[Alternative therapies for choroidal neovessels resulting from age-related macular degeneration].

Classic neovessels (CNVs) identified on fluorescein angiography may benefit from thermal laser photocoagulation when sparing the fovea. If they extend into it, photodynamic therapy may halt the natural progression to a central scotoma. Occult CNVs, when subfoveal, may benefit from photodynamic therapy when isolated (not associated with classic CNV or with a pigment epithelium detachment).

We report a case of a 27-year-old woman with a cirsoid aneurysm, also called congenital arteriovenous communication of the retina. The cirsoid aneurysm, stage I according to Archer's classification, was revealed by a serous retinal detachment. The exudation resolved, and acuity was recovered after a short time, but exudation recurred during the 5 years of follow-up.

Several studies suggest that apoptosis might play a major role in age-related macular degeneration. Apoptosis in retinal pigment epithelium cells undergoing severe oxidative stress has been reported and could therefore be involved in the pathogenesis of AMD. The processes of drusen formation seem to be similar to the processes described in apoptosis.

Photodynamic therapy with Verteporfine (Visudyne Novartis Ophthalmics) achieves a photo occlusion of subfoveal choroidal neovascularization (CNV) resulting from age-related macular degeneration (AMD) in phase 1 and 2 studies. Thereafter, two international randomized clinical trials evaluated the product's efficacy in term of visual acuity. Eyes with neovascular lesions due to AMD of less than 5,400 micro, in size involving the fovea and presenting well-defined CNV on fluorescein angiography could be included when visual acuity was between 5 and 1/10.

Purpose: To study the progression of choroidal neovascularization (CNV) after macular translocation in age-related macular degeneration (AMD) and degenerative myopia.

Patients And Methods: The data from 42 consecutive eyes (28 AMD, and 14 degenerative myopia) operated on by limited macular translocation (DeJuan technique), with a follow-up of 6 months or more, were prospectively analyzed. In the AMD group, neovascularization was classic in 54% of eyes and classic and occult in 46% of eyes.

The prognosis of herpetic keratitis has been dramatically improved during the past years because of a better understanding of their physio-pathologic phenomenons, and mostly because of the use of very efficient antiviral drugs. Superficial keratitis are easily cured in most cases; deep stromal keratitis, that often require corticosteroid treatment, are more difficult to heal. Prophylactic treatment of the recurrences by oral antiviral drugs represents a promising new concept for herpetic keratitis.

Three brothers, with the macular coloboma type Leber's congenital amaurosis aged 10, 8, and 6 years respectively, are described in this report. Examination of the patient provided an opportunity to describe pathophysiology, knowledge on the ornithine aminotransferase gene, and finally the therapeutics used in this disease.

Photodynamic therapy (PDT) is a new approach for subfoveal choroidal new vessels (CNV) in age-related macular degeneration (ARM) and myopia, currently being evaluated in clinical trials. PDT is a two-step procedure: the intravenous perfusion of a photosensitizer is followed by light irradiation at the adapted wavelength. Verteporfin, the photosensitizer under investigation, has a maximum absorption at 692nm.
