We report the case of a 56-year-old female with a past medical history of multiple sclerosis on disease-modifying therapy of fingolimod who presented with disseminated infection, initially of the ankles bilaterally before progressing to the central nervous system. CNS coccidiomycosis has thus far not been associated with any pharmacological therapy for multiple sclerosis. Clinicians should have a high degree of suspicion for infection in immunosuppressed patients living in endemic areas.

Outcomes of a step-up approach to the treatment of neurogenic cough.

Am J Otolaryngol

August 2024

University of Arizona, Department of Otolaryngology-Head and Neck Surgery, United States of America.

Introduction: Neurogenic cough (NC) is thought to be related to sensory neuropathy in the hypopharynx and larynx. Defined as a cough persisting longer than 8 weeks refractory to standard therapy, it is a diagnosis of exclusion when other common etiologies (asthma, gastroesophageal reflux disease (GERD), medication side effects) are ruled out. It affects roughly 11 % of Americans and can negatively impact quality of life.

Article Synopsis
  • The study aims to evaluate the cost-effectiveness of toripalimab compared to pembrolizumab for patients with advanced nonsquamous non-small cell lung cancer in the U.S. in 2024, while also considering the potential for expanded patient access to toripalimab through budget-neutral solutions.
  • Using simulation modeling, the research assessed the financial implications of different treatment regimens and patient populations, revealing significant savings associated with toripalimab.
  • The findings indicate that using toripalimab could allow for the funding of thousands of additional treatment cycles and regimens without increasing the budget, which emphasizes the potential benefits of this alternative therapy.

  • Transmembrane protein 184B (TMEM184B) plays a crucial role in brain development and is linked to neurodevelopmental disorders when mutated.
  • Six pediatric patients with neurodevelopmental issues, such as intellectual disability and microcephaly, were found to have rare genetic variants in TMEM184B affecting protein stability and function.
  • Experimental studies in zebrafish and cell lines showed that these mutations lead to similar symptoms observed in patients, indicating that TMEM184B variants disrupt cellular processes and contribute to abnormal neural development.

Derived from the survival advantage needed to eradicate harmful irritants from the skin's surface, itch, also known as pruritus, is an intriguing phenomenon and a key feature of many dermatologic diseases. The pathologic form, chronic itch, is a condition that precedes itself by its complex pathophysiology, numerous etiologies, and interplay of mechanisms that often make it difficult to diagnose and treat. One particular sufferer of this condition, the 18th-century French emperor Napoleon Bonaparte, makes for a compelling case study of the complexity of chronic itch.
