Background: Increasing pressure on limited intensive care capacities often requires a subjective assessment of a patient's discharge readiness in the absence of established Admission, Discharge, and Transfer (ADT) guidelines. To avoid suboptimal care transitions, it is important to define clear guidelines for the admission and discharge of intensive care patients and to optimize transfer processes between the intensive care unit (ICU) and lower care levels. To achieve these goals, structured insights into usual ICU discharge and transfer practices are essential.

Aims/hypothesis: While the association between coeliac disease and type 1 diabetes is well documented, the association of coeliac disease with type 2 diabetes risk remains undetermined. We conducted a nationwide cohort and Mendelian randomisation analysis to investigate this link.

Methods: This nationwide matched cohort used data from the Swedish ESPRESSO cohort including 46,150 individuals with coeliac disease and 219,763 matched individuals in the comparator group selected from the general population, followed up from 1969 to 2021.

Natural Language Processing to Identify Home Health Care Patients at Risk for Becoming Incapacitated With No Evident Advance Directives or Surrogates.

J Am Med Dir Assoc

August 2024

Brookdale Department of Geriatrics and Palliative Medicine, Icahn School of Medicine at Mount Sinai, New York, NY, USA; Center for Nursing Research and Innovation, Mount Sinai Health System, New York, NY, USA. Electronic address:

Objectives: Home health care patients who are at risk for becoming Incapacitated with No Evident Advance Directives or Surrogates (INEADS) may benefit from timely intervention to assist them with advance care planning. This study aimed to develop natural language processing algorithms for identifying home care patients who do not have advance directives, family members, or close social contacts who can serve as surrogate decision-makers in the event that they lose decisional capacity.

Design: Cross-sectional study of electronic health records.

Delayed intervention for ST-segment elevation myocardial infarction (STEMI) is associated with higher mortality. The association of door-to-ECG (D2E) with clinical outcomes has not been directly explored in a contemporary US-based population. This was a three-year, 10-center, retrospective cohort study of ED-diagnosed patients with STEMI comparing mortality between those who received timely (<10 min) vs.

View Article and Find Full Text PDF
Article Synopsis
  • This study investigates how the time taken to obtain an ECG (door-to-ECG or D2E time) impacts the time between ECG and the PCI procedure (ECG-to-PCI or E2B) in STEMI patients across 10 emergency departments over three years.
  • Findings indicate that patients with a D2E time greater than 10 minutes had longer E2B intervals compared to those with timely ECGs, especially during the triage phase in the ED.
  • The research suggests that reducing D2E times, particularly for patients diagnosed during triage, could significantly enhance the efficiency of care for STEMI patients.

Influenza most often causes a febrile viral syndrome inclusive of pulmonary irritation with cough, shortness of breath, and congestion. However, severe infection can also occur, causing significant viral pneumonia with Type 1 respiratory failure. and rare but life-altering complications such as pneumomediastinum, secondary bacterial pneumonia, acute respiratory distress syndrome (ARDS), viremia, and death.

Article Synopsis
  • Sinus rhythm activation time is a key indicator for evaluating the substrate of heart tissue following an infarct, particularly in the context of ventricular tachycardia (VT).
  • A study involving dogs post-infarction revealed that analyzing sinus rhythm and VT electrograms can reveal patterns that predict VT inducibility.
  • The findings suggest that the sinus rhythm activation signature can help differentiate between conditions that support reentrant VT and those that do not, which is important for targeting treatment locations within the heart.

Respiratory drive heterogeneity associated with systemic inflammation and vascular permeability in acute respiratory distress syndrome.

Crit Care

April 2024

Columbia Respiratory Critical Care Trials Group, Columbia University College of Physicians and Surgeons, and New York-Presbyterian Hospital, 622 West 168th Street, New York, NY, 10032, USA.

Background: In acute respiratory distress syndrome (ARDS), respiratory drive often differs among patients with similar clinical characteristics. Readily observable factors like acid-base state, oxygenation, mechanics, and sedation depth do not fully explain drive heterogeneity. This study evaluated the relationship of systemic inflammation and vascular permeability markers with respiratory drive and clinical outcomes in ARDS.

Background: Improved technologies paired with an increase in access to genetic testing have led to the availability of expanded carrier screening evaluating hundreds of disorders. Currently, most autosomal dominant mutations, such as BRCA1, are not included in expanded carrier assays. Screening pregnant or preconception reproductive-aged women for BRCA1 may present a unique opportunity to perform population-based screening for patients at a time when precancer screening, chemoprevention, and/or risk-reducing surgery may be beneficial.

Background: As of November 8, 2023, 24 states and the District of Columbia have legalized cannabis for both recreational and medical use (RMCL-states), 14 states have legalized cannabis for medical use only (MCL-states) and 12 states have no comprehensive cannabis legislation (NoCL-states). As more states legalize cannabis for recreational use, it is critical to understand the impact of such policies on driving safety.

Methods: Using the 2019 and 2020 Fatality Analysis Reporting System data, we performed multivariable logistic regression modeling to explore the association between state level legalization status and cannabis positivity using toxicological testing data for 14,079 fatally injured drivers.

July 2024

Division of Gynecologic Oncology, Department of Obstetrics and Gynecology, Columbia University College of Physicians and Surgeons, New York, NY, USA. Electronic address:

Article Synopsis
  • The study aimed to evaluate clinical and pregnancy characteristics, surgical practices, and complications from cesarean hysterectomy performed due to placenta accreta spectrum, focusing on the surgeon's specialty.
  • Data were analyzed from the Premier Healthcare Database for patients who had cesarean deliveries and hysterectomies between 2016 and 2020, comparing outcomes based on different types of surgeons: general obstetricians, maternal-fetal medicine specialists, and gynecologic oncologists.
  • Results showed that while the types of surgeons varied, surgical complications such as hemorrhage and bladder injuries were comparable across the groups, but gynecologic oncologists had patients with more severe cases (placenta increta/percreta) and specific surgical

Systemic lupus erythematosus (SLE) is a highly heterogenous autoimmune disease that affects multiple organs, including the heart. The mechanisms by which myocardial injury develops in SLE, however, remain poorly understood. Here we engineered human cardiac tissues and cultured them with IgG fractions containing autoantibodies from SLE patients with and without myocardial involvement.

A cerebro-cerebellar network for learning visuomotor associations.

Nat Commun

March 2024

Dept. of Neuroscience, Mahoney Center for Brain and Behavior Research, Zuckerman Mind, Brain, and Behavior Institute, Columbia University, New York, NY, USA.

Consensus is rapidly building to support a role for the cerebellum beyond motor function, but its contributions to non-motor learning remain poorly understood. Here, we provide behavioral, anatomical and computational evidence to demonstrate a causal role for the primate posterior lateral cerebellum in learning new visuomotor associations. Reversible inactivation of the posterior lateral cerebellum of male monkeys impeded the learning of new visuomotor associations, but had no effect on movement parameters, or on well-practiced performance of the same task.
