Microprolactinoma Growth During Pregnancy With Pituitary Tumor Apoplexy: Case Report and Review of the Literature.

Case Rep Endocrinol

January 2025

Unidad Académica de Endocrinología y Metabolismo, Hospital de Clínicas "Dr. Manuel Quíntela", Facultad de Medicina, Universidad de la República, Montevideo, Uruguay.

Prolactinomas are the most prevalent subtype of pituitary adenomas and represent one of the leading etiological factors responsible for amenorrhea and infertility in women. The primary therapeutic approach entails the use of dopamine agonists, which effectively restore fertility. In cases of microprolactinomas, the likelihood of experiencing a symptomatic enlargement of the tumor during pregnancy is exceptionally low, estimated at a mere 2.

J Clin Psychopharmacol

December 2024

Human Molecular Genetics Laboratory, Institut Pasteur de Montevideo, Montevideo, Uruguay.

Purpose/background: Clozapine is the recommended drug for treatment-resistant schizophrenia. Drug response could be affected by numerous factors such as age, sex, body mass index, co-medication, consumption of xanthine-containing beverages, smoking, and genetic variants of the enzymes involved in clozapine metabolism (CYP1A2, CYP3A4, and, to a lesser extent, CYP2C19 and CYP2D6). This study evaluated genetic and nongenetic variables that may affect clozapine plasma concentrations in Uruguayan patients with schizophrenia.

Antimalarials in Lupus Nephritis: How Strong Is the Evidence?

Kidney360

December 2024

Division of Nephrology and Hypertension, Mayo Clinic, Rochester, Minnesota, United States of America.

SLE is a chronic multisystem autoimmune disease that affects the kidneys in approximately 50% of patients, with the prevalence rising to as high as 70% in certain populations, such as African American and Asian people. Antimalarials-and particularly hydroxychloroquine (HCQ)-are currently considered a mainstay of therapy, together with immunosuppressants. Over the past decades, several studies have extensively investigated the mechanisms of action of antimalarial agents and their potential beneficial properties in patients with SLE in general.

Article Synopsis
  • A retrospective study of 183 adult patients treated with rituximab for podocytopathies showed that 82% achieved complete or partial remission within 6 months.
  • *Long-term follow-up revealed that 55% of initial responders maintained relapse-free survival over three years, with improved outcomes linked to maintenance therapy.
  • *Patients on maintenance therapy experienced significantly fewer relapses per year and maintained stable kidney function compared to non-responders.

Assessing the Relevance of Non-molecular Prognostic Systems for Myelodysplastic Syndrome in the Era of Next-Generation Sequencing.

Ann Lab Med

January 2025

Laboratorio de Genética Hematológica, Instituto de Medicina Experimental (IMEX-CONICET)/Academia Nacional de Medicina, Ciudad de Buenos Aires, Argentina.

Background: The Molecular International Prognostic Scoring System (IPSS-M) has improved the prediction of clinical outcomes for myelodysplastic syndromes (MDS). The Artificial Intelligence Prognostic Scoring System for MDS (AIPSS-MDS), based on classical clinical parameters, has outperformed the IPSS, revised version (IPSS-R). For the first time, we validated the IPSS-M and other molecular prognostic models and compared them with the established IPSS-R and AIPSS-MDS models using data from South American patients.

Impact of the COVID-19 Pandemic on Global TAVR Activity: The COVID-TAVI Study.

JACC Cardiovasc Interv

February 2024

Department of Cardiology, Galway University Hospital, Galway, Ireland; School of Medicine, University of Galway, Galway, Ireland. Electronic address:

Article Synopsis
  • The COVID-19 pandemic led to significant treatment delays for patients needing transcatheter aortic valve replacement (TAVR), with a marked decrease in procedure volumes during the first two waves of the pandemic.
  • The study analyzed TAVR case data from 130 centers in 61 countries, finding a 15% drop in cases during the first wave and 7% in the second, particularly affecting regions like Africa, Central-South America, and Asia.
  • Factors such as hospital type (private vs public), urban vs rural location, low procedure volumes, a country's socioeconomic status, and stringent public health measures contributed to these reductions, highlighting the need for improved public health policies for future health crises.

Background And Objectives: The Woven EndoBridge (WEB) device has been increasingly used to treat wide-neck aneurysms showing a safe and effective profile, but a relatively high number of thromboembolic events (TEEs) have been reported with such treatment. We aimed to evaluate the incidence and management of TEEs and possible predictive factors related to WEB embolization of ruptured and unruptured intracranial aneurysms.

Methods: A single-center database with consecutive aneurysms treated with a WEB device between July 2012 and May 2022 was reviewed for intraoperative and delayed TEEs.

Article Synopsis
  • Waldenström macroglobulinemia (WM) is a rare and slow-growing cancer that affects B-cells and is marked by high levels of a specific type of antibody (IgM), along with abnormal cell growth in lymph nodes and bone marrow.
  • Diagnosis methods and access to new treatment options are lacking in many areas, particularly in lower-resource settings, which complicates patient care.
  • Older medications like bendamustine and cyclophosphamide are still used for treating WM, and there is a need for more studies in resource-limited regions to improve diagnosis and treatment strategies.

September 2022

Departamento Básico de Medicina, Hospital de Clínicas Dr. Manuel Quintela, Facultad de Medicina, Universidad de la República, Montevideo, Uruguay.

Introduction: Regulatory T Cells (Tregs) play an important role in carcinogenesis and tumor immunoediting by preventing the development of effective antitumor immunity. Several reports showed that circulating Tregs are increased in patients with solid tumors, including lung cancer. Treg population could be categorized into "naive," "effector," and "memory" subtypes, bearing potential unique functions.
