Medicines Intelligence Centre of Research Excellence, School of Population Health, Faculty of Medicine and Health, UNSW Sydney, Sydney, Australia.

Article Synopsis
  • Dual antiplatelet therapy with P2Y12 inhibitors and aspirin after acute myocardial infarction is common, but patients with atrial fibrillation may also need oral anticoagulants, increasing their bleeding risk.
  • A study in New South Wales examined the post-discharge prescribing patterns for 1,330 patients with a history of atrial fibrillation who were hospitalized for acute myocardial infarction, finding that 74.3% were prescribed an OAC, but issues like underprescribing and prolonged P2Y12i usage were noted.
  • Recommendations include closer monitoring and adherence to guidelines to prevent overdosing on therapies associated with increased bleeding and address gaps in therapy management for patients transitioning from hospital to community care.

Follicular lymphoma (FL) is the most common indolent non-Hodgkin lymphoma subtype, accounting for 15-20% of all lymphoma diagnoses. Although typically slow-growing and responsive to frontline therapies, advanced-stage FL remains incurable with current treatments and typically follows a chronic relapsing/remitting course with increasingly shorter responses to subsequent lines of therapy. Outcomes are highly variable; some patients experience prolonged first remissions that may approximate a 'functional cure'.

Introduction: The therapeutic effects of steroids in immunoglobulin A nephropathy (IgAN) global (TESTING) study reported that methylprednisolone reduces the risk of major kidney events in individuals with IgAN at high risk of disease progression compared to supportive care alone but is associated with increased serious adverse events (SAEs) primarily with full-dose therapy. The risk benefit balance of the reduced-dose methylprednisolone regimen is examined in this prespecified analysis of the reduced-dose cohort of the TESTING trial.

Methods: Between 2017 and 2019, patients with IgAN, proteinuria ≥1 g/d despite 3 months of renin-angiotensin-system blockade and estimated glomerular filtration rate (eGFR) 30 to 120 ml/min per 1.

NHMRC Clinical Trials Centre, University of Sydney, Sydney, NSW, Australia; UQ Centre for Clinical Research, University of Queensland, Brisbane, QLD, Australia; Australian and New Zealand Urogenital and Prostate Cancer Trials Group, Australia. Electronic address:

Article Synopsis
  • - This research examines why positive treatment effects seen in the first interim analysis of clinical trials may decline in later analyses, focusing on issues like overestimation bias, non-proportional hazards, and varied recruitment.
  • - The study analyzed 71 oncology randomized clinical trials, showing that hazard ratios often overestimate treatment effects, especially when fewer events have occurred by the interim analysis.
  • - The findings highlight the need to apply adjusted hazard ratios to reduce overestimation bias and stress the importance of considering various factors when reporting on positive interim analysis results.

  • Oculopharyngodistal myopathy (OPDM) is a genetic muscle disease that causes drooping eyelids, trouble swallowing, and weakness in the arms and legs.
  • Recent research found repeating sequences in a gene called ABCD3 in people with OPDM from European backgrounds, while similar repeats were only discovered in certain Asian groups before.
  • These long repeats in the ABCD3 gene might play a role in the muscle problems seen in OPDM, suggesting a link between these repeats and the weakness that affects patients.

Article Synopsis
  • The study investigates the relationship between ganglion cell-inner plexiform layer (GCIPL) measurements from widefield optical coherence tomography (OCT) scans and clinical visual field (VF) data across different eye conditions.
  • It involved analyzing OCT scans from healthy individuals, glaucoma suspects, and glaucoma patients to determine how GCIPL thickness correlates with visual field thresholds, using statistical models to assess diagnostic accuracy.
  • Results show significant variability in visual field data at defective locations and suggest that using GCIPL parameters could improve the prediction of VF status, potentially aiding in clinical decision-making for eye care.

Background: The Latarjet procedure was developed for the treatment of anterior shoulder instability in young, high-demand patients with attritional glenoid bone loss, whose risk of redislocation following primary dislocation may exceed 90%. Coracoid graft osteolysis and prominent screws are commonly observed in late computed tomography (CT) scans of patients who re-present following the procedure, but the clinical relevance of osteolysis in the overall Latarjet cohort is undetermined. We aimed to evaluate clinical and radiological outcomes in patients who underwent the Latarjet procedure, and to determine if severe coracoid graft osteolysis compromised clinical outcomes.

The assessment of gas exchange under varying ambient and metabolic conditions is an important and fundamental investigation of respiratory function. The gold standard is an arterial blood gas (ABG) sample; however, the procedure is not universally performed by medical scientists, is not standardised, and is typically taught by a subjective Halsted 'see one, do one' approach. The Australian and New Zealand Society of Respiratory Science recognised the need to create an ABG position statement that includes the required pre-requisite education, an evidence-based procedure and the minimum reporting and competency assessment requirements.

Pregnancy in women with early-onset Parkinson's disease (PD) is likely to have a higher frequency given the trend toward increasing maternal age, thus resulting in a greater overlap time between childbearing age and PD risk. Deep brain stimulation (DBS) therapy is nowadays offered to PD patients at earlier stage of the disease, when women can still be pre-menopausal. However, few data are available about DBS safety during pregnancy.
