Development of a simplified dysphagia assessment by dentists in older persons.

Community Dent Oral Epidemiol

June 2018

Graduate Studies Program in Dentistry, Faculty of Dentistry, Universidade Federal do Rio Grande do Sul, Porto Alegre, Brazil.

Objective: This study aimed to evaluate the diagnostic accuracy of a simplified clinical examination of swallowing by dentists and the Eating Assessment Tool (EAT-10), when compared with the diagnosis provided by a speech pathologist (gold standard).

Methods: Three dentists and 1 speech pathologist clinically evaluated 265 older persons in southern Brazil, 123 were residents in long-term care and 142 were community-dwelling, all able to respond to the research protocol independently. Accuracy, sensitivity, specificity, positive and negative predictive values (+PV and -PV), and positive and negative likelihood ratios (+LR and -LR) were calculated according to standard methods.

The availability of evidence-based guidelines does not ensure their implementation and use in clinical practice or policy making. Inequities in health have been defined as those inequalities within or between populations that are avoidable, unnecessary and also unjust and unfair. Evidence-based clinical practice and public health guidelines ('guidelines') can be used to target health inequities experienced by disadvantaged populations, although guidelines may unintentionally increase health inequities.

Chronic liver failure-consortium acute-on-chronic liver failure and acute decompensation scores predict mortality in Brazilian cirrhotic patients.

World J Gastroenterol

July 2017

Rafael Veiga Picon, Franciele Sabadin Bertol, Cristiane Valle Tovo, Gastroenterology and Hepatology Unit, Nossa Senhora da Conceição Hospital, Porto Alegre, Rio Grande do Sul 91350-200, Brazil.

Article Synopsis
  • The study aimed to validate the effectiveness of certain prognostic scores for predicting mortality in Brazilian patients with acute decompensation of cirrhosis and acute-on-chronic liver failure.
  • It utilized a prospective cohort design to compare the chronic liver failure-consortium scores (CLIF-C AD and CLIF-C ACLF) against other models, including MELD and Child-Pugh, focusing on 28-day and 90-day mortality rates.
  • Results indicated that the CLIF-C ACLF score was notably effective in predicting mortality in ACLF patients, while both CLIF-C AD and other scores showed similar predictive capabilities for patients with acute decompensation.

Background: To evaluate whether the presence of both prostate atrophy (PA) and chronic prostate inflammation (CPI) in the same biopsy and in the same biopsy core are associated with prostate cancer (PCa) risk and grade in repeat biopsies.

Methods: Retrospective analyses of 6132 men who were 50-75 years old undergoing 2-year repeat prostate biopsy after a negative baseline biopsy for PCa in the REduction by DUtasteride of prostate Cancer Events (REDUCE) study. PA, CPI and PCa were determined by central pathology.

Objective: To provide guidance for guideline developers on how to consider health equity at key stages of the guideline development process.

Study Design And Setting: Literature review followed by group discussions and consensus building.

Results: The key stages at which guideline developers could consider equity include setting priorities, guideline group membership, identifying the target audience(s), generating the guideline questions, considering the importance of outcomes and interventions, deciding what evidence to include and searching for evidence, summarizing the evidence and considering additional information, wording of recommendations, and evaluation and use.

Association of NR1I2 gene polymorphisms and time of progression to AIDS.

Mem Inst Oswaldo Cruz

April 2017

Universidade Federal de Ciências da Saúde de Porto Alegre, Programa de Pós-Graduação em Patologia, Porto Alegre, RS, Brasil.

Background: The time of progression towards AIDS can vary greatly among seropositive patients, and may be associated with host genetic variation. The NR1I2 (PXR) gene, a ligand-activated transcription factor, regulates the transcription immune pathway genes and can therefore be targets of viral replication mechanisms influencing time of progression to AIDS.

Objective: To verify the association of single nucleotide polymorphisms (SNPs) rs3814057, rs6785049, rs7643645, and rs2461817 in the NR1I2 (PXR) gene with progression to AIDS in HIV-1 infected patients.

Objective: Supplementation with ω-3 polyunsaturated fatty acids (PUFAs) can positively contribute to neurologic development, modulating inflammatory responses, promoting homeostasis, and having a positive effect on animal behaviors associated with mental disorders. The aim of this study was to evaluate behavioral and biochemical effects of ω-3 fatty acid supplementation in an animal model for mental disorders by prenatal maternal exposure to lipopolysaccardies (LPS) from the maternal immune activation.

Methods: Twelve pregnant Wistar rats were used.

Background: Arterial hypertension is an important risk factor for cardiovascular outcomes. Blood pressure (BP) control levels remain largely out of target among primary healthcare (PHC) patients. Twenty-four-hour ambulatory blood pressure monitoring (ABPM) may contribute toward the identification of cardiovascular risk groups.

HLA-G and CD8+ regulatory T cells in the inflammatory environment of pre-eclampsia.

Reproduction

December 2016

Laboratory of ImmunogeneticsDepartment of Genetics, UFRGS, Porto Alegre, RS, Brazil

During pregnancy, the maternal immune system is tolerant to foetal antigens via the engagement of immune regulatory mechanisms. Failure in regulating the maternal immunity to foetal antigens may lead to pre-eclampsia (PE). We addressed the role of HLA-G gene polymorphisms and protein expression as well as regulatory T cells and Th1/Th2/Th17 cytokines in healthy and pathological pregnancies.

Background: Although serogroup 20 is not part of any conjugate pneumococcal vaccine, its serotype 20A, but not 20B, belongs to the polysaccharide 23-valent formula. Little is known about its clinical, laboratorial and epidemiological characteristics.

Methods: The purpose of this study was to evaluate the bacterial genotypes (by PFGE and MLST), clinical characteristics of patients (from review of medical records) and antimicrobial susceptibility of serogroup 20 isolates which were recovered from patients with invasive pneumococcal disease (IPD) from 2007 to 2012.

Aims: HIV infection is a chronic disease that requires intensive treatment in its later phases, including dietary supplementation. Several studies have suggested clinical improvements in patients with high levels of selenium, linking these levels with a longer progression to AIDS. The objective of this study was to verify the association of two polymorphisms in the SEP15 gene, which encodes a selenoprotein that is responsible for the transport of selenium in cells, with the time of progression to AIDS in HIV-1-infected patients.

Article Synopsis
  • Cytokines play a crucial role in the progression of HIV, with plasma levels evaluated in extreme progressors (slow and rapid) before starting HIV treatment (HAART).
  • Significant differences were found in the levels of IL-6 and IL-10 across different HIV infection stages, with elevated levels in slow progressors before starting HAART compared to other groups.
  • These findings suggest that increased IL-6 and IL-10 levels may serve as indicators of inflammation and potentially reflect the disease progression in HIV-infected individuals, regardless of their progression status.
