Achieving a stable plantigrade foot after repair of complex post-burn deformities poses a particularly difficult challenge for the foot and ankle surgeon. We report an unusual case of a child with severe bilateral forefoot contracture deformities treated by soft tissue release and lengthening, and conventional wound coverage with split-thickness skin grafting.

Bilateral sudden deafness is uncommon and characterized by an acute sensorineural hearing loss in both ears of 30 dB or more in 3 consecutive frequencies. Since the introduction of antituberculous medications, the incidence of laryngeal tuberculosis (LTB) has decreased. However, it is a fact the increasing number of cases of pulmonary or extrapulmonary TB mainly caused by the Acquired Immunodeficiency Syndrome. We report the clinical case of a 72 years old male with dysphonia secondary to LTB which masqueraded a carcinoma.
View Article and Find Full Text PDFCogan Sindrome is described as the association of non-syphilic interstitial keratitis with vestibulo-auditory symptoms in its typical form. We are reporting a case corresponding to a 32 years old male who consulted at first place for ophthalmic clinic and, two months later, affected of bilateral neurosensorial hearing loss with tinnitus and vestibular dysfunction. In spite of the corticoid treatment our patient has not improved.

View Article and Find Full Text PDFWe report our study of 32 patients diagnosed as hyperparathyroidism due to chronic renal failure (27 secondary forms and 5 tertiary ones), between June 1990 and 2003, that were sent from Nefrology Department for to surgical intervention. The following parameters have been analysed: Age, sex, clinical symptoms, kind of surgery performed, postoperative complications and anatomopathologic result (AP). On 64 nerves exposed, there were no recurrent nerve palsies.

Hyperparathyroid crisis is an uncommon clinical state characterized by severe hypercalcemia which can produce an acute confusional syndrome (ACS) or delirium. We are reporting the case of a 58-year male with calcemias greater than 14 mg/dl, that were due to a parathyroid adenoma (primary hyperparathyroidism), and caused his admission in UCI with change in the conscience level, global afectation of the cognitive functions, letargia and disorentiation among other clinical symptoms. The patient was operated by our Service on lower left parathyroidectomy with PTH rapid or turbo assay and decreasing such hormone levels about 75%.

We report the clinical case of a 36 years old female without significant previous history who was diagnosed with an acute maxillary sinusitis with not responding to antibiotic treatment. After her admission we asked for a CT which showed an occupation in the left maxillary sinus with a calcified area inside suggesting a mycetoma. The patient was operated by Functional Endoscopy Sinusal Surgery (FESS) plus Caldwell-Luc techniques sending the obtained sample for microbiological study, that was reported as sinusal mycosis by Zygomicetos.

Tertiary hyperparathyroidism represents an autonomous proliferation state of the parathyroid glands that takes place in spite of performing a renal transplantation in patients with chronic renal failure due to the illness. We are reporting 5 own clinical cases collected in a 14 years period corresponding to 2 men and 3 women, with ages between 43 and 53, that were sent from Nephrology for surgical intervention. The AP result was hyperplasia in 4 samples and 1 adenoma in the rest.

The antrochoanal polyps or solitary Killian polyps are seldom benign pathologies. Since 1990 to October 2002 we have registered a total of 30 patients with these diagnosis (20 male and 10 female) between 5 and 70 years old. The nasal obstruction, usually unilateral, has been constant in all of them and in more than a 63% they referred it since more than one year.

The sinusites orbital complications usually associated to ethmoidites are rare in the childhood. We present a revision of 14 adults patients (10 males and 4 females) 18 to 61 years old. Seven presented a preseptal cellulitis, two had a subperiostial abscess and five an orbitary cellulitis in accordance with the Candler classification.
