Article Synopsis
  • Prompt treatment of sepsis and septic shock is crucial to reducing mortality risk, and tools like alerts and rapid response teams are often used to speed up care, though their effectiveness is not well understood.
  • A study assessed an Inpatient Code Sepsis protocol at a hospital over three years to determine its impact on 30-day mortality and other outcomes in patients with sepsis.
  • The findings showed no significant change in mortality rates after implementing the protocol, but there was a notable decrease in the time it took to administer antibiotics.

Functional and postoperative outcomes in ideal pouch-anal anastomosis in patients with parkinson disease and multiple sclerosis.

Langenbecks Arch Surg

October 2024

Department for Colorectal Surgery, Digestive Disease Institute, Cleveland Clinic Main Campus, 9500 Euclid Ave, A30, Cleveland, OH, 44122, USA.

Introduction: Patients with multiple sclerosis and Parkinson's disease may experience pelvic floor dysfunction and constipation which can affect ileoanal pouch emptying. This can lead to complications such as pouchitis, pouch dysfunction, and failure. We hypothesized that patients with neurological diseases have a higher rate of pouch failure and complications than healthy controls.

Article Synopsis
  • The introduction of dual-energy X-ray absorptiometry (DXA) in the 1980s transformed how osteoporosis is diagnosed and managed globally.
  • DXA results can be impacted by various technical factors, including instrument quality, patient positioning, and the interpretation of T-scores and Z-scores.
  • This report aims to highlight important technical and clinical elements of DXA use and discusses how to better integrate these practices into clinical settings, along with considering non-bone mineral density measurements.

: Although heartburn and reflux are frequently reported in ulcerative colitis [UC], the correlation between UC and gastroesophageal reflux disease [GERD], and its complications, esophageal stricture and Barrett's esophagus [BE], is not well understood. This study aims to examine the prevalence and associated risk of GERD and its complications within the UC population. : We analyzed the National Inpatient Sample (NIS) dataset, consisting of 7,159,694 patients, comparing GERD patients with and without UC to those without GERD.

Transanal circumferential pouch advancement for treatment of pouch vaginal fistulae.

Tech Coloproctol

August 2024

Department of Colon and Rectal Surgery, Digestive Disease and Surgery Institute, Cleveland Clinic Main Campus, 9500, Cleveland, OH, 44122, USA.

Background: Ileal pouch anal anastomosis (IPAA) circumferential pouch advancement (CPA) involves full-thickness transanal 180-360° dissection of the distal pouch, allowing the advancement of healthy bowel to cover the internal opening of a vaginal fistula. We aimed to describe the long-term outcomes of this rare procedure.

Methods: Patients with IPAA who underwent transanal pouch advancement for any indication between 2009 and 2021 were included.

To evaluate radiation exposure in standard interventional radiology procedures using a twin robotic X-ray system compared to a state-of-the-art conventional angiography system. Standard interventional radiology procedures (port implantation, SIRT, and pelvic angiography) were simulated using an anthropomorphic Alderson RANDO phantom (Alderson Research Laboratories Inc. Stamford, CT, USA) on an above-the-table twin robotic X-ray scanner (Multitom Rax, Siemens Healthineers, Forchheim, Germany) and a conventional below-the-table angiography system (Artis Zeego, Siemens Healthineers, Forchheim, Germany).

Predictive model for iris melanoma.

Br J Ophthalmol

October 2024

Department of Quantitative Health Sciences & Taussig Cancer Institute, Cleveland Clinic Main Campus Hospital, Cleveland, Ohio, USA.

Article Synopsis
  • A study aimed to create a predictive model for diagnosing iris melanoma by analyzing data from 100 confirmed cases of melanoma and 112 cases of iris naevus.
  • Researchers collected patient demographics, clinical features, and tumor characteristics and used Lasso logistic regression for analysis.
  • The model identified key indicators of melanoma, achieving an optimism-corrected AUC of 0.865, and suggested that a prediction calculator app could improve diagnosis and patient communication, with potential for further improvement using more data.
