Our understanding of the underlying psychological processes of development, maintenance, and treatments for stress-induced exhaustion disorder (ED) remains limited. Therefore, the current study aimed to explore whether sleep concerns, pathological worry, perfectionistic concerns, and psychological flexibility mediate change in exhaustion symptoms during a Multimodal intervention for ED based on Cognitive behavioral therapy principles. Participants ( = 913) were assessed at three time points, and mediation was explored using a two-criteria analytical model with linear mixed-effects models (criterion one) and random intercepts cross-lagged panel modeling (criterion 2).

Article Synopsis
  • The study aimed to assess how well a baseline 12-lead ECG can predict the cause of syncope when monitored with an implantable loop recorder (ILR).
  • Among 300 patients, those with abnormal baseline ECGs, particularly those with bifascicular block, were more likely to get a diagnosis via ILR, indicating a significant correlation between these findings and arrhythmic syncope.
  • The findings suggest that patients with bifascicular block and unexplained syncope could benefit more from permanent pacing than from just the ILR monitoring strategy.

Low-flow anaesthesia is considered beneficial for the patient and the environment, and it is cost reducing due to reduced anaesthetic gas consumption. An initial high-flow to saturate the circle system ( ) is desirable from a clinical point of view. We measured the wash-in and wash-out times (time to saturate and to eliminate the anaesthetic agent, AA), for sevoflurane and desflurane, in a test-lung with fixed 3 MAC vaporizer setting at different fresh gas flow (FGF) and calculated the consumption of AA.

Wash-in and wash-out of sevoflurane in a test-lung model: A comparison between Aisys and FLOW-i.

F1000Res

March 2017

Department of Anaesthesia & Intensive Care, Institution for Clinical Science, Karolinska Institutet, Danderyds University Hospital, Stockholm, 182 88 , Sweden.

Modern anaesthesia workstations are reassuringly tight and are equipped with effective gas monitoring, thus providing good opportunities for low/minimal flow anaesthesia. A prerequisite for effective low flow anaesthesia is the possibility to rapidly increase and decrease gas concentrations in the circle system, thereby controlling the depth of anaesthesia.  We studied the wash-in and wash-out of sevoflurane in the circle system with fixed fresh gas flow and vaporizer setting.
