Purpose: The 5-item modified frailty index (mFI-5) has been established as a reliable indicator of poor postoperative outcomes following a variety of orthopaedic procedures. This study aims to determine whether the mFI-5 can be used by surgeons to predict the likelihood of postoperative complications in patients undergoing open reduction internal fixation (ORIF) for tibial plateau fractures.

Methods: From 2006 to 2019, patients aged 50 years or older undergoing ORIF for tibial plateau fracture were identified in the National Surgical Quality Improvement Program database.

Bloodstream infections caused by the opportunistic pathogen Klebsiella pneumoniae are associated with adverse health complications and high mortality rates. Antimicrobial resistance (AMR) limits available treatment options, thus exacerbating its public health and clinical burden. Here, we aim to elucidate the population structure of K.

Expert Panel Perspective: Shared Decision-Making in Multidisciplinary Team-Based Cardiovascular Care.

JACC Adv

July 2024

Preventive Cardiology, Barbra Streisand Women's Heart Center, Preventive and Cardiac Rehabilitation Center, Women's Cardiovascular Medicine & Research, Smidt Heart Institute, Cedars Sinai Medical Center, Los Angeles, California, USA.

Shared decision-making (SDM) and multidisciplinary team-based care delivery are recommended across several cardiology clinical practice guidelines. However, evidence for benefit and guidance on implementation are limited. Informed consent, the use of patient decision aids, or the documentation of these elements for governmental or societal agencies may be conflated as SDM.

October 2024

Department of Medicine, Division of Gastroenterology, Western University, London, Ontario, Canada.

Article Synopsis
  • The study aimed to evaluate various instruments for assessing bowel preparation (BP) quality in patients with Crohn's disease, a condition where BP performance is not well understood.!
  • Five different scales were used to analyze BP quality in videos from 40 patients, focusing on reliability and validity through statistical methods.!
  • Results showed that most instruments demonstrated substantial reliability, with a negative correlation between BP quality and disease activity scores, indicating these tools are useful in clinical practice and research for Crohn's disease patients.!

Department of Pathology and Laboratory Medicine, Dartmouth-Hitchcock Medical Center, One Medical Center Drive, Lebanon, NH 03756, USA; Cedars-Sinai Medical Center, 8700 Beverly Boulevard, Los Angeles, CA 90048, USA.

Article Synopsis
  • - Recent advancements in cytopathology involve developing consensus rules for diagnosing specimens, improving accuracy and consistency in diagnoses.
  • - These diagnostic systems aim to minimize variability among observers, eliminate vague categories, reduce "atypical" diagnoses, and standardize communication through quantitative scoring.
  • - Computational pathology emerges from these improvements, offering standardized and reproducible diagnoses using quantitative methods, thereby reducing human bias.

Article Synopsis
  • Thyroid cytology focuses on diagnosing thyroid nodules, distinguishing between benign and malignant growths, and assessing risk when a clear diagnosis can't be made.
  • Fine-needle aspiration and standardized reporting systems like the Bethesda System streamline the diagnosis process for these nodules.
  • Recent advancements in molecular testing have become important for categorizing patients' risks of malignancy, aiding in the decision-making for treatment.

Tumors with pathogenic DICER1 mutation are rare and encompass sporadic or hereditary benign, intermediate and malignant tumors. DICER1-associated sarcomas are heterogeneous; however, the prototypical ones in the GYN-tract include embryonal rhabdomyosarcoma, adenosarcoma and moderately to poorly differentiated Sertoli-Leydig tumor. In this report, we present three unique uterine sarcomas with DICER1 mutation and remarkable diffuse round/spindle cell morphology.

Barriers to Engagement with Gastrointestinal Health Management.

Dig Dis Sci

October 2024

Center for Digestive Health, Dartmouth-Hitchcock Medical Center, Medical Center Drive, Lebanon, NH, 03756, USA.

Background: Although behavioral interventions have been effective in gastrointestinal (GI) conditions, barriers exist in implementing these interventions into clinical practice. The majority of previously published studies have focused on workforce limitations and have not considered individual and social determinants of health (SDoH) factors that can impact engagement in GI behavioral healthcare.

Aims: To characterize barriers to engagement in appointment attendance and health management, explore individual and SDoH factors impacting GI behavioral healthcare engagement, and identify barriers that occur more often for patients with SDoH-related vulnerability (low health literacy and/or financial insecurity).

Background: To help navigate the complex treatment landscape of ulcerative colitis (UC), we quantified the benefit-risk trade-offs that patients were willing to make when choosing treatment.

Methods: Patients completed an online discrete choice experiment. Eligible patients had a UC diagnosis for ≥6 months, were aged ≥18 years, and resided in France, Germany, Italy, Spain, or the UK.

Pleural effusion is the most common manifestation of pleural disease, and chest ultrasound is crucial for diagnostic workup and post-treatment monitoring. Ultrasound helps distinguish the various types of pleural effusion and enables the detection of typical manifestations of empyema, which presents as a complicated, septated effusion. This may benefit from drainage and the use of intrapleural enzyme therapy or may require more invasive approaches, such as medical or surgical thoracoscopy.

A Comprehensive Survey of 2024 Funding for Radiation Oncology Visiting Medical Student Electives.

J Cancer Educ

August 2024

Department of Radiation Oncology and Applied Sciences, Dartmouth-Hitchcock Medical Center, Geisel School of Medicine at Dartmouth, Lebanon, NH, USA.

Article Synopsis
  • Visiting electives in radiation oncology (RO) can impact medical students' residency match outcomes, but high costs deter some, particularly underrepresented students, from participating.
  • A review identified 92 RO electives, with 40 offering scholarships for underrepresented in medicine (URiM) students; the median stipend was $2000, but financial support is not evenly distributed across the U.S.
  • The geographical disparities in scholarship availability suggest a need for interventions to improve access to these valuable training opportunities for all medical students.

The implementation of screening colonoscopy with polyp removal has significantly decreased mortality rates associated with colorectal cancer (CRC), although it remains a major cause of cancer-related deaths globally. CRC typically originates from adenomatous polyps, and increased removal of these growths has led to reduced CRC incidence and mortality. Endoscopic polypectomy techniques, including hot and cold snare polypectomy, play a pivotal role in this process.

Background And Objectives: Children with medical complexity (CMC) may be at a high risk of neurodevelopmental and mental health conditions given disease comorbidities and lived experiences. Little is known about the prevalence of these conditions at a population level. In this study, we estimated the prevalence of neurodevelopmental and mental health diagnoses in CMC relative to children without medical complexity and measured associations between these diagnoses in CMC and subsequent health care utilization and in-hospital mortality.
