Machine learning-based prediction of adverse events following an acute coronary syndrome (PRAISE): a modelling study of pooled datasets.

Lancet

January 2021

Division of Cardiology, Cardiovascular and Thoracic Department, Città della Salute e della Scienza, Turin, Italy; Cardiology, Department of Medical Sciences, University of Turin, Turin, Italy.

Background: The accuracy of current prediction tools for ischaemic and bleeding events after an acute coronary syndrome (ACS) remains insufficient for individualised patient management strategies. We developed a machine learning-based risk stratification model to predict all-cause death, recurrent acute myocardial infarction, and major bleeding after ACS.

Methods: Different machine learning models for the prediction of 1-year post-discharge all-cause death, myocardial infarction, and major bleeding (defined as Bleeding Academic Research Consortium type 3 or 5) were trained on a cohort of 19 826 adult patients with ACS (split into a training cohort [80%] and internal validation cohort [20%]) from the BleeMACS and RENAMI registries, which included patients across several continents.

Int J Environ Res Public Health

November 2020

Department of Maternal, Infantile and Urological Sciences, Sapienza University of Rome, 00161 Rome, Italy.

Article Synopsis
  • From March 9 to May 3, 2020, Italy implemented a lockdown due to COVID-19, significantly impacting pediatric emergency room (ER) visits.
  • During the lockdown, ER admissions dropped by 81% compared to the same period in 2019, with air communicable diseases showing the largest decrease.
  • While critical cases (red codes) remained stable, less severe cases (yellow codes) increased, possibly indicating delays in primary care that led to more urgent ER visits.

Background: The role of planned angiographic control (PAC) over a conservative management driven by symptoms and ischaemia following percutaneous coronary intervention (PCI) of the unprotected left main (ULM) with second-generation drug-eluting stents remains controversial. PAC may timely detect intrastent restenosis, but it is still unclear if this translated into improved prognosis.

Methods And Analysis: PULSE is a prospective, multicentre, open-label, randomised controlled trial.

Introduction: Oligometastatic disease has emerged as an intermediate state between localized and systemic cancer. Improvements in diagnostic modalities such as functional imaging allow a greater frequency of oligometastases diagnosis. Patients with selected oligometastatic epithelial ovarian carcinoma (EOC) may be treated with metastasis-directed stereotactic body radiotherapy (SBRT) rather than chemotherapy.

Background: Cancer trials involving multiple treatment lines substantially increase our understanding of therapeutic strategies. However, even when the primary end-point of these studies is progression-free survival (PFS), their statistical analysis usually focuses on each line separately, or does not consider repeated events, thus missing potentially relevant information. Consequently, the evaluation of the effectiveness of treatment strategies is highly impaired.

Article Synopsis
  • - This study examines how the prognostic nutritional index (PNI) affects survival rates in patients with advanced hepatocellular carcinoma (HCC) receiving sorafenib treatment.
  • - Researchers analyzed data from a total of 664 HCC patients across multiple cohorts and found that a PNI cut-off value of 31.3 significantly correlated with median overall survival (OS), showing better outcomes for patients with higher PNI levels.
  • - The findings suggest that PNI is a simple, cost-effective prognostic tool that could be used in clinical settings to help predict survival in patients with advanced HCC.

The application of non-targeted serum metabolomics profiling represents a noninvasive tool to identify new clinical biomarkers and to provide early diagnostic differentiation, and insight into the pathological mechanisms underlying hepatocellular carcinoma (HCC) progression. In this study, we used proton Nuclear Magnetic Resonance (H-NMR) Spectroscopy and multivariate data analysis to profile the serum metabolome of 64 HCC patients, in early (n = 28) and advanced (n = 36) disease stages. We found that H-NMR metabolomics profiling could discriminate early from advanced HCC patients with a cross-validated accuracy close to 100%.
