Our systematic review and meta-analysis elucidates the postoperative complication rates associated with 3 major surgical approaches, including the scleral tunnel, glued, and flanged techniques, for sutureless intrascleral intraocular lens (IOL) fixation. An online search was performed for articles published in PubMed and EMBASE between January 1, 1975, and January 7, 2023. Original studies reporting postoperative complications of sutureless intrascleral IOL fixation were included.

Background: Endophthalmitis is an acute infection of anterior and posterior segment of the eye involving aqueous humour and vitreous cavity along with the other structures. It is classified into many types including bacterial, Fungal, protozoal, viral and atypical forms. Most common route of infection is exogenous either after the surgery or traumatic perforation Objectives were of evaluate the efficacy of intracameral moxifloxacin versus topical moxifloxacin in preventing postoperative acute endophthalmitis after cataract surgery by phacoemulsification.

[Clinical outcomes of SMILE Xtra for myopia correction].

Vestn Oftalmol

November 2024

OOO Oftalmologicheskaya klinika Spektr, Moscow, Russia.

Article Synopsis
  • This study evaluated the outcomes of the SMILE Xtra technique for patients with thin corneas at risk for keratectasia, involving 53 patients with myopia.
  • It found significant improvements in uncorrected visual acuity and stable refractive results over 12 months, with no cases of keratectasia or major complications reported.
  • The initial decrease in central corneal thickness and mild post-operative symptoms resolved within a week, indicating that SMILE Xtra is a viable option for those with thin corneas.

Background: Keratoplasty is a surgical procedure in which a damaged or diseased cornea is replaced with healthy donor tissue, thereby restoring vision. Recent advancements have led to the replacement of penetrating keratoplasty (PKP) with the more selective deep anterior lamellar keratoplasty (DALK) procedure, especially for treating keratoconus. Although DALK typically has a shorter recovery time, less pain and a lower risk of rejection, PKP is still being performed for more severe corneal diseases.

Purpose: The novel endothelial keratoprosthesis EndoArt improves corneal edema by reducing the inflow of aqueous humor into the cornea. We assessed the early outcome after EndoArt implantation in patients at a high risk of graft failure after keratoplasty.

Methods: This retrospective study included 14 patients with high-risk eyes owing to at least one of the following risk factors for graft failure after keratoplasty: multiple previous surgeries (glaucoma surgery, keratoplasty) and recurrent intraocular inflammation because of uveitis, aniridia, or anterior synechia.

Introduction: Cataract surgery is one of the most commonly performed surgical procedures worldwide. Intraoperative and postoperative complications of cataract surgery include posterior capsule rupture, retinal detachment, suprachoroidal hemorrhage, cystoid macular edema, endophthalmitis, and intraocular lens dislocation. As corneal perforation is rarely related to cataract surgery, we report a case of intraoperative iatrogenic corneal puncture that led to a full-thickness laceration that was managed without severe complications.

Triple treatment therapy for congenital stromal iris cyst.

Indian J Ophthalmol

November 2024

Cornea and External Disease, Cataract and Refractive, Ocular Oncology and Low Vision Services, All India Institute of Medical Sciences, New Delhi, India.

Article Synopsis
  • Iris cysts can cause a range of serious eye issues, so their complete removal is crucial to prevent recurrence and improve patient outcomes.
  • A new three-step technique was developed for managing iris cysts, which includes aspiration, wall removal, and cauterization while minimizing tissue damage.
  • This minimally invasive approach led to fewer complications, no reported recurrences in six months, and overall better visual outcomes compared to more aggressive treatments.

Effectiveness and biocompatibility of a novel Schlemm's canal microstent for glaucoma management.

Sci Rep

October 2024

Zhengzhou University People's Hospital, Henan Provincial People's Hospital, Henan Eye Hospital, Henan Eye Institute, No. 7 Weiwu Road, Zhengzhou, 450003, Henan, China.

To evaluate the effectiveness and biocompatibility of Wistend, a novel Schlemm's canal (SC) microstent made of Nitinol designed to improve aqueous humor outflow. New Zealand white (NZW) rabbits were divided into blank, sham-operated and Wistend groups. ICare Tonovet Plus, swept-source optical coherence tomography (SS-OCT), slit lamp biomicroscopy, retinal camera and scanning electron microscopy (SEM) were used for preoperative and postoperative observations.

Purpose: Report a case where the patient desired spectacle independence after phacoemulsification and Descemet membrane endothelial keratoplasty (DMEK) due to Fuchs' endothelial dystrophy.

Observations: A 52-year-old female presented with corrected distance visual acuity (CDVA) of 20/40 in both eyes with low ametropia. Slit lamp examination revealed corneal edema 1+/4+, guttae, and nuclear cataract 2+/4+ in both eyes.

Cataract surgery with endothelial keratoplasty.

Curr Opin Ophthalmol

January 2025

Cornea, External Eye Diseases and Refractive Surgery Division, King Khaled Eye Specialist Hospital, Riyadh, Saudi Arabia.

Article Synopsis
  • This review focuses on the combined approach of cataract surgery with endothelial keratoplasty (EK), emphasizing key factors such as intraocular lens (IOL) selection and surgical technique.
  • Recent advancements in imaging have enhanced the detection of corneal edema, which is crucial for effective surgical decision-making regarding EK methods.
  • The review highlights that both sequential and triple-EK can achieve similar vision outcomes, but they differ in recovery time and complication rates, with careful preoperative planning being essential for optimal patient results.

Topic: To evaluate the complication rates of different IOL placement methods in adults with inadequate capsular bag support.

Clinical Relevance: The surgical correction of inadequate capsular bag support for the intraocular lens (IOL) harbors several challenges, and there is a wide-range of surgical procedures.

Methods: For the purpose of this meta-analysis, surgical methods were grouped based on the location of IOL placement: (i) AC placement, (ii) iris fixation, which included prepupillary or retropupillary placement of an iris-claw IOL, or iris suturing of an IOL (iii) scleral fixation, which included scleral sutured and sutureless techniques.

Glaucoma is a disease that leads to optic nerve damage and irreversible loss of the visual field. Gonioscopy-assisted transluminal trabeculotomy (GATT) is a minimally invasive glaucoma surgery technique, where the surgeon opens the trabecular network and Schlemm's canal to improve the outflow of the aqueous humor. This method has shown efficacy in reducing intraocular pressure in patients with primary and secondary glaucoma.
