Aims: To evaluate the association between transcatheter edge-to-edge repair (TEER) and outcomes in patients with significant mitral regurgitation (MR) following acute myocardial infarction (MI), focusing on the aetiology of acute post-MI MR in high-risk surgical patients.
Methods And Results: The International Registry of MitraClip in Acute Mitral Regurgitation following Acute Myocardial Infarction (IREMMI) includes 187 patients with severe MR post-MI managed with TEER. Of these, 176 were included in the analysis, 23 (13%) patients had acute papillary muscle rupture (PMR) and 153 (87%) acute secondary MR.

Introduction: Complete lymph node dissection (CLND) was the standard practice for patients with melanoma and a positive sentinel lymph node biopsy (SLNB) until the results of two clinical trials published in 2016 and 2017 demonstrated that it did not improve melanoma-specific survival (MSS). However, it continues to be performed in some scenarios. No studies have ever been published on lymph node management after a positive SLNB in the routine clinical practice in our setting.

Objectives: The study aimed to explore a definition for complicated coagulase-negative staphylococci bloodstream infections (CoNS BSIs) and to identify predictors for mortality.
Methods: A prospective cohort study was conducted from October 2016 to March 2017 in 26 Spanish hospitals. Complicated CoNS BSI criteria included lack of early catheter removal in catheter-related cases, foreign indwelling implant, persistent bacteraemia, fever ≥72 hours on active therapy, metastatic infection or deep-seated focus, and infective endocarditis.

BK hemorrhagic cystitis (BK-HC) is a common complication following hematopoietic stem cell transplantation (HSCT), particularly when posttransplant cyclophosphamide (PTCy) is used as graft-versus-host disease (GVHD) prophylaxis. However, comparative studies of BK-HC incidence in matched sibling donors (MSD) and unrelated donors (MUD) often include small haploidentical (HAPLO) donor cohorts and usually lack detailed information on disease evolution, coinfections, management and impact on outcomes. This study aimed to evaluate the incidence, risk factors, and outcomes in patients with hematologic malignancies undergoing HSCT from MSD, MUD, HAPLO donors using PTCy as GVHD prophylaxis.

Extrahepatic metastases in hepatocellular carcinoma (HCC) often signify a poor prognosis. This case details the five-year survival of a patient with HCC and adrenal metastasis managed with an aggressive multimodal approach, including ablation, systemic therapy, and surgical resection. Despite therapeutic advances, metastatic HCC management remains a clinical challenge, underscoring the need for individualized treatment strategies beyond traditional staging systems.

We present the case of a 64-year-old male diagnosed with corticosteroid-dependent ulcerative colitis in 1999, who underwent total proctocolectomy with a J-pouch in 2005. Three years later, he developed recurrent histologically confirmed pouchitis, with clinical worsening. Several therapies were tried, including Infliximab, which provided stability for six years, until the development of antibodies led to moderate pouchitis in the rectal stump.

The most common extranodal localization of lymphomas is the small intestine, with large B-cell lymphoma and MALT lymphoma being the most typical types. Intestinal follicular lymphomas are an infrequent entity; however, due to improved endoscopic definition, diagnosis is more ando more common. We present the case of a 56-year-old woman diagnosed with follicular lymphoma, an entity that, although infrequent, has a typical endoscopic appearance.
View Article and Find Full Text PDFThe concept of "disease related malnutrition" (DRM) is far from the concept of malnutrition classically diagnosed in public health, which is determined by socio-demographic factors. In 2008, the Spanish Society of Clinical Nutrition and Metabolism (SENPE) and the Spanish Society of Medical Documentation (SEDOM) published a consensus defining more precisely the types of malnutrition seen in hospitals and their corresponding coding. Changes in the International Classification of Diseases (ICD) and the evolution of clinical information have made it necessary to revise and update this consensus in order to establish new criteria to guide the recording and coding of diagnoses of malnutrition in the field of hospital care, thus facilitating the work of both clinicians and coders and improving the visibility of DRE, by adapting from the clinical diagnoses of undernutrition based on the GLIM criteria (Global Leadership Initiative on Malnutrition) proposed by the scientific societies to the current codes proposed by ICD-10-ES.
