Article Synopsis
  • * It includes data from 413 patients (142 HBV-HDV and 271 HBV monoinfected) and finds that those with HBV-HDV are generally younger and experience a significantly higher incidence of HCC over 5 years compared to HBV-only patients.
  • * The results indicate that HDV is an independent risk factor for the development of HCC in patients with cirrhosis, highlighting its important role regardless of HBV

Article Synopsis
  • * Out of 230 patients evaluated, TE showed strong diagnostic accuracy for detecting cirrhosis and advanced fibrosis, with area under the receiver operating characteristic curves of 0.88 and 0.86, outperforming other non-invasive tests.
  • * The findings suggest that LSM values above 10 kPa indicate a high probability of advanced fibrosis, while values below 6 kPa almost completely rule out significant fibrosis, necessitating further discussion for values between 6 and 10 kPa.

Article Synopsis
  • * Among 976 CA patients, the prevalence of AS was notably higher in those with wild type transthyretin amyloid (ATTRwt) at 26%, compared to 8% in hereditary variant transthyretin amyloid (ATTRv) and 5% in immunoglobulin light-chain (AL) CA.
  • * Despite ATTRwt patients having a higher prevalence of AS than hospital controls, moderate or greater AS did not significantly impact overall mortality risk in this group.

Article Synopsis
  • Surgical resection (SR) is a potential cure for hepatocellular carcinoma (HCC), but high recurrence rates pose a challenge, necessitating the development of better risk stratification tools for predicting recurrence.
  • A new scoring system, called the Early Recurrence Score (ERS), was created from data of 2359 patients who underwent SR for HCC, evaluating factors like alpha-fetoprotein levels and tumor characteristics to forecast 2-year recurrence risk.
  • The ERS effectively stratified patients into four risk categories with good accuracy and can help personalize surveillance and therapy plans post-surgery.

How infection by a viral variant showing antigenic drift impacts a preformed mature human memory B cell (MBC) repertoire remains an open question. Here, we studied the MBC response up to 6 months after SARS-CoV-2 Omicron BA.1 breakthrough infection in individuals previously vaccinated with three doses of the COVID-19 mRNA vaccine.

Background: Survivors of severe-to-critical coronavirus disease 2019 (COVID-19) may have functional impairment, radiological sequelae and persistent symptoms requiring prolonged follow-up. This pragmatic study aimed to describe their clinical follow-up and determine their respiratory recovery trajectories, and the factors that could influence them and their health-related quality of life.

Methods: Adults hospitalised for severe-to-critical COVID-19 were evaluated at 3 months and up to 12 months post-hospital discharge in this prospective, multicentre, cohort study.

Background & Aims: Identifying individuals at higher risk of developing hepatocellular carcinoma (HCC) is pivotal to improve the performance of surveillance strategies. Herein, we aimed to evaluate the ability of single nucleotide polymorphisms (SNPs) to refine HCC risk stratification.

Methods: Six SNPs in PNPLA3, TM6SF2, HSD17B13, APOE, and MBOAT7 affecting lipid turnover and one variant involved in the Wnt-β-catenin pathway (WNT3A-WNT9A rs708113) were assessed in patients with alcohol-related and/or HCV-cured cirrhosis included in HCC surveillance programmes (prospective CirVir and CIRRAL cohorts).

Background:  Although a growing number of very elderly patients with atrial fibrillation (AF), multiple conditions, and polypharmacy receive direct oral anticoagulants (DOACs), few studies specifically investigated both apixaban/rivaroxaban pharmacokinetics and pharmacodynamics in such patients.

Aims:  To investigate: (1) DOAC concentration-time profiles; (2) thrombin generation (TG); and (3) clinical outcomes 6 months after inclusion in very elderly AF in-patients receiving rivaroxaban or apixaban.

Methods:  Adage-NCT02464488 was an academic prospective exploratory multicenter study, enrolling AF in-patients aged ≥80 years, receiving DOAC for at least 4 days.

Electronic address:

Background: Caregiver burden is widely recognized in Huntington's disease, but little is known about the factors determining its evolution over time in the absence of longitudinal studies. Our objective was to identify typical patterns of caregiver burden level and evolution using both patients' and caregivers' characteristics over a one-year period to identify potential levers for alleviation.

Methods: We conducted a prospective multicenter longitudinal study in caregiver/patient pairs in Huntington's disease (NCT02876445) between March 2011 and May 2015.

Background & Aims: Reinforced hepatocellular carcinoma (HCC) surveillance using magnetic resonance imaging (MRI) could increase early tumour detection but faces cost-effectiveness issues. In this study, we aimed to evaluate the cost-effectiveness of MRI for the detection of very early HCC (Barcelona Clinic Liver Cancer [BCLC] 0) in patients with an annual HCC risk >3%.

Methods: French patients with compensated cirrhosis included in 4 multicentre prospective cohorts were considered.

Beclin-1 increases with obstructive sleep apnea severity.

Obstructive sleep apnea is a common chronic disorder that leads to chronic intermittent hypoxia described as an important factor contributing to the pathogenesis of OSA-related comorbidities. Besides, recent data suggest that intermittent hypoxia can induce adaptative cardiovascular pathways inducing a relative resistance to ischemic insults. Adaptative pathways induced by hypoxia could implicate autophagic processes and Beclin-1, one of the first mammalian autophagy effectors.

Recommended preventive strategies before kidney transplantation include screening and treatment of latent tuberculosis infection (LTBI), and updating of the recommended vaccines. We prospectively evaluated in dedicated infectious diseases consultations, from 2014 to 2018, the clinical and vaccination data of new adult kidney allograft candidates. Patients were offered an updated vaccination schedule, if appropriate, and were screened for LTBI using chest imaging and interferon gamma release assay (IGRA).

Electronic address:

Article Synopsis
  • Memory B cells are crucial for fighting viruses, but their specific role in SARS-CoV-2 infection has been unclear until now.
  • A study tracked the B cell response in COVID-19 patients for 6 months, finding that early B cell activation led to strong antibody production and ongoing immune response.
  • The research shows that specific memory B cells evolved with somatic mutations over time, indicating a persistent immune activation that could help provide long-term protection against SARS-CoV-2.

Background: Direct oral anticoagulants (DOACs) account for an increasing proportion of prescriptions in patients with non-valvular atrial fibrillation (NVAF) in primary care. Inappropriate dosing of DOACs is a common problem, with under-dosing being a particular issue. However, conflicting results have been reported about the factors independently associated with inappropriate dosing.

Article Synopsis
  • This research aims to assess the effectiveness of a multiprofessional education program in reducing adverse events (AE) in neonatal intensive care units (NICUs), recognizing the significance of patient safety.
  • The study employs a stepped-wedge cluster randomized controlled trial design over 20 months, involving 12 NICUs in France where different education interventions will be implemented in a sequential manner.
  • The expected outcome is a reduction of AEs from 60 to 50 per 1000 patient-days, with AE rates serving as the primary metric for evaluating the intervention's success.

Increased emergency medical services (EMS) response times and areas of low socioeconomic status are both associated with poorer outcomes for several time-sensitive medical conditions attended to by medical personnel before a patient is hospitalized. We evaluated the association between EMS response times, area deprivation level, and on-scene access constraints encountered by EMS in a large urban area in France. We conducted a multicenter prospective cohort study of EMS dispatches occurring in the forty-seven cities in a region southeast of Paris.
