Osteoarthritis (OA) is a joint disease characterized by articular cartilage degradation. Persistent low-grade inflammation defines OA pathogenesis, with crucial involvement of pro-inflammatory M1-like macrophages. While mesenchymal stromal cells (MSC) and their small extracellular vesicles (sEV) hold promise for OA treatment, achieving consistent clinical-grade sEV products remains a significant challenge.

Overweight/obesity (OW/OB) have been associated with gut dysbiosis, changes in gastrointestinal motility and sedentary behavior, contributing to metabolic and inflammatory alterations. This systematic review aims to assess the evidence supporting the influence of physical activity and exercise on gut microbiota composition and diversity in OW/OB and was conducted according to PRISMA guidelines, using Medline, Embase, Ebsco and Scopus databases. Risk of bias was assessed with RoB 2 for randomized controlled trials (RCT), ROBINS-I for non-RCT and JBI Critical Appraisal tool for cross-sectional studies.

Liver X receptor unlinks intestinal regeneration and tumorigenesis.

Nature

November 2024

Division of Immunology and Respiratory Medicine, Department of Medicine Solna, Karolinska Institutet and University Hospital, Stockholm, Sweden.

Article Synopsis
  • Uncontrolled regeneration in the intestinal epithelium can lead to cancer, highlighting the need for precise regulation during tissue renewal.
  • Researchers discovered that activating the liver X receptor (LXR) pathway helps balance intestinal regeneration and tumor growth after damage.
  • LXR activation enhances regeneration by producing amphiregulin and is crucial for controlling tumor growth, with diminished LXR-related activity found in human colorectal cancer samples.

Pharmacologic interventions for primary glenohumeral osteoarthritis.

JAAPA

July 2024

Ausberto Velasquez Garcia practices orthopedic surgery at the Clinica Universidad de los Andes in Chile and is a research fellow at the Mayo Clinic in Rochester, Minn. Liborio Ingala Martini practices orthopedic surgery at IVSS Dr. Luis Ortega Hospital and the Hospital Clinicas del Este, both in Porlamar, Venezuela. Andres Franco Abache practices orthopedic surgery at the Hospital de Especialidades Guayaquil MSP in Guayaquil, Ecuador. The authors have disclosed no potential conflicts of interest, financial or otherwise.

Primary glenohumeral osteoarthritis is a multifactorial condition with a complex cause that affects patients across different age groups, impairing physiologic and psychologic well-being, and substantially reducing patient quality of life and overall productivity. To effectively manage this condition, healthcare providers need to be well informed about treatment guidelines, as well as the available therapeutic options and the evidence supporting their use. Nonsurgical interventions should be regarded as the primary treatment option, particularly for patients in the initial phases of this condition.

Radiofrequency Echographic Multi Spectrometry (REMS) is a radiation-free, portable technology, which can be used for the assessment and monitoring of osteoporosis at the lumbar spine and femoral neck and may facilitate wider access to axial BMD measurement compared with standard dual-energy x-ray absorptiometry (DXA).There is a growing literature demonstrating a strong correlation between DXA and REMS measures of BMD and further work supporting 5-year prediction of fracture using the REMS Fragility Score, which provides a measure of bone quality (in addition to the quantitative measure of BMD).The non-ionising radiation emitted by REMS allows it to be used in previously underserved populations including pregnant women and children and may facilitate more frequent measurement of BMD.
