Vulvovaginal Atrophy Following Treatment for Oncogynecologic Pathologies: Etiology, Epidemiology, Diagnosis, and Treatment Options.

Medicina (Kaunas)

September 2024

Clinic of Obstetrics and Gynaecology, Faculty of Medicine, Institute of Clinical Medicine, Vilnius University, LT-03101 Vilnius, Lithuania.

Vulvovaginal atrophy, characterized by the thinning of vaginal mucosa typically resulting from reduced estrogen levels, is frequently exacerbated by oncogynecologic treatments such as chemotherapy, hormonal therapy, radiotherapy, or surgery. This condition significantly impacts the quality of life for cancer survivors, leading to persistent discomfort, heightened infection risk, and negative effects on sexual function and self-esteem. Despite being a relatively common complication, vulvovaginal atrophy is not always discussed before the start of treatment.

Background: Despite the widespread prevalence of obesity and its potential adverse impacts on health, the majority of interventions aimed at weight loss stay ineffective. This study aimed to assess illness perception in people with overweight/obesity and its impact on bio-functional age (BFA) and cognitive patterns governing eating and movement behavior.

Methods: A total of 40 subjects from the original overweight/obesity subcohort of the Bern Cohort Study 2014 (BeCS14) were included and assessed for a follow-up from 2019-11-29 to 2020-07-14.

  • The study aimed to assess the effectiveness of virtual autopsy using 7 Tesla MRI compared to traditional autopsy for identifying structural abnormalities in second-trimester fetuses (13-19 weeks gestation).
  • A total of 30 fetuses were analyzed, and the results showed that virtual autopsy had a high sensitivity (92.04%) and specificity (97.87%) for detecting anatomical issues, achieving an overall diagnostic accuracy of 95.68%.
  • The findings indicated that virtual autopsy performs particularly well in examining the nervous system compared to classical autopsy when evaluating small fetuses.

An ovarian Sertoli-Leydig cell tumor is a rare type of sex cord-stromal tumor of the ovary. Typically, it presents as abdominal pain or androgenic manifestations in women in the second to third decade of life. While cases of ovarian Sertoli-Leydig cell tumor associated with increased levels of alpha-fetoprotein are rare, they are reported to be the most common alpha-fetoprotein-producing ovarian non-germ cell tumor.

Article Synopsis
  • - Since the beginning of the war in Ukraine, many women and children have fled to Poland, making vaccination of these refugees crucial to prevent disease.
  • - A study tested different messaging strategies to encourage Ukrainian mothers to vaccinate their children, finding that a message focused on the risks of not vaccinating significantly increased their intention to vaccinate.
  • - Key factors influencing vaccination intention included the perceived importance of vaccination and trust in information from official health sources and social media, suggesting that targeted communication can effectively promote health measures among refugees.

Article Synopsis
  • A descriptive study aimed to analyze pregnant women with cancer who died during pregnancy, delivery, or within the first year postpartum, using data from the International Network on Cancer, Infertility and Pregnancy (INCIP) registry.
  • Out of 2,359 registered women, 131 (5.6%) experienced maternal mortality, with lung, gastro-oesophageal cancer, and acute leukaemia showing the highest mortality rates.
  • The findings indicated that maternal mortality was linked to fewer live births, more elective caesarean sections, and earlier deliveries, resulting in an increased incidence of preterm births.

Article Synopsis
  • The study examines how metabolic factors, particularly the triglyceride-glucose (TyG) index and visceral adiposity index (VAI), affect male sexual function and erectile dysfunction (ED) in men with sexual dysfunction.
  • Higher TyG and VAI levels were linked to an increased risk of ED, with the presence of metabolic syndrome (MetS) further impairing aspects of sexual function.
  • The research suggests that TyG and VAI could serve as useful tools for predicting sexual dysfunction, emphasizing the importance of early detection and management of metabolic disorders to improve male sexual health.

Pregnancy is generally studied as a biological interaction between a mother and a fetus; however, the father, with his characteristics, lifestyle, genetics, and living environment, is by no means unrelated to the outcome of pregnancy. The half of the fetal genetic heritage of paternal derivation can be decisive in cases of inherited chromosomal disorders, and can be the result of de novo genetic alterations. In addition to the strictly pathological aspects, paternal genetics may transmit thrombophilic traits that affect the implantation and vascular construction of the feto-placental unit, lead to placenta-mediated diseases such as pre-eclampsia and fetal growth retardation, and contribute to the multifactorial genesis of preterm delivery.

Osteogenesis imperfecta (OI), also known as brittle bone disease, belongs to a rare heterogeneous group of inherited connective tissue disorders. In experienced prenatal centers, severe cases of OI can be suspected before birth from the first trimester prenatal ultrasound screening. In this article, we describe a case report of OI suspected at the 26th week of gestation and the patient's outcomes in infancy one year after birth, as well as compare our case to other prenatally or soon-after-birth suspected and/or diagnosed OI clinical case reports in the literature.

Article Synopsis
  • Congenital cytomegalovirus (cCMV) infection during pregnancy poses serious risks for fetal health, often detected through ultrasound and MRI.* -
  • A review followed the PRISMA protocol to analyze studies linking CMV infection with sonographic findings, identifying key fetal features such as abnormal periventricular echogenicity and ventriculomegaly.* -
  • Ultrasonography is effective in spotting fetal anomalies due to cCMV, but signs may overlap with other viral infections, requiring more research to differentiate them; targeted fetal neurosonography is highly recommended.*
