Introduction: Regional anesthesia, specifically fascia iliaca compartment blocks (FICB), is highly effective in managing pain, especially in military settings. However, a significant barrier to its implementation is the lack of provider confidence in performing ultrasound-guided procedures. This study evaluates the ability of physician assistant (PA) students, who are often first-line providers in austere locations, to identify the fascia iliaca compartment (FIC) using point-of-care ultrasound (POCUS) after a brief training session and assesses their retention of this skill over a 60- to 90-day period.

Article Synopsis
  • Oral commissure stenosis and lower eyelid ectropion from burns are difficult to treat due to a lack of preclinical models; this study describes a method for inducing controlled burns in swine to aid future research.
  • The experiment involved inducing burns of various durations (10 to 30 seconds) on swine and observing changes over time using techniques like laser speckle imaging for assessing tissue health and healing.
  • Results indicated that longer burn times led to more severe tissue damage and inflammation, but no significant functional impairments were noted, suggesting the need for further exploration of treatments with this new burn model.

Article Synopsis
  • The study examined 595 SARS-CoV-2 sequences from Military Health System beneficiaries to assess how vaccination affected viral diversity from December 2020 to April 2022.
  • Results indicated that vaccination had limited influence on the diversity of the SARS-CoV-2 spike protein and showed little evidence of a significant sieve effect among major variants.
  • The findings suggest that during periods of rapid variant replacement, other factors overshadowed the influence of vaccination on viral diversity, and caution is needed when using sieve analysis methods in non-clinical trial settings.

Introduction: Airway compromise is the third leading cause of preventable death on the battlefield. Most combat medics carry supraglottic airway (SGA) devices for airway management. However, exchanging an SGA device for a definitive airway can be challenging, especially in austere environments.

Article Synopsis
  • The use of whole blood for medical emergencies before patients reach the hospital is on the rise.
  • Existing methods for delivering blood, like intraosseous and peripheral venous catheters, can slow down the flow of these critical fluids during resuscitation.
  • Central venous catheters offer a more effective option in prehospital settings, allowing for faster and larger volumes of blood infusion that can save lives.

The aim of this study was to examine effects of the 2022 infant formula shortage as experienced by neonatal intensive care units (NICUs) in the United States, a previously unreported perspective. A mixed-method approach was utilized. Data were collected using an online survey of NICU medical directors.

Article Synopsis
  • This review analyzes the definitions and management strategies for normal and abnormal amniotic fluid volumes (AFVs), drawing from various studies and organizational guidelines.
  • A timeline was created to track the evolution of AFV thresholds, revealing substantial differences in definitions and management recommendations from 13 different organizations.
  • The review emphasizes the need for clarity in managing abnormalities like oligohydramnios and polyhydramnios, proposing algorithms to improve care in the meantime while identifying gaps for future research.

Introduction: Difficulty controlling anger is a common postdeployment problem in military personnel. Chronic and unregulated anger can lead to inappropriate aggression and is associated with behavioral health, legal, employment, and relationship problems for military service members. Military-related betrayal (e.

The potential adverse health effects of antiperspirant use are of interest to patients, primary care providers, dermatologists, and pathologists. In rare instances, antiperspirants containing aluminum-zirconium complexes have been associated with granulomatous dermatoses despite being deemed non-sensitizing in experiments. In this case study, we present a detailed examination of an axillary granuloma in a 28-year-old female who had been using an aluminum-zirconium-based antiperspirant for several years and presented with a left axillary nodule that was excised and analyzed using scanning electron microscopy with energy-dispersive x-ray analysis (SEM/EDXA).

Background: Post-COVID conditions (PCC) are difficult to characterize, diagnose, predict, and treat due to overlapping symptoms and poorly understood pathology. Identifying inflammatory profiles may improve clinical prognostication and trial endpoints.

Symptom accommodation by family members (FMs) of individuals with posttraumatic stress disorder (PTSD) includes FMs' participation in patients' avoidance/safety behaviours and constraining self-expression to minimise conflict, potentially maintaining patients' symptoms. The Significant Others' Responses to Trauma Scale (SORTS) is the only existing measure of accommodation in PTSD but has not been rigorously psychometrically tested. We aimed to conduct further psychometric analyses to determine the factor structure and overall performance of the SORTS.

Background: Chronic neuropsychological sequelae following SARS-CoV-2 infection, including depression, anxiety, fatigue, and general cognitive difficulties, are a major public health concern. Given the potential impact of long-term neuropsychological impairment, it is important to characterize the frequency and predictors of this post-infection phenotype.

Methods: The Epidemiology, Immunology, and Clinical Characteristics of Emerging Infectious Diseases with Pandemic Potential (EPICC) study is a longitudinal study assessing the impact of SARS-CoV-2 infection in U.

Military personnel and veterans are at heightened risk for exposure to traumatic events and posttraumatic stress disorder (PTSD), as well as intimate relationship problems associated with PTSD. The purpose of this study was to evaluate the relative efficacy of CBCT and PE in improving intimate relationship functioning in active duty military personnel or veterans and their intimate partners; both conditions were hypothesized to significantly improve PTSD. Method: In this study, 32 military service members or veterans with PTSD and their intimate partners were randomized to receive either Cognitive-Behavioral Conjoint Therapy for PTSD ( = 15; CBCT; [Monson, C.

Background: Static balance is often impaired in patients after ankle sprains. The ability to identify static balance impairments is dependent on an effective balance assessment tool. The Sway Balance Mobile Application (SWAY App) (Sway Medical, Tulsa, OK) uses a smart phone or tablet to assess postural sway during a modified Balance Error Scoring System (mBESS) assessment and shows promise as an accessible method to quantify changes in static balance after injury.

Background: Considered the second largest and most diverse microbiome after the gut, the human oral ecosystem is complex with diverse and niche-specific microorganisms. Although evidence is growing for the importance of oral microbiome in supporting a healthy immune system and preventing local and systemic infections, the influence of craniomaxillofacial (CMF) trauma and routine reconstructive surgical treatments on community structure and function of oral resident microbes remains unknown. CMF injuries affect a large number of people, needing extensive rehabilitation with lasting morbidity and loss of human productivity.

Objective: Overtriage (ie, delivering less severely injured patients via helicopter) is costly, raises safety concerns, and reduces efficiency of the trauma system. The Air Medical Prehospital Triage (AMPT) scoring system was developed to determine which trauma patients would gain a survival benefit by air transport. The objective of this study was to evaluate the AMPT scoring system as a method of reducing trauma overtriage when helicopter emergency medical services were used.

Background And Objectives: The Accreditation Council for Graduate Medical Education (ACGME) requires education on health care disparities (HCD), but research assessing formal curricula is limited. To improve knowledge and confidence in HCD, the family medicine residency program at Darnall Army Medical Center implemented a formal HCD curriculum.

Methods: During the 2021-2022 academic year, starting July 2021, a formal HCD curriculum was implemented for family medicine residents and faculty.
