Background: Health care systems are increasingly focused on assessing patient well-being and functioning. The objective of the current analysis was to evaluate a pragmatic question: to what extent and in what way can the PHQ-2, a routinely collected screening measure, be used to help clinicians and a learning health system understand the well-being and functioning of its beneficiaries?

Methods: The current analysis focused on 2872 Veterans who completed a large-scale longitudinal survey about health and wellness for whom we were able to link survey responses to PHQ-2 scores recorded in their electronic health records (EHR). Regression analyses examined the cross-sectional and longitudinal associations between PHQ-2 scores recorded in the EHR and measures of well-being (life satisfaction, purpose in life, and social health) and functioning (pain severity and interference, physical and mental health, and perceived stress).

Background: Measuring patient well-being during clinical care may enhance patient-centered communication and treatment planning. The Veterans Health Administration (VHA) piloted the use of the Well-Being Signs (WBS), a self-report measure of psychosocial well-being, in clinical care.

Objective: To understand early WBS implementation and health care team member experiences with its use before developing detailed support materials and finalizing the measure.

Purpose: Limited literature exists substantiating benefits of robotic arm-assisted total knee arthroplasty (raTKA) over conventional total knee arthroplasty (cTKA). This study compared postoperative pain, complications, and costs between patients undergoing raTKA and cTKA using large, propensity score-matched cohorts. We hypothesize that the raTKA cohort will be associated with lower pain, lower anemia, and similar cost and other complications.

Background & Aims: Despite the poor prognosis associated with missed or delayed spontaneous bacterial peritonitis (SBP) diagnosis, <15% get timely paracentesis, which persists despite guidelines/education in the United States. Measures to exclude SBP non-invasively where timely paracentesis cannot be performed could streamline this burden.

Methods: Using Veterans Health Administration Corporate Data Warehouse (VHA-CDW) we included patients with cirrhosis between 2009 and 2019 who underwent timely paracentesis and collected relevant clinical information (demographics, cirrhosis severity, medications, vitals, and comorbidities).

Role of Endoscopic Ultrasound in Pancreatic Cancer Diagnosis and Management.

Diagnostics (Basel)

May 2024

Division of Digestive and Liver Diseases, Dallas VA Medical Center, VA North Texas Healthcare System, Dallas, TX 75216, USA.

The emergence of endoscopic ultrasound (EUS) has significantly impacted the diagnosis and management of pancreatic cancer and its associated sequelae. While the definitive role of EUS for pancreatic cancer remains incompletely characterized by currently available guidelines, EUS undoubtedly offers high diagnostic accuracy, the precise staging of pancreatic neoplasms, and the ability to perform therapeutic and palliative interventions. However, current challenges to EUS include limited specialized expertise and variability in operator proficiency.

Impact of head and neck radiation on long-term outcomes after carotid revascularization.

J Vasc Surg

August 2024

Division of Vascular and Endovascular Surgery, Department of Surgery, University of Texas Southwestern Medical Center, Dallas, TX; Division of Vascular and Endovascular Surgery, Department of Surgery, Dallas VA Medical Center, Dallas, TX. Electronic address:

Objective: Radiation-induced carotid artery stenosis (RICS) is a well-described phenomenon seen after head and neck cancer radiation. Previously published literature suggests that, compared with atherosclerotic disease, RICS may result in worse long-term outcomes and early restenosis. This study aims to evaluate the effect of radiation on long-term outcomes after various carotid revascularization techniques using a multi-center registry database.

Article Synopsis
  • Early childhood and adolescent obesity may increase the risk of developing multiple sclerosis (MS), as obesity can lead to inflammation that affects immune responses linked to MS.
  • This study aimed to investigate the relationship between weight-loss drugs and MS using data from the FDA's Adverse Event Reporting System (FAERS) over a nearly 20-year period.
  • Results showed that certain anti-diabetic weight-loss medications, like semaglutide and metformin, were associated with a lower likelihood of developing MS, while other weight-loss drugs did not show this protective effect.

Article Synopsis
  • - The Banff Working Group on Liver Allograft Pathology convened in September 2022 with a diverse group of experts to discuss long-term health monitoring of liver transplants, focusing on noninvasive methods and optimizing immunosuppression.
  • - The group considered revising the rejection classification scheme to better identify and communicate late T cell-mediated rejection patterns and related changes, like nodular regenerative hyperplasia.
  • - They emphasized the need for personalized immunosuppression strategies based on individual patient needs and proposed incorporating interface hepatitis and fibrosis staging into the rejection classification, which will undergo further testing and discussion before the next conference.

Background: Periprosthetic fractures after total knee arthroplasty (TKA) are a challenging problem due to complex fracture patterns, poor bone quality, and a high-risk patient population. Treatment of both periprosthetic fractures and aseptic complications can include revision TKA. In this study, we compared systemic and orthopaedic complications following periprosthetic fracture associated revision TKA to aseptic revision TKA.

Article Synopsis
  • The study investigates the presence of achalasia in patients identified with absent contractility on high-resolution manometry (HRM), distinguishing it from other conditions by employing additional tests like barium esophagography and FLIP.
  • Out of 164 patients, 20 (12.2%) were diagnosed with achalasia, with notable clinical differences between them and those without obstruction, particularly in symptoms like dysphagia and specific HRM features.
  • The findings suggest that even when absent contractility is detected, there remains a significant chance of underlying achalasia, emphasizing the need for thorough diagnostic evaluation beyond just HRM results.

Article Synopsis
  • Dementia and hepatic encephalopathy (HE) can be difficult to differentiate, and undiagnosed cirrhosis in dementia patients may lead to missed treatment opportunities for HE.
  • A study analyzed 177,422 veterans with dementia to explore the prevalence of undiagnosed cirrhosis and its risks, using the Fibrosis-4 (FIB-4) score to assess liver health.
  • Results showed that 5.3% of participants had a high FIB-4 score indicating potential cirrhosis, with older age, being male, and having certain health conditions linked to increased risk, while White race and diabetes were associated with a lower risk.

Purpose Of Review: Barrett's esophagus (BE) is associated with chronic gastroesophageal reflux disease and is a known precursor to esophageal adenocarcinoma. While endoscopic surveillance strategies and the role for endoscopic eradication therapy have been well established, there has been much interest in identifying chemopreventive agents to disrupt or halt the metaplasia-dysplasia-carcinoma sequence in patients with BE.

Recent Findings: No pharmacological agent has held more hope in reducing the risk of neoplastic progression in BE than proton pump inhibitors (PPIs).

Natalizumab (TYSABRI®) was the first high-efficacy monoclonal antibody disease-modifying therapy (DMT) approved as a monotherapy for the treatment of adults with relapsing forms of multiple sclerosis (MS), including clinically isolated syndrome, relapsing-remitting MS, and active secondary progressive MS. Because natalizumab is administered by intravenous infusion, infusion nurses play a key role in the care of natalizumab-treated patients. In the 16 years since approval, substantial data have been gathered on the long-term, real-world effectiveness and safety of natalizumab.
