An atypical case of atrioventricular block in an elderly man: A fortuitous escape.

Paroxysmal atrioventricular block (PAVB) is an important but under-recognized etiology of syncope. It is characterised by sudden and unexpected change from 1:1 atrioventricular conduction to complete atrioventricular block. This poorly defined entity is usually associated with absence or delayed emergence of an adequate escape rhythm.

Wide-complex tachycardia in a patient with old myocardial infarction.

An elderly man with a history of coronary artery disease presented with sudden-onset palpitations. The electrocardiogram showed wide complex tachycardia with left bundle branch morphology which terminated spontaneously. Subsequent ECGs confirmed the diagnosis as atrial fibrillation with aberrant conduction because of a diseased left bundle.

Background: Inflammation has been implicated in the initiation and perpetuation of non-valvular atrial fibrillation (AF). However, there is a lack of similar data on AF in rheumatic heart disease (RHD).
