J Am Board Fam Med

August 2024

From the Family Medicine Residency Faculty, Harbor UCLA Family Medicine, Los Angeles, CA (CA); Assistant Professor, Department of Family and Community Medicine, Creighton University School of Medicine, Omaha, NE (DMB); Core Faculty, Department of Family Medicine, Southwest IL Family Medicine Residency at St. Louis University, O'Fallon, IL (LWN); Assistant Professor, Department of Family and Community Medicine, Wake Forest University School of Medicine, Winston-Salem, NC (BAT); Tenured Professor of Family and Preventive Medicine, Associate Vice President for Health Equity, Diversity and Inclusion, University of Utah, Salt Lake City UT (JER); Professor and Chair, Department of Family Medicine, University of Texas Medical Branch, Galveston, TX (KMC).

Academic medicine continues to characterize the experiences of Black and other minoritized faculty in medicine to enhance their careers and promote their advancement. An issue of discussion is tenure and its role in the advancement and retention of this group. Tenure is a sign of national presence, command of an area of study, and can demonstrate support from the institution in terms of permanent employment, eligibility to apply for awards, sit or vote on certain committees or qualify for certain leadership opportunities.

Article Synopsis
  • - Treatment-induced ototoxicity from chemotherapy or antibiotics can lead to significant hearing loss, making early intervention crucial; this study explores a local delivery method to the inner ear using crosslinked hybrid nanoparticles (cHy-NPs) in a thermoresponsive hydrogel to combat this issue.
  • - The study focuses on two therapeutic agents, Flunarizine (a calcium channel blocker) and Honokiol (an antioxidant), co-encapsulated in the nanoparticles to provide cytoprotection against cisplatin-induced toxicity in specific cell models and in living zebrafish.
  • - The findings demonstrate that the combination of Flunarizine and Honokiol within the nanoparticles enhances their protective effects; the nanoparticles were successfully synthesized with a high drug

The incidence of esophageal adenocarcinoma (EAC) has surged by 600% in recent decades, with a dismal 5-year survival rate of just 15%. Barrett's esophagus (BE), affecting about 2% of the population, raises the risk of EAC by 40-fold. Despite this, the transcriptomic changes during the BE to EAC progression remain unclear.

Background: Despite their improved safety, by and large, cardiac electrophysiology procedures including catheter ablation (CA), are presently performed in hospital outpatient departments.

Objective: This large multicenter study investigated the safety and outcomes associated with various cardiac electrophysiology procedures performed at 6 ambulatory surgery centers (ASCs), primarily during the coronavirus disease 2019 pandemic under the Center for Medicare and Medicaid Services Hospitals Without Walls program.

Methods: We retrospectively analyzed the outcomes from consecutive electrophysiology procedures performed in ASCs with same-day discharge, including transesophageal echocardiography, cardioversion, cardiac implantable electronic device (CIED) implantation, electrophysiology studies, and CA for atrial fibrillation (AF), atrial flutter (AFL)/supraventricular tachycardia, ventricular premature complexes (VPCs), and atrioventricular node.

Hearing loss is a major health concern in our society, affecting more than 400 million people worldwide. Among the causes, aminoglycoside therapy can result in permanent hearing loss in 40% to 60% of patients receiving treatment, and despite these high numbers, no drug for preventing or treating this type of hearing loss has yet been approved by the US Food and Drug Administration. We have previously conducted high-throughput screenings of bioactive compounds, using zebrafish as our discovery platform, and identified piplartine as a potential therapeutic molecule.

The clinical course and treatment of hypercalcemia from a granulomatous disease in the setting of an infectious etiology, namely disseminated coccidioidomycosis, remains incompletely understood. The mechanism and treatment of hypercalcemia have been documented in most granulomatous disorders, with sarcoidosis being the most well-understood so far. We discuss a case of a patient with a recent diagnosis of disseminated coccidioidomycosis who presented with hypercalcemia despite adequate infection control.

Currently, risk stratification calculators for acute pancreatitis (AP) can at best predict acute pancreatitis mortality at 12 hours from the presentation. Given the severe morbidity associated with AP, the identification of additional prognostic indicators, which may afford earlier prediction in length of stay (LOS) and mortality, is desired. Metabolic acidosis can be a prognostic marker for the severity of AP, and venous bicarbonate can reliably and accurately be substituted for arterial base deficit to detect metabolic acidosis.

Article Synopsis
  • A 47-year-old man with hypertension and obesity developed severe symptoms including back pain, migratory joint pain, dyspnea, and arm weakness, linked to Coxsackie B virus-induced myositis and myocarditis, which are uncommon for this virus.
  • Initial tests showed elevated enzyme levels (CK and troponin I) raising concerns for a non-ST-elevation myocardial infarction (NSTEMI), leading to immediate heart attack treatment protocols.
  • After discovering the patient's recent sore throat and potential contact with an ill family member, a viral test confirmed Coxsackie B virus infection, and the patient recovered fully with supportive care, underscoring the need for thorough viral testing in similar cases.
