Background And Aim: We conducted a retrospective study to identify incidence rates and potential risk factors of major adverse cardiac events (MACE) in Japanese patients with ulcerative colitis (UC), as existing data are scarce, inconsistent, and provide limited representation of the real-world situation of MACE in Japan.

Methods: We utilized administrative claims data, collected between January 2013 and December 2022, from Medical Data Vision, Japan. Patients (aged ≥ 20 years) diagnosed with UC within ± 1 month of the prescription date during the study period were included in the incident cohort.

Article Synopsis
  • * A colonoscopy revealed a significant amount of hard stool in the sigmoid colon, along with ulcers and erosions in the proximal area.
  • * After the procedure, the patient's symptoms improved, but the decline in intestinal function from ischemic enteritis may have led to the obstruction; fecal obstructive enterocolitis is rare, highlighting the need for careful preparation during colonoscopy.

Aplastic Anemia With Non-Occlusive Mesenteric Ischemia.

Cureus

October 2024

Endocrinology and Diabetes, Hinchingbrooke Hospital, Huntingdon, GBR.

This case highlights the multifaceted challenges of managing aplastic anemia, especially when complicated by non-occlusive mesenteric ischemia (NOMI). The patient's clinical course underscores the importance of a conservative, multidisciplinary approach in balancing the risks of invasive procedures with the need for effective diagnostics and treatment. Recognizing the risks associated with pancytopenia, including life-threatening bleeding and infections, is critical in managing such patients.

  • Ischemic colitis (IC) is a condition caused by reduced blood flow to the colon, which can result in severe complications, including the need for surgery.
  • This study reviewed the cases of 141 patients diagnosed with IC at a hospital over nearly 20 years, finding key clinical features and risk factors affecting outcomes.
  • Significant findings included that patients with right-sided colon issues, fever, and lack of bleeding were at greater risk for needing surgery, highlighting the importance of a collaborative treatment approach for these patients.

Association Between SARS-CoV-2 Infection and Acute Ischemic Colitis.

J Inflamm Res

October 2024

Department of Gastroenterology, Ankang Central Hospital, Ankang, Shaanxi Province, People's Republic of China.

Article Synopsis
  • Ischemic colitis is a rare but serious complication observed in about 33.3% of COVID-19 patients who underwent endoscopy, prompting a study to explore its symptoms, treatment, and patient outcomes.
  • The prospective study conducted from December 2022 to January 2023 included 12 hospitalized patients, confirming diagnoses through colonoscopy and biopsies, and followed up for a year via phone interviews.
  • Results indicated that patients typically experienced abdominal pain and bleeding within 2 weeks post-COVID-19, primarily affecting the descending colon, with findings showing no chronic issues after 12 months.

Article Synopsis
  • This study examined the prevalence of gastrointestinal (GI) symptoms in patients with lupus and identified factors linked to increased mortality.
  • Out of 1160 systemic lupus erythematosus (SLE) patients, 91 displayed GI manifestations, with prevalence rates of 8.53% in kids and 7.75% in adults, showing no significant difference between the groups.
  • Key findings highlighted that conditions like ischemic colitis were correlated with longer disease duration and specific autoantibodies, and patients with GI involvement had a higher mortality rate due to complications like sepsis and organ damage.

  • The study investigates the relationship between dependent functional status (DFS) and 30-day outcomes in patients with ruptured and nonruptured abdominal aortic aneurysm (AAA) undergoing endovascular aneurysm repair (EVAR), highlighting the need for differentiated analysis due to previous study limitations.
  • Utilizing data from the American College of Surgeons database between 2012-2022, the researchers compared outcomes by stratifying patients based on their functional status, with a focus on matching those with DFS and independent functional status (IFS) in nonruptured cases.
  • Results showed that nonruptured patients with DFS had similar 30-day mortality rates as those with IFS, but a higher risk of requiring blood transfusions and longer hospital stays

Article Synopsis
  • Hepatic portal venous gas is a rare condition linked to severe bowel issues like ischemic colitis or cancer, as illustrated by a case involving a 72-year-old man with ileocecal cancer and sepsis from Klebsiella pneumoniae.
  • The patient was admitted for diabetes control but developed fever and septic shock without typical abdominal symptoms; imaging revealed gas in the portal vein, indicating a serious underlying infection.
  • Early detection through abdominal ultrasound was crucial, highlighting that hepatic portal venous gas should prompt urgent investigation to address potential infections or complications from underlying conditions.

Comorbidity and Disease Activity in Multiple Sclerosis.

JAMA Neurol

September 2024

Department of Internal Medicine, Max Rady College of Medicine, Rady Faculty of Health Sciences, University of Manitoba, Winnipeg, Manitoba, Canada.

Importance: Multiple studies suggest that comorbidity worsens clinically relevant outcomes in multiple sclerosis (MS), including the severity of disability at diagnosis and rate of disability worsening after diagnosis. However, less is known regarding the association of comorbidity with measures of disease activity, such as relapse rate and magnetic resonance imaging lesion accrual, which are relevant to clinicians and clinical trialists.

Objective: To evaluate the association of comorbidities with disease activity in clinical trials of disease-modifying therapies (DMTs) in populations with MS.

Ischemic colitis is the most common form of ischemic bowel disease; identification of ischemic colitis with PET/CT is rare. We present a case of unexplained ischemic colitis in a 66-year-old man who underwent 18 F-FDG PET/CT to exclude intestinal malignancy and was subsequently recruited in a clinical trial regarding 68 Ga-FAPI. 18 F-FDG PET/CT showed intense activity in the intestinal lumen where mucosa had chronic inflammation.

The use of immune checkpoint inhibitors (ICIs) for the treatment of various malignancies is increasing. Immune-related adverse events can occur after ICI administration, with gastrointestinal adverse events constituting a significant proportion of these events. When ICI-related diarrhea/colitis is suspected, endoscopic evaluation is recommended to differentiate it from other etiologies and assess the severity of colitis.

Background: Distal embolization as the first manifestation of an abdominal aortic aneurysm (AAA) is relatively rare. AAAs presenting with multiple organs embolization are rarer and serious systemic conditions. There are no reports of life-saving outcomes in patients with AAAs presenting with multiple organs embolization prior to surgery.
