Article Synopsis
  • - The study focused on the effects of mammographic breast density reduction (MDR) on treatment outcomes in premenopausal women with estrogen receptor-positive breast cancer who received either tamoxifen alone or in combination with ovarian function suppression (OFS).
  • - Results from the ASTRRA trial showed that while the rate of MDR positivity was similar in both treatment groups, patients receiving tamoxifen plus OFS who were MDR-positive experienced significantly better disease-free survival compared to those who were MDR-negative.
  • - The findings highlight the potential importance of MDR as a positive prognostic factor specifically in patients treated with tamoxifen and OFS, suggesting a need for tailored treatment approaches based on MDR status.

Purpose: Fertility preservation (FP) is an important issue for young survivors of breast cancer. Although international guidelines recommend pre-treatment fertility counseling for women with breast cancer, there is no standardized protocol or referral system for FP in South Korea. There are also barriers to discussing FP that make patient-centered decision making difficult.

Article Synopsis
  • The study aimed to identify the risk factors for chronic obstructive pulmonary disease (COPD) in both a general population and referred COPD patients, utilizing data from two distinct datasets.
  • Findings showed that the mean age of COPD patients was 67 years, with notable differences in gender distribution and forced expiratory volume between participants from the general population and referral clinics.
  • Key risk factors identified included cigarette smoking, infections, pollution, and asthma, with significant variations in prevalence between the datasets, highlighting a need to investigate unidentified risk factors in the general population.

J Crit Care

February 2024

Department of Pulmonary and Critical Care Medicine, Asan Medical Center, University of Ulsan College of Medicine, Seoul, South Korea. Electronic address:

Article Synopsis
  • The study examined current mechanical ventilation practices in Asian intensive care units, specifically focusing on tidal volume, plateau pressure, and PEEP in patients.
  • Data from 1408 patients were analyzed, highlighting that while low tidal volume was utilized in patients with ARDS, a large proportion of those without ARDS received intermediate tidal volumes, with adequate plateau pressure noted in most cases.
  • The research found that factors like income classification, patient age, severity of illness, and certain clinical measures significantly influenced mortality rates, which escalated with higher predictive scores for mortality.

Background: Timely reperfusion therapy is recommended for patients with ST-segment elevation myocardial infarction (STEMI), and system delay <90 minutes and door-to-device (D2D) time <60 minutes are recommended by the 2017 ESC Guidelines for the management of STEMI patients and have been proposed as a performance measure for triaging patients for primary percutaneous coronary intervention (PCI). However, previous research produced contradictory results regarding the association between D2D time and mortality. Therefore, this study aimed to examine the associations between D2D time and mortality in Thailand.
