Background: Acute appendicitis remains an uncommon cause of non-obstetric abdominal pain during pregnancy, with surgery being the preferred management option. We examined our experience with the surgical management of appendicitis during pregnancy, particularly the risk of foetal loss during the 1st and 2nd trimester and performed a meta-analysis of the available literature.

Methods: We performed a retrospective analysis of all patients who had an appendicectomy during pregnancy (January 2010 to December 2019) and a meta-analysis comparing foetal death in open appendicectomy versus laparoscopic appendicectomy during the 1st and 2nd trimester.

The HEV Ventilator: at the interface between particle physics and biomedical engineering.

R Soc Open Sci

March 2022

European Organization for Nuclear Research, Espl. des Particules 1, 1211 Meyrin, Geneva, Switzerland.

A high-quality, low-cost ventilator, dubbed HEV, has been developed by the particle physics community working together with biomedical engineers and physicians around the world. The HEV design is suitable for use both in and out of hospital intensive care units, provides a variety of modes and is capable of supporting spontaneous breathing and supplying oxygen-enriched air. An external air supply can be combined with the unit for use in situations where compressed air is not readily available.

Rationale: Severe acute paediatric asthma may require treatment escalation beyond systemic corticosteroids, inhaled bronchodilators and low-flow oxygen. Current large asthma datasets report parenteral therapy only.

Objectives: To identify the use and type of escalation of treatment in children presenting to hospital with acute severe asthma.

Introduction: Extracorporeal Treatment (ECTR) is an essential component in management of severe lithium toxicity. The Extracorporeal Treatments in Poisoning (EXTRIP) group's suggested indications for ECTR include "if the expected time to obtain a [Li+] < 1.0mEq/L with optimal management is >36h".

Background: Periprosthetic joint infection (PJI) is a devastating condition and there is a lack of evidence to guide its management. We hypothesized that treatment success is independently associated with modifiable variables in surgical and antibiotic management.

Methods: The is a prospective, observational study at 27 hospitals across Australia and New Zealand.

Ileocolic anastomosis after right hemicolectomy: stapled end-to-side, stapled side-to-side, or handsewn?

Int J Colorectal Dis

March 2022

Department of Colorectal Surgery, Dandenong Hospital, Monash Health, 135 David Street, Dandenong, Victoria, 3175, Australia.

Purpose: Anastomotic leak (AL) following ileocolic anastomosis is a cause of significant morbidity and mortality. Stapled end-to-side (ESA), stapled side-to-side (SSA), and handsewn anastomoses (HSA) are commonly performed techniques. There is however conflicting data on the superiority of one technique over the other.

Introduction: Some studies have reported that early administration of acetylcysteine using a 3-bag regimen may not fully prevent development of liver injury in some patients. We compared the incidence of acute liver injury (ALI) in patients receiving acetylcysteine within eight hours of ingestion between the two-bag acetylcysteine regimen (200 mg/kg over four hours, 100 mg/kg over 16 h) and the three-bag regimen (150 mg/kg over 1 h, 50 mg/kg over 4 h, 100 mg/kg over 16 h).

Method: This was a retrospective cohort study of the two-bag and three-bag acetylcysteine regimens from Monash Health, Victoria, Australia (2009-2020), compared to the three-bag acetylcysteine regimen data from the Canadian Acetaminophen Overdose Study (CAOS) database (1980-2005).

Healthcare professional views on barriers to implementation of evidence-based practice in prevention of ventilator-associated events: A qualitative descriptive study.

Intensive Crit Care Nurs

February 2022

School of Nursing and Midwifery, Monash University, McMahons Road, Frankston, Victoria, Australia; Monash Health, Dandenong Hospital, 135 David Street, Dandenong, Victoria, Australia.

Article Synopsis
  • The study aimed to understand the views of healthcare professionals on challenges they face in using evidence-based practices to prevent ventilator-associated events in intensive care settings.
  • Conducted through semi-structured interviews with 20 experienced nurses and doctors in two ICUs in Victoria, Australia, the research highlighted four main barriers: prioritizing certain situations, poor application of evidence, perceived staffing and equipment shortages, and insufficient training on guidelines.
  • The findings indicated a critical need for improved policies and support to enhance adherence to evidence-based practices, ultimately aiming to improve patient care in intensive care units.

Double-Screw Osteosynthesis in an Unstable Scaphoid Fracture Model: A Biomechanical Comparison of Two Screw Configurations.

J Hand Surg Am

November 2022

Hand and Wrist Biomechanics Laboratory, O'Brien Institute, Fitzroy, Victoria, Australia; Division of Hand Surgery, Department of Orthopaedic Surgery, Monash University, Dandenong Hospital, Dandenong, Australia; Department of Plastic and Hand Surgery, St Vincent's Hospital, Fitzroy, Victoria, Australia. Electronic address:

Purpose: Although there is evidence that a single headless compression screw is sufficient for fixation of most scaphoid fractures, double-screw osteosynthesis has been shown to result in higher failure strength and stiffness than a single screw. However, the biomechanical effect of different screw configurations has not been determined.

Methods: A standardized unstable fracture model was produced in 28 cadaveric scaphoids.

Endoscopic Removal of a Fecalith or Fecomucolith in a Rectal Stump or Pouch.

Ann Coloproctol

October 2021

Department of Colorectal Surgery, Monash Health, Dandenong Hospital, Dandenong, VIC, Australia.

Fecaliths or fecomucoliths can form in rectal stumps after a Hartmann procedure or in a coloneovaginal conduit. They can cause significant distress to the patient by causing symptoms such as discharge and odor. We describe a novel and effective method of endoscopic obliteration and removal where other techniques such as removal during examination under anesthesia or serial enemas have failed.

Use of peritoneum in neovagina construction in gender-affirming surgery: A systematic review.

ANZ J Surg

March 2022

Department of Plastic and Reconstructive Surgery, Monash Health (Dandenong Hospital), Dandenong, Victoria, Australia.

Background: Gender dysphoria is defined as discomfort or distress that is caused by a discrepancy between a person's gender identity and that person's sex assigned at birth. The main objective of this study was to review the application, efficacy and outcomes of a novel surgical technique, peritoneal pull-through technique vaginoplasty, in gender-affirming surgery. Specific outcome parameters include (1) healing time (2) depth of cavity achieved (3) alleviation of dysphoria (4) morbidity of the surgery.
