Borges and colleagues' article entitled "More Pain, More Gain! The Delivery of COVID-19 Vaccines and the Pharmaceutical Industry's Role in Widening the Access Gap," analyzes the role of pharmaceutical companies in providing equitable access to COVID-19 vaccines. They concluded that with the failure of COVID-19 Vaccine Global Access (COVAX), the health gaps have widened due to the profit-driven pharmaceutical sector. In this commentary, we highlight the role of COVAX and its attempt to bridge some access gaps since its inception and the need for reforms in policy-making and global health governance.

Health system resilience refers to the capacity of a health system to effectively anticipate, assimilate, adjust to and recuperate from unforeseen disruptions and pressures. Evidence indicates that low- and middle-income countries (LMICs) have a double burden of dealing with the existing shortage of health resources in managing both non-emergency care and emergency care during epidemics. Intersectoral collaboration plays a pivotal role in managing crises such as pandemics.

An effective disease surveillance system can detect outbreaks of emerging zoonotic diseases early and allow the system to respond immediately and control the epidemic. Gujarat is a western Indian state with a population of more than 65 million humans and 26 million livestock, and it has the respective surveillance systems under the Department of Health & Family Welfare for humans and the Department of Animal Husbandry for animals. A veterinarian is placed on establishing cross-sectoral collaborations between these systems, but the joint collaborative activities and their effect on the early warning response are least understood.

Even though it is rare, herpes simplex virus (HSV) esophagitis has a significant adverse impact on immunocompromised people, such as those with HIV, cancer patients receiving chemotherapy or radiation to the neck, and recipients of transplants receiving immunosuppressive treatments. This makes a high level of clinical suspicion necessary for a precise diagnosis and successful treatment. Although rare, its occurrence in immunocompetent patients poses unique challenges for diagnosis and therapy.
