disruption of the dopaminergic system as well as genes implicated in PD, including GBA1 and LRRK2.

Objectives: Our goal was to investigate the effects of genetic variants on risk and time to LID.

Background: A recent Lancet commission called for more research on palliative care in low- and middle-income (LMIC) countries such as Colombia. A research priority setting approach has been recommended by The Global Forum for Health Research to address the huge gap in research output between LMIC and high-income countries, with influential health service bodies recommending the active involvement of non-research expert stakeholders in establishing research priorities to address service user needs.

Method: Priority setting partnership (PSP) following the four stages of the James Lind Alliance methodology; establishing the partnership, identifying evidence uncertainties, refining questions and uncertainties, and prioritization.

Currently, interferon-gamma release assay (IGRA) is costly and not included as latent tuberculosis infection (LTBI) screening test strategy in Thailand's Universal Coverage Scheme (UCS) benefit package. The objective of this study was to assess the cost-utility of LTBI screening strategies among tuberculosis (TB) contacts in Thailand. A hybrid decision tree and Markov model was developed to compare the lifetime costs and health outcomes of tuberculin skin test (TST) and IGRA, in comparison to no screening, based on a societal perspective.

Article Synopsis
  • Researchers aimed to create an algorithm based on inflammation-related markers to improve the diagnostic accuracy for idiopathic inflammatory myopathies (IIMs) compared to traditional histopathologic methods.
  • The study analyzed muscle biopsy samples from 129 IIM patients and 73 controls, using a variety of immunostaining results to develop a diagnostic algorithm via classification and regression tree (CART) analysis.
  • The new algorithm using CD163 and MHC class I expression achieved a diagnostic accuracy of 95.5%, significantly outperforming traditional methods and correctly identifying 94.1% of IIM cases not meeting existing diagnostic criteria.

Article Synopsis
  • Cardiopulmonary resuscitation (CPR) is essential for cardiac arrest treatment, with extracorporeal CPR (ECPR) improving survival rates and neurological outcomes, studied here through the timing of ECMO initiation.
  • This retrospective analysis involved 198 patients who received ECPR, categorizing the time from CPR to ECMO initiation into three intervals: ≤20 min, 20-40 min, and >40 min, to assess its effect on mortality rates.
  • Results indicated that longer delays in ECMO initiation correlated with higher short- and long-term mortality rates, highlighting the importance of timely ECMO application for better patient outcomes in ECPR.

  • * Sixty adults with MASLD were divided into two groups: one used the mobile app for interventions, while the other received standard educational materials, with liver enzyme levels assessed before and after 4 weeks.
  • * Results showed significant decreases in liver enzymes (alanine aminotransferase and gamma-glutamyl transferase) in the intervention group, suggesting that mobile lifestyle interventions could help improve liver health without impacting body weight or waist circumference, though more extensive studies are needed for confirmation.

  • Postoperative cognitive dysfunction (POCD) is linked to cognitive decline after major surgery, affecting outcomes like physical function, length of hospital stay, dementia, and mortality.
  • A review of 20 studies found that patients with POCD following cardiac surgery had more than double the risk of death, while those with POCD after noncardiac surgery also faced increased mortality risks.
  • Patients with POCD experienced longer hospital stays compared to those without, emphasizing the need for early identification and intervention in at-risk surgical patients, though the role of postoperative delirium (POD) complicates these findings.

Background: In breast reconstruction, arterial coupling has been reported to be more favorable in the thoracodorsal artery (TDA) than the internal mammary artery (IMA). This technique may help overcome anastomosis in a small, deep space. Understanding the arteries' mechanical properties is crucial for breast reconstruction's safety and success.

Background: The incidence of comorbidities is higher in HIV-positive patients than in the general population due to factors, such as HIV-related chronic inflammation. There is no consensus on whether a low CD4 lymphocyte count after virological suppression at long-term follow-up increases the risk of comorbidities. This study evaluates the association between CD4 lymphocyte count and the incidence of comorbidities during the first 5 years of virological suppression after highly active antiretroviral treatment.

Article Synopsis
  • The optimal management of patients with atrial fibrillation (AF) requiring oral anticoagulation (OAC) during percutaneous coronary intervention (PCI) is debated, with current guidelines suggesting a temporary triple therapy regimen that includes aspirin, which raises bleeding risks.
  • A new study, the WOEST-3 trial, seeks to compare a 30-day dual antiplatelet therapy (DAPT) strategy that temporarily omits OAC against guideline-directed therapy post-PCI for AF patients, aiming to reduce bleeding without sacrificing efficacy.
  • With a sample of 2,000 patients, the trial will evaluate the rates of major bleeding and adverse ischemic events, making it the first randomized controlled trial to explore the omission of OAC
