Peduncular hallucinosis is characterized by vivid hallucinations associated with organic midbrain disease. In the case reported here, the computed tomography brain scan showed basilar arterial atherosclerotic disease with central and cortical atrophy. Carotid imaging revealed bilateral stenosis.
