Secondary fracture prevention in primary care: a narrative review.

Osteoporos Int

August 2024

Bone Research Program, ANZAC Research Institute, The University of Sydney at Concord Campus, Hospital Rd, Gate 3, Concord, NSW, Australia.

The global burden of osteoporosis continues to rise with an ageing population. Untreated osteoporotic fractures not only heighten the risk of subsequent fractures but are associated with excess mortality. Although primary care guidelines consistently stress the importance of secondary fracture prevention, fewer than 20% of patients are appropriately treated for osteoporosis following an initial osteoporotic fracture.

Polatuzumab vedotin (Pola) is an approved therapy in combination with rituximab and bendamustine for relapsed or refractory diffuse large B-cell lymphoma (RR-DLBCL) based on positive results of the landmark phase II randomised G029365 trial. However, trial results for many approved novel therapies in RR-DLBCL have not been replicated in routine care cohorts, as RR-DLBCL patient populations are heterogeneous and trial eligibility is increasingly restrictive. We evaluated outcomes from pola ± bendamustine and rituximab in patients with RR-DLBCL enrolled in a compassionate access program with no alternative treatment options identified via the Australasian Lymphoma and Related Diseases Registry according to their eligibility for the original phase II published study.

Article Synopsis
  • Immune checkpoint inhibitors (ICIs) like pembrolizumab are gaining traction for treating cancers that don't respond to traditional chemotherapy, but they can have serious side effects, including a rare condition called pneumatosis intestinalis (PI).
  • A report describes a 50-year-old woman who experienced severe abdominal symptoms months after starting pembrolizumab, with imaging revealing extensive PI along with other complications.
  • The report discusses potential causes of ICI-induced PI and outlines how to manage patients who experience it, highlighting the need for awareness of these risks when using ICIs in cancer treatment.

Toward a Robust Definition of Sport Sex.

Endocr Rev

September 2024

Andrology Department, ANZAC Research Institute, University of Sydney, Concord Hospital, Syndey, NSW 2139, Australia.

Elite individual sports in which success depends on power, speed, or endurance are conventionally divided into male and female events using traditional binary definitions of sex. Male puberty creates durable physical advantages due to the 20- to 30-fold increase in circulating testosterone producing a sustained uplift in men's muscle, bone, hemoglobin, and cardiorespiratory function resulting from male puberty and sustained during men's lives. These male physical advantages provide strong justification for a separate protected category of female events allowing women to achieve the fame and fortune from success they would be denied if competing against men.

Mobile phone text messaging for medication adherence in secondary prevention of cardiovascular disease.

Cochrane Database Syst Rev

March 2024

Westmead Applied Research Centre, Faculty of Medicine and Health, University of Sydney, Sydney, Australia.

Background: Cardiovascular diseases (CVDs) are the leading cause of death globally, accounting for almost 18 million deaths annually. People with CVDs have a five times greater chance of suffering a recurrent cardiovascular event than people without known CVDs. Although drug interventions have been shown to be cost-effective in reducing the risk of recurrent cardiovascular events, adherence to medication remains suboptimal.

Article Synopsis
  • There is a growing interest in using historical patient data as synthetic controls for evaluating new drugs, but real-world outcomes often don't match those from clinical trials due to a lack of detailed cancer treatment data.
  • The Australasian Leukaemia and Lymphoma Group's National Blood Cancer Registry (ALLG NBCR) provides comprehensive information on various factors influencing treatment outcomes, allowing for a comparison of 942 AML patients to clinical trial data for five specific drugs.
  • The analysis reveals significant differences in treatment approaches and outcomes between real-world patients and clinical trial participants, indicating that while some results may align, discrepancies must be considered for accurately assessing the effectiveness of new therapies across different populations.

Magnetic Resonance Imaging Characteristics of LGI1-Antibody and CASPR2-Antibody Encephalitis.

JAMA Neurol

May 2024

Oxford Autoimmune Neurology Group, Nuffield Department of Clinical Neurosciences, University of Oxford, Oxford, United Kingdom.

Importance: Rapid and accurate diagnosis of autoimmune encephalitis encourages prompt initiation of immunotherapy toward improved patient outcomes. However, clinical features alone may not sufficiently narrow the differential diagnosis, and awaiting autoantibody results can delay immunotherapy.

Objective: To identify simple magnetic resonance imaging (MRI) characteristics that accurately distinguish 2 common forms of autoimmune encephalitis, LGI1- and CASPR2-antibody encephalitis (LGI1/CASPR2-Ab-E), from 2 major differential diagnoses, viral encephalitis (VE) and Creutzfeldt-Jakob disease (CJD).
