Minimally Invasive Distal Gastrectomy: Evolving Surgical Techniques.

Surg Clin North Am

February 2025

Division of Surgical Oncology, Department of Surgery, Danbury Hospital, Nuvance Health, 95 Locust Avenue, Danbury, CT 06810, USA; Department of Surgery, University of Vermont, Burlington, VT, USA. Electronic address:

Surgical management of gastric cancer has evolved over the years. Incorporation of minimally invasive techniques has led to shorter hospital stays, reduced wound complications, and earlier return to functional activity. Nevertheless, the development of minimally invasive total gastrectomy as a repertoire in the management of proximal or diffuse gastric cancer requires significant commitment and support owing to the complexity of the procedure.

Article Synopsis
  • Proton pump inhibitors (PPIs) are common medications for gastrointestinal issues, but recent studies indicate a possible link between long-term use and increased cardiovascular (CV) risks, raising clinical concerns.
  • This review examines the existing evidence on the potential CV risks associated with PPIs, analyzing mechanisms like drug interactions and electrolyte imbalances that might affect heart and blood vessel function.
  • Healthcare providers must weigh the risks and benefits for individual patients, considering alternative treatments for those at high risk of bleeding and seeking to understand the long-term impact of PPIs on cardiovascular health for future research.

Introduction: Cutaneous melanoma is among the most common solid tumors to metastasize to the gastrointestinal (GI) tract. Literature summarizing the clinical experience and features of this unique pathology is lacking.

Methods: A systematic review of the available literature reporting clinically salient features of melanoma metastases to the small and large intestines was conducted.

BACKGROUND Simultaneous occurrence of chronic lymphocytic leukemia (CLL) and plasma cell myeloma (PCM) is an uncommon hematological condition, with most patients presenting in late adult life. When these diagnoses occur concurrently, it often poses diagnostic challenges to the pathologist, with other important differential diagnoses including lymphoplasmacytic lymphoma, marginal-zone lymphoma, and chronic lymphocytic leukemia with plasmacytoid differentiation. Ancillary studies are indispensable in arriving at a reliable diagnosis in this clinical scenario.

The Transformative Therapeutic Relationship: A Challenge and Opportunity.

Psychodyn Psychiatry

Clinical Professor of Psychiatry & Behavioral Sciences, New York Medical College, Valhalla, NY; Attending Psychiatrist, Danbury Hospital, Danbury, CT.

The transformative therapeutic relationship is a unique interpersonal and intrapsychic phenomenon between analyst and patient. It occurs within the therapeutic framework, and is characterized by specific boundaries of time, place, and verbal and behavioral interactions. Collectively, these interactions may facilitate mutual trust, caring, respect, and affection between analyst and patient.

Neutropenia is a relatively uncommon but notable secondary effect of HIV infection. While the various hematopoietic effects of HIV and AIDS are well-described in the literature, high-quality evidence directly linking neutropenia with mortality in HIV-infected patients remains limited. The multifactorial etiology of neutropenia complicates its diagnosis, particularly when it occurs secondary to HIV.

  • Pembrolizumab is an immune checkpoint inhibitor that targets the PD1 receptor and is used to treat various cancers.
  • Adrenal insufficiency is a rare side effect of pembrolizumab, but its symptoms can be vague, leading to delayed diagnosis and potentially serious consequences.
  • A case study emphasizes the need for healthcare providers to be vigilant about endocrine issues in patients undergoing treatment with immune checkpoint inhibitors to ensure timely diagnosis and intervention.

Background: Nonmuscle invasive bladder cancer (NMIBC) has a favorable prognosis but has high propensity for recurrence. Recent development in one of the urinary biomarker tests, Bladder EpiCheck™, offers a noninvasive and accurate method to detect NMIBC recurrence. In this study, we aimed to compare the diagnostic performance of Bladder EpiCheck™ with urine cytology to detect NMIBC recurrence.

Article Synopsis
  • - The study analyzed the effectiveness of dendritic cell (DC) vaccines in improving survival rates for glioblastoma patients by reviewing seven clinical trials involving 3,619 subjects.
  • - Results indicated that patients receiving the DC vaccine alongside standard treatment had significantly better overall survival (OS) and progression-free survival (PFS), particularly in newly diagnosed cases.
  • - Although the findings suggest that DC vaccines could positively impact survival, further research is needed to confirm benefits for specific patient subgroups facing newly diagnosed or recurrent glioblastoma.

Diabetic ketoacidosis (DKA) is one of the hyperglycemic emergencies seen in patients with poorly controlled diabetes mellitus. One of the potential cardiovascular complications of this hyperglycemic crisis, not that well documented in the literature, is takotsubo cardiomyopathy (TCM) also known as stress-induced cardiomyopathy or "broken heart syndrome". It is a reversible condition where the heart muscle becomes suddenly weakened and stunned, which is mostly known to develop in patients who have suffered a stressful life event or are undergoing an acute illness.

A review of the current recommendations and practices for hemodialysis access monitoring and maintenance procedures.

The growing number of prevalent arteriovenous (AV) accesses has been associated with an increase in the incidence of procedures being performed to maintain patency. To reduce the rate of unnecessary procedures, the 2019 Kidney Disease Outcome Quality Initiative guidelines addended the AV access surveillance recommendations, which includes clinical monitoring and assessment of dialysis adequacy alone. Abnormal clinical findings would necessitate follow-up angiography with or without confirmatory duplex ultrasound.

There are limited data on the optimal choice of anticoagulation in multiple myeloma (MM) patients receiving immunomodulatory drugs (IMiDs). We conducted a propensity score-matched cohort study using the TriNetX database to compare the efficacy and safety of factor Xa inhibitors and warfarin in this patient population. Compared to warfarin, factor Xa inhibitors had a similar risk of deep vein thrombosis (hazard ratio [HR]: 1.

  • Hyperosmolar hyperglycemic state (HHS) is a severe emergency linked to uncontrolled diabetes, increasing the risk of ischemia.
  • The relationship between hyperglycemic crises and stroke is under-researched, despite known risks in chronic hyperglycemia.
  • A case study of an 86-year-old male with HHS highlights challenges in recognizing strokes during these crises, stressing the need for clinical vigilance and further research on associated risks.

Objective: Lower extremity acute limb ischemia (LE-ALI) is associated with high morbidity and mortality rates, and a burden on patient quality of life (QoL). There is limited medium- to long-term evidence on mechanical aspiration thrombectomy (MT) in patients with LE-ALI. The STRIDE study was designed to assess safety and efficacy of MT using the Indigo Aspiration System in patients with LE-ALI.

Cervical cancer is a common gynecological malignancy worldwide. Cervical cancer is staged based on the International Federation of Gynecology and Obstetrics (FIGO) classification system, which was revised in 2018 to incorporate radiologic and pathologic data. Imaging plays an important role in pretreatment assessment including initial staging and treatment response assessment of cervical cancer.
