What Do I Do if the Valve Don't Pass? Use a Snare Catheter!

Clin Case Rep

December 2024

Department of Cardiology Clermont-Ferrand University Hospital Center, CNRS, Clermont Auvergne University Clermont-Ferrand France.

We report a case of a complex transcatheter aortic valve implantation (TAVI) complicated by severe calcifications, which prevented the delivery system from advancing through the aortic valve. To address this challenge, we employed an innovative solution using a Snare catheter. This approach enabled stabilization and guidance of the delivery system, facilitating the crossing of the calcified obstruction and the successful completion of the procedure.

Background: The onset of hematological malignancies can lead to acute and critical situations. It can also result in adverse outcome despite the significant advancements made in their therapeutic management. In this context, advance care planning and, in particular, advance directives (AD) play an essential role.

Article Synopsis
  • A study examined the cancer risk in people with multiple sclerosis (PwMS) compared to a matched sample from the general French population over a 10-year period, using national health data.
  • Among the 140,649 PwMS, the incidence of cancer was found to be slightly higher (799 per 100,000 person-years) than that of the matched controls (736 per 100,000 person-years), indicating a small overall increased risk for PwMS.
  • The risk for different cancer types varied, with higher risks for bladder, brain, and cervical cancers, while prostate, breast, and colorectal cancers showed a lower risk in PwMS.

Department of Pediatric Neurology, French Greater South‒West Reference Center for Neuromuscular Diseases, Hôpital des Enfants, University Hospital Center Toulouse, 330 av de Grande Bretagne-TSA, 31059, Toulouse, France.

Article Synopsis
  • Spinal muscular atrophy type 1 (SMA1) is a severe genetic disease affecting motor neurons, and onasemnogene abeparvovec gene transfer therapy (GT) has significantly impacted its treatment, although real-world data is limited.
  • A study in France identified 95 SMA1 patients between June 2019 and June 2022, focusing on 29 who received GT and had over a year of follow-up.
  • Results indicated positive motor development and maintenance of respiratory and feeding functions in treated infants, although many developed spinal deformities, and two patients sadly passed away shortly after treatment.

Article Synopsis
  • * Out of 410 oncologists surveyed in France, 75% viewed AD as relevant, but only 36% regularly inform their patients about them, often due to concerns that AD discussions may cause anxiety.
  • * There is a notable division among oncologists regarding the usefulness and safety of AD, with many preferring to designate a "person of trust" over formal AD.

Background: Cardiogenic shock (CS) is the most severe form of acute heart failure. Discrepancies have been reported between sexes regarding delays, pathways and invasive strategies in CS complicating acute myocardial infarction. However, effect of sex on the prognosis of unselected CS remains controversial.

Electronic address:

Objectives: To compare the direct and indirect medical costs for patients with suspected Lyme borreliosis according to whether or not they had used an informal care pathway.

Patients And Methods: We retraced the care pathways of participating patients by a prospective questionnaire survey and a retrospective analysis of care records. Direct and indirect costs were estimated using a micro-costing method from different perspectives.

Article Synopsis
  • This study aimed to assess the effectiveness and safety of ethanol lock therapy (ELT) versus vancomycin lock therapy (VLT) for treating infections in totally implantable venous access devices (TIVAD) caused by coagulase-negative staphylococci.
  • Results showed a treatment success rate of 58.1% for the ELT group and 46.7% for the VLT group, with no significant difference in failure rates between the two therapies.
  • The study concluded that both therapies had high treatment failure rates, and emphasizes that removing the TIVAD device is crucial to avoid complications, except in rare cases.

Axicabtagene ciloleucel (axi-cel) demonstrated superior efficacy compared to standard of care as second-line therapy in patients with high-risk relapsed/refractory (R/R) large B cell lymphoma (LBCL) considered eligible for autologous stem cell transplantation (ASCT); however, in clinical practice, roughly half of patients with R/R LBCL are deemed unsuitable candidates for ASCT. The efficacy of axi-cel remains to be ascertained in transplant-ineligible patients. ALYCANTE, an open-label, phase 2 study, evaluated axi-cel as a second-line therapy in 62 patients with R/R LBCL who were considered ineligible for ASCT.

Vitamin D deficiency contributes to overtraining syndrome in excessive trained C57BL/6 mice.

Scand J Med Sci Sports

November 2023

Human Nutrition Unit, UMR 1019 INRAE/UCA, CRNH-Auvergne, Clermont-Ferrand, France.

Overtraining syndrome is a condition resulting from excessive training load associated with inadequate recovery and poor sleep quality, leading to performance decrements and fatigue. Here we hypothesized that vitamin D (VitD) deficiency is a lead factor in the development of the overtraining syndrome. To test this hypothesis, two groups of 60-week-old C57BL/6 mice followed a 16-week excessive eccentric-based overtraining by excessive downhill running with or without dietary VitD depletion (EX and EX-D groups).

Human Nutrition Unit, INRAE, Auvergne Human Nutrition Research Center, Clermont Auvergne University, 63000 Clermont-Ferrand, France.

Aging is associated with a progressive loss of skeletal muscle mass and function termed sarcopenia. Various metabolic alterations that occur with aging also increase the risk of undernutrition, which can worsen age-related sarcopenia. However, the impact of undernutrition on aged skeletal muscle remains largely under-researched.

Schizophrenia is a serious mental illness that requires continuous and effective long-term management to reduce symptoms, improve quality of life, and prevent relapse. Oral antipsychotic medications have proven efficacy for many patients taking these medications; however, a considerable number of patients continue to experience ongoing symptoms and relapse, often due to lack of adherence. The advent of long-acting injectable (LAI) formulations of antipsychotic medications provided an opportunity to improve treatment adherence and overall patient outcomes.

Introduction: During pregnancy, maternal obesity increases the risk of fetal abnormalities. Despite advances in ultrasound imaging, the assessment of fetal anatomy is less thorough among these women. Currently, the construction of ultrasound images uses a conventional ultrasound propagation velocity (1540 m/s), which does not correspond to the slower speed of propagation in fat tissue.
