This study investigated the effect of mannitol administration on postoperative renal function during robot-assisted partial nephrectomy (RAPN) in patients with renal cell carcinoma (RCC). Patients with RCC who underwent RAPN at eight Japanese facilities between March 2016 and November 2023 were enrolled. In this study, patients were categorized into two groups according to those who received mannitol during RAPN (Group I) and those who did not receive mannitol (Group II).

Despite advancements in treatment for patients with unresectable locally advanced non-small cell lung cancer (LA-NSCLC), overall survival (OS) remains poor. The specific effects of varying heart and lung doses on OS in LA-NSCLC patients have not been thoroughly investigated, especially their combined impact on survival. This study aimed to examine the impact on OS of both individual and combined heart and lung doses in patients with LA-NSCLC treated with radiotherapy over a three-year follow-up period.

  • Spontaneous regression of cancer is a rare phenomenon, poorly understood, with limited reports on the changes in morphology and metabolism during this process.
  • An 80-year-old man diagnosed with stage IVA lung cancer experienced a complete regression of rib metastasis after 4 months without treatment, while his mediastinal lymph node metastasis only partially regressed before growing again after 10 months.
  • Analysis via CT and FDG-PET/CT revealed that the rib metastasis had a higher presence of immune cells, indicating immune activation, whereas the mediastinal lymph node metastasis showed fewer immune cells, indicating tumor activation.

  • Fournier's gangrene is a serious soft tissue infection that can develop in patients with weakened immune systems, such as those undergoing chemotherapy; in this case, a 54-year-old man developed it after chemotherapy for testicular cancer.
  • The patient showed symptoms including severe anal pain and dark discoloration of the skin, eventually leading to septic shock and requiring extensive medical intervention over 32 days, including emergency surgery to remove infected tissue.
  • It is crucial to monitor individuals with severe myelosuppression for signs of Fournier's gangrene, even in the absence of visible anorectal lesions, as they may exhibit severe pain and discoloration in the area.

  • - Chronic subdural hematoma (CSDH) often recurs, and this study examines the effects of middle meningeal artery embolization (MMAE) on treated cases and the timing of this treatment.
  • - Out of 72 patients with initially recurring CSDH, those who had a shorter time between first and second recurrences were more likely to experience repeat issues, with 37.5% having a second recurrence.
  • - Early MMAE, within 52.4 days after initial surgery, was linked to a higher risk of recurrence and a shorter interval before recurrence compared to patients who were treated later.

Article Synopsis
  • The study investigates the long-term outcomes of pulmonary arterial hypertension (PAH) linked to congenital heart disease (CHD) using a scoring system called the index of pulmonary vascular disease (IPVD), which categorizes lung pathology severity.
  • Out of 764 patients under 20 years old with CHD-PAH, those with an IPVD score of 2.0 or higher experienced significantly worse survival rates, highlighting the relationship between IPVD scores and cardiovascular death.
  • The findings suggest that the IPVD scoring system can help predict outcomes, indicating that patients with higher IPVD scores may require more careful treatment strategies, such as considering palliative measures before any surgical corrections.

Risk Factors for Delayed-Onset Infection after Mandibular Wisdom Tooth Extractions.

Healthcare (Basel)

March 2023

Department of Oral and Maxillofacial Surgery, Kagawa University Faculty of Medicine, 1750-1, Ikenobe, Miki-cho, Kita-gun, Kagawa 761-0793, Japan.

Wisdom tooth extraction is one of the most commonly performed procedures by oral maxillofacial surgeons. Delayed-onset infection (DOI) is a rare complication of wisdom tooth extraction, and it occurs ~1-4 weeks after the extraction. In the present study, risk factors for DOI were investigated by retrospectively analyzing the cases of 1400 mandibular wisdom tooth extractions performed at Kagawa University Hospital from April 2015 to June 2022.

Solitary fibrous tumors (SFTs) are rare mesenchymal tumors that can occur at any location. Since the identification of specific NAB2-STAT6 fusion in SFTs, the fusion gene variants, NAB2 exon 4-STAT6 exon 2/3 and NAB2 exon 5/6/7-STAT6 exon 16/17/18, have been reported to be associated with clinicopathological features, and the latter variant is predominant in meningeal SFTs. SFTs developing in the salivary glands are rare, and more rarely, those involving ectopic salivary glands (ESGs) have been reported in the cerebellopontine angle (CPA); however, their characteristics remain not well understood.

A woman in her 50s was a super responder to benralizumab administered for the treatment of severe bronchial asthma (BA) with eosinophilic chronic rhinosinusitis with nasal polyp (ECRS) and eosinophilic otitis media (EOM). She exhibited the gradual exacerbation of ECRS/EOM despite good control of BA approximately 1 year after benralizumab initiation. Therefore, the treatment was switched to dupilumab, and the condition of the paranasal sinuses and middle ear greatly improved with the best control of her asthma.

Article Synopsis
  • The study assessed how accurate insulin-like growth factor-1 (IGF-1) is for screening children for growth hormone deficiency (GHD), focusing on kids with short stature or slow growth.
  • Out of 298 examined children, only 60 were diagnosed with GHD, but IGF-1 levels did not show a significant difference between those with and without the deficiency.
  • The findings indicate that IGF-1 alone is not effective for GHD screening, suggesting the need for developing better predictive biomarkers.

Severe bronchial asthma is a challenging disorder to treat and can impair quality of life (QOL) under conventional therapeutic modalities. We report the case of a 52-year-old woman with severe asthma associated with eosinophilic chronic rhinosinusitis (ECRS) and eosinophilic otitis media (EOM). Although the patient was treated with a full dose of inhaled corticosteroid, leukotriene receptor antagonist (LTRA), theophylline, burst use of oral corticosteroids (OCS), her asthmatic condition aggravated, disrupting her daily life.
