How to Manage a Neonate Born from a SARS-CoV-2-Positive Mother: A Narrative Review.

Pathogens

November 2024

Neonatal and Pediatric Unit, Polo Ospedaliero Oltrepò, ASST Pavia, 27058 Voghera, Italy.

From 2020 to today, considerable knowledge on SARS-CoV-2 has been collected, even on pregnant women and their fetuses and newborns, and clinical guidelines have been written and implemented worldwide. Vaccination has considerably improved outcomes, but hesitancy amongst pregnant patients and the emergence of variants remain challenging, and SARS-CoV-2 positivity during pregnancy continues to be associated with an increased risk of maternal complications, premature delivery, and higher neonatal mortality and morbidity. A body of data now exists on the effect of SARS-CoV-2 during pregnancy on early neonatal outcomes, medical education in obstetrics and pediatrics, and longer-term developmental outcomes.

Eur J Obstet Gynecol Reprod Biol

December 2024

Department of Obstetrics and Gynecology, Del Ponte Hospital, Varese, Italy; Department of Medicine and Surgery, University of Insubria, Varese, Italy.

During fetal life the right valve of the sinus venosus directs oxygenated blood from the inferior vena cava to the left atrium through the foramen ovale, until the regression of the right valve which usually occurs within the 15th week of pregnancy. Incomplete regression of the right venous valve in varying degrees can lead to different types of remnants such as the Eustachian valve, the Chiari Network and Cor Triatriatum Dexter. Prenatal diagnosis of remnants of right venous valve has only rarely been reported in the literature.

: Mixed urinary incontinence (MUI) has always represented a major therapeutic challenge and the management of this type of incontinence is often complicated by uncertain outcomes. Surgical options include interventions targeting both stress urinary incontinence (SUI) and urge urinary incontinence (UUI), although there are no international published guidelines that dictate whether it is better to start with surgical management to address the SUI or UUI component after the failure of conservative treatment. The aim of the present study is to evaluate the effectiveness of the Macroplastique (MPQ) procedure on overactive bladder (OAB) symptoms in women with MUI with a minimum follow-up of 1 year.

The use of tension-free vaginal tape obturator (TVT-O) for the treatment of stress urinary incontinence (SUI) has been widely debated over the last decade due to the lack of evidence on its long-term outcomes. The aim of this prospective study is to assess, for the first time in the available literature, the efficacy and safety of TVT-O implantation in women with pure SUI over a 17-year follow-up period. : We included all women who complained of pure SUI symptoms (confirmed urodynamically) and underwent the TVT-O procedure.

Article Synopsis
  • - The study investigates the link between congenital lung malformations (CLMs) and the risk of developing cancer by examining genetic mutations in various lung malformations through whole-genome sequencing.
  • - Analysis revealed that 95% of the CLMs exhibited genomic instability, with 30% harboring genetic variants associated with tumor development, suggesting a potential predisposition to malignancy.
  • - The findings indicate that while CLMs are often benign, their genetic alterations, combined with environmental carcinogenic factors, may increase the likelihood of lung cancer, highlighting a significant biological connection.

September 2024

Unit of Obstetrics and Gynecology, "Paolo Giaccone" Hospital, 90127, Palermo, Italy; Department of Health Promotion, Mother and Child Care, Internal Medicine and Medical Specialties (PROMISE), University of Palermo, 90127, Palermo, Italy. Electronic address:

Article Synopsis
  • The management of infertility in women with deep endometriosis (DE) is contentious, with insufficient evidence regarding the link between infertility and the severity of DE as categorized by the rASRM stages.* -
  • A systematic review encompassing 16 studies found that out of 947 infertile women who underwent surgery, 486 achieved pregnancy, indicating an average pregnancy rate of 51.3% and suggesting surgery may enhance reproductive outcomes, especially when coupled with assisted reproductive technology (ART).* -
  • However, due to the varied data and lack of clarity on surgical outcomes relative to the location of DE, stronger conclusions cannot be drawn, highlighting the need for further research to optimize surgical strategies in relation to ART treatments.*

International consensus on sleep problems in pediatric palliative care: Paving the way.

Sleep Med

July 2024

Pediatric Palliative Care, Pain Service, Department of Women's and Children's Health, University Hospital of Padua, Padua, Italy.

Objective: Sleep problems constitute a common and heterogeneous complaint in pediatric palliative care (PPC), where they often contribute to disease morbidity and cause additional distress to children and adolescents and their families already facing the burden of life-threatening and life-limiting conditions. Despite the significant impact of sleep problems, clinical evidence is lacking. The application of general pediatric sleep recommendations appears insufficient to address the unique challenges of the PPC dimension in terms of disease variability, duration, comorbidities, complexity of needs, and particular features of sleep problems related to hospice care.

Article Synopsis
  • The study investigated the frequency and features of gastroesophageal reflux (GER) in infants who cry persistently without an obvious cause, focusing on those aged 0-12 months.
  • Out of 50 infants studied, 60% had normal conditions according to MII-pH monitoring, despite 66% of them showing abnormal scores on a reflux questionnaire.
  • The findings suggest that nocturnal crying in infants should not routinely lead to the use of acid suppressants, as abnormal MII-pH results were not reliably predicted by symptoms or regurgitation episodes.
