Background: Cytomegalovirus (CMV) infection remains among the leading complications after solid organ transplantation (SOT). Large international surveys mainly focused on high-income countries, detected considerable variability in the management of this infection after SOT. Limited data are available from resource-limited settings.

Acute Hepatitis Related to Hepatitis E Virus Genotype 3f Infection in Brazil.

J Med Virol

November 2024

Laboratório de Gastroenterologia e Hepatologia Tropical, LIM-07, Instituto de Medicina Tropical, Faculdade de Medicina FMUSP, Universidade de Sao Paulo, Sao Paulo, Brazil.

Article Synopsis
  • * In group 1 (44 patients with acute hepatitis A), 4% tested positive for HEV IgM and 14.5% for IgG, while in group 2 (47 patients with non-A-C AH), these rates were 4.3% for IgM and 14.9% for IgG; only one sample from group 2 tested positive for HEV RNA.
  • * The findings suggest HEV should be considered in non-A-C acute hepatitis cases, particularly as one patient with HEV recently traveled to a high-serop

Background Aims: Cut-points for non-invasive tests (NITs) for risk stratification in metabolic dysfunction-associated steatotic liver disease (MASLD) were derived from predominantly non-Hispanic populations. It is unknown if these cut-points perform adequately in Hispanic individuals. We assessed the performance characteristics of current NIT cut-points among Hispanic patients and determined whether they could be further optimized.

The Effect of Ascorbic Acid on Hepatic Ischaemia-Reperfusion Injury in Wistar Rats: An Experimental Study.

Int J Mol Sci

August 2024

Laboratório de Investigação Medica 37, Departamento de Gastroenterologia, Hospital das Clínicas HCFMUSP, Faculdade de Medicina, Universidade de São Paulo, São Paulo 05508-220, SP, Brazil.

Liver ischaemia-reperfusion (IR) during hepatic surgeries can lead to liver cell death via oxidative stress and the activation of immune cells, the release of cytokines, and damage-associated molecular patterns. Ascorbic acid has been shown to confer potential protective effects against IR injury, mainly due to its antioxidant properties. This study evaluated the effect of ascorbic acid infusion at different time points during hepatic IR in rats.

Purpose: Understanding patients' motives for undergoing metabolic and bariatric surgery (MBS) is key to managing postoperative expectations. We aimed to translate and validate the 14-item European Obesity Academy Questionnaire on Expectations about Surgical Treatment (EOAQ-EST) to Brazilian Portuguese for research and clinical use.

Materials And Methods: This study included a total of 198 candidates for MBS at a reference academic hospital in Brazil from January 2021 to February 2022.

Purpose: To evaluate glycemic variability (GV) using continuous glucose monitoring (CGM) in individuals with and without type 2 diabetes mellitus (T2DM) undergoing Roux-en-Y gastric bypass (RYGB).

Methods: This prospective cohort study compared the CGM data of fourteen patients with T2DM (n = 7) and without T2DM (n = 7) undergoing RYGB. After 6 months, these patients were compared to a non-operative control group (n = 7) matched by BMI, sex, and age to the T2DM group.

Mitochondrial dysfunction is a recognized feature of sepsis, characterized by ultrastructural damage, diminished oxidative phosphorylation, and depletion of mitochondrial antioxidant capacity observed in deceased septic patients. LPS tolerance induces a controlled response to sepsis. This study aimed to evaluate the function of tolerant mitochondria after cecal ligation and puncture (CLP)-induced sepsis.

Article Synopsis
  • - The study investigates the relationship between Religiosity/Spirituality (R/S) and Quality of Life (QoL) among 151 Crohn's disease patients at a medical facility in Brazil, using various questionnaires to gather data.
  • - Results indicated that the majority of participants had high levels of religious and spiritual engagement, but this did not correlate with improved QoL or differences in disease phenotypes.
  • - The authors conclude that while R/S is prevalent among patients, it may serve more as a tool for treatment adherence rather than impacting quality of life directly, highlighting the need for diverse participant samples for future evaluations.

Noninvasive assessment of liver fibrosis can predict clinical outcomes at late follow-up after a sustained virological response in HCV patients?

Clinics (Sao Paulo)

May 2024

Divisão de Gastroenterologia e Hepatologia Clínica, Departamento de Gastroenterologia, Hospital das Clínicas, Faculdade de Medicina, Universidade de São Paulo ((HCFMUSP), São Paulo, SP, Brazil. Electronic address:

Objectives: The primary objective was to evaluate Liver-Related Events (LREs), including hepatic decompensation (ascites, hemorrhagic varices and encephalopathy) and Hepatocellular Carcinoma (HCC), as well as changes in liver stiffness during the follow-up period among patients who achieved a Sustained Virological Response (SVR) after treatment for chronic Hepatitis C Virus (HCV) infection.

Methods: A total of 218 patients with HCV were treated, and those who achieved an SVR were followed up for 3-years. Transient Elastography (TE) using FibroScan® was performed at various time points: before treatment, at the end of treatment, at 6-months post-treatment, at 1-year post-treatment, at 2-years post-treatment, and at 3-years post-treatment.

Is There a Safe Alcohol Consumption Limit for the General Population and in Patients with Liver Disease?

Semin Liver Dis

February 2024

Liver Unit, Institut d'Investigacions Biomèdiques August Pi i Sunyer (IDIBAPS), Hospital Clinic, Barcelona, Spain.

Article Synopsis
  • Excessive alcohol consumption is a significant global health issue, linked to liver disease and cancer-related deaths, often underestimated due to reliance on self-reported data.
  • The Alcohol Use Disorders Identification Test - version C is commonly used for screening, but objective assessment through alcohol biomarkers is recommended for patients with liver disease.
  • It is advised that all patients with alcohol-associated liver disease abstain from alcohol, as this can slow or reverse liver damage, while studies suggest that even small amounts of alcohol increase health risks, particularly in those with chronic liver conditions.

Machine Learning-Based Prediction of Responsiveness to Neoadjuvant Chemoradiotheapy in Locally Advanced Rectal Cancer Patients from Endomicroscopy.

Crit Rev Oncog

March 2024

Departamento de Radiologia e Oncologia, Instituto do Cancer do Estado de Sao Paulo, Hospital das Clinicas HCFMUSP, Faculdade de Medicina, Universidade de Sao Paulo, Sao Paulo 01246-000, SP, Brazil; Escola de Artes, Ciencias e Humanidades, Universidade de Sao Paulo, Sao Paulo 03828-000, SP, Brazil.

The protocol for treating locally advanced rectal cancer consists of the application of chemoradiotherapy (neoCRT) followed by surgical intervention. One issue for clinical oncologists is predicting the efficacy of neoCRT in order to adjust the dosage and avoid treatment toxicity in cases when surgery should be conducted promptly. Biomarkers may be used for this purpose along with in vivo cell-level images of the colorectal mucosa obtained by probe-based confocal laser endomicroscopy (pCLE) during colonoscopy.

Background and aim Endoscopic ultrasound (EUS)-guided drainage is the gold standard approach for the treatment of encapsulated pancreatic collections (EPCs) including pseudocyst and walled-off pancreatic necrosis (WON), and is associated with an equivalent clinical efficacy to surgical drainage with fewer complications and less morbidity. Drainage may be achieved via several types of stents including a fully covered self-expandable metallic stent (SEMS) and lumen-apposing metal stent (LAMS). However, to date there have been no randomized trials to compare these devices.
