Genomic Reporting Practices Across 5 Molecular Disciplines: A Study From the College of American Pathologists.

Arch Pathol Lab Med

November 2024

the Department of Pathology and Laboratory Medicine, Alpert Medical School at Brown University, Women & Infants Hospital of Rhode Island, Providence (Palomaki).

Article Synopsis
  • Genomic reports in molecular diagnostics are often narrative and inconsistent in content and format, despite existing regulatory guidelines.
  • A study evaluated 69 reports from 31 laboratories across five disciplines, finding that most met compliance but exhibited variability in how required elements were presented.
  • The findings highlight a need for improved consistency in report formatting to enhance communication of genetic test results to healthcare providers and patients.

Article Synopsis
  • The study aimed to outline how mismatch repair (MMR) and microsatellite instability (MSI) testing is conducted in various laboratories, specifically before the 2022 CAP/AMP guidelines were published.
  • Researchers analyzed data from supplementary questionnaires sent to 542 laboratories regarding their testing practices and types of specimens used.
  • Results showed that domestic labs test for MSI/MMR more frequently than international ones, with academic hospitals leading in testing rates; most labs are aware of the clinical implications of high MSI or MMR-deficient results for immune checkpoint therapy.

Increasing pursuit of subspecialized training has quietly revolutionized physician training, but the potential impact on physician workforce estimates has not previously been recognized. The Physicians Specialty Data Reports of the Association of American Medical Colleges, derived from specialty designations in the American Medical Association (AMA) Physician Professional Data (PPD), are the reference source for US physician workforce estimates; by 2020, the report for pathologists was an undercount of 39% when compared with the PPD. Most of the difference was due to the omission of pathology subspecialty designations.

Entrustable professional activities (EPAs) are observable clinical skills and/or procedures that have been introduced into medical education at the student and resident levels in most specialties to determine readiness to advance into residency or independent practice, respectively. This publication describes the process and outcomes of a pilot study looking at the feasibility of using two anatomic pathology and two clinical pathology EPAs in pathology residency in 6 pathology residency programs that volunteered for the study. Faculty development on EPAs and their assessment was provided to pilot program faculty, and EPA assessment tools were developed and used by the pilot programs.
