Background: The risk for recurrence in patients with distal gastric cancer can be reduced by surgical radicality. However, dispute exists about the value of the proposed minimum proximal margin distance (PMD). Here, we assess the prognostic value of the safety distance between the proximal resection margin and the tumor.

: International guidelines recommend transarterial chemoembolization (TACE) for intermediate-stage hepatocellular carcinoma (HCC). However, it is used outside these recommendations and has proven beneficial in prolonging survival. Since the role of TACE outside BCLC stage B is unclear, the present study analyzed the results of TACE performed at a tertiary center in Switzerland for different treatment groups, and aims to highlight the treatment outcomes for these groups.

Article Synopsis
  • - Teratomas are rare germ cell tumors usually found in children, and a unique case of retroperitoneal papillary thyroid carcinoma in an adult woman is discussed here.
  • - A 41-year-old woman experienced intermittent abdominal pain, leading to the discovery of a retroperitoneal mass that turned out to be papillary thyroid carcinoma after surgical removal.
  • - Diagnosing primary retroperitoneal teratomas can be challenging due to vague symptoms and limited imaging findings; surgery is the main treatment, but decisions on further treatments like total thyroidectomy and radioiodine therapy remain debated.

Background: Anastomotic leakage (AL), a severe complication following colorectal surgery, arises from defects at the anastomosis site. This study evaluates the feasibility of predicting AL using machine learning (ML) algorithms based on preoperative data.

Methods: We retrospectively analyzed data including 21 predictors from patients undergoing colorectal surgery with bowel anastomosis at four Swiss hospitals.

Background: With an increase in robot-assisted surgery across all specialties, adequate training and credentialing strategies need to be identified to ensure patients safety. The meta-analysis assesses the transferability of technical surgical skills between laparoscopic surgery, open surgery, and robot-assisted surgery.

Design: A systematic search was conducted in Medline, Cochrane Central Register of Controlled Trials, and Web of Science.

One-anastomosis gastric bypass (OAGB) has gained importance as a simple, safe, and effective operation to treat morbid obesity. We previously found that Roux-en-Y gastric bypass surgery with a long compared with a short biliopancreatic limb (BPL) leads to improved weight loss and glucose tolerance in obese mice. However, it is not known whether a long BPL in OAGB surgery also results in beneficial metabolic outcomes.

Synovial sarcoma is a rare and highly malignant soft tissue sarcoma. The inconspicuous and diversity of its early symptoms make it a highly misdiagnosed disease. The management of synovial sarcomas is challenging as they are rare and have a poor prognosis.

Article Synopsis
  • The study aimed to assess the effectiveness of a virtual reality (VR) environment using a head-mounted display for making decisions in liver surgery compared to traditional 2D and 3D imaging methods.
  • Medical students were divided into three groups, analyzing liver surgery cases with either 2D-tomography, 3D visualizations, or in a VR environment, with results indicating that both 3D and VR groups outperformed the 2D group in terms of correct answers and speed of response.
  • The VR environment was particularly praised for its ability to help identify anatomical anomalies and improve the understanding of complex surgical information during operations.

Educational challenges and opportunities for the future generation of surgeons: a scoping review.

Langenbecks Arch Surg

March 2024

Department of Visceral Surgery, Clarunis - University Digestive Healthcare Center Basel, University Hospital Basel and St. Clara Hospital, Basel, Switzerland.

Purpose: Surgery offers exciting opportunities but comes with demanding challenges that require attention from both surgical program administrators and aspiring surgeons. The hashtag #NoTrainingTodayNoSurgeonsTomorrow on 𝕏 (previously Twitter) underscores the importance of ongoing training. Our scoping review identifies educational challenges and opportunities for the next generation of surgeons, analyzing existing studies and filling gaps in the literature.

Background: Vision is an important and defining element of laparoscopy and significantly affects the outcome of surgery in terms of time, error, and precision. Several new imaging systems have become available for laparoscopic surgery, including three-dimensional (3D) high-definition (HD) and two-dimensional (2D) ultra-high-resolution (4K) monitors. 3D HD systems offer a number of potential benefits to surgeons and patients over traditional 2D systems, including reduced operating time, blood loss, and hospital stay.

February 2024

Department of Biomedical Engineering, Faculty of Medicine, University of Basel, Allschwil, Switzerland.

Background: The integration of virtual reality (VR) in surgery has gained prominence as VR applications have increased in popularity.

Methods: A scoping review was undertaken, gathering the most relevant sources, utilising a detailed literature search of medical and academic databases including EMBASE, PubMed, Cochrane, IEEE, Google Scholar, and the Google search engine.

Results: Of the 18 articles included, 7 focused on VR in colon surgery, 5 addressed VR in pancreas surgery, and the remaining 6 concentrated on VR in liver surgery.

Recurrence poses a notable challenge after hepatocellular carcinoma (HCC) treatment, impacting more than 70% of patients who undergo surgical resection. Recurrence stems from undetected micro-metastasis or de novo cancer, potentially triggered by postsurgical liver regeneration. Prior research employed HCC cell lines in orthotopic models to study the impact of liver regeneration, but their limited validity prompted the need for a more representative model.

  • * A study using a consensus method involved 78 bariatric surgeons globally, who agreed on 54 statements regarding when to use MBS for Class I and II obesity, determining a 70% agreement threshold.
  • * The experts concluded that MBS is cost-effective for Class II obesity and Class I obesity with inadequate weight loss from non-surgical methods, and identified specific surgical options including intra-gastric balloon, endoscopic sleeve gastroplasty, sleeve gastrectomy, Roux-en-Y gastric bypass, and one

Article Synopsis
  • Pancreatic ductal adenocarcinoma (PDAC) is expected to become the second leading cause of cancer-related deaths by 2030, with very low overall survival rates of less than 10%.
  • Despite advancements in surgery and chemotherapy, many patients face limited treatment options, leading to interest in local ablative techniques that improve local tumor control and survival for locally advanced pancreatic cancer (LAPC).
  • These local ablations not only reduce tumor size but also stimulate antitumor immune responses, which may help in preventing or treating metastases, making them a potential therapeutic strategy for metastatic PDAC.

Background: After esophagectomy, the postoperative rate of anastomotic leakage is up to 30% and is the main driver of postoperative morbidity. Contemporary management includes endoluminal vacuum sponge therapy (EndoVAC) with good success rates. Vacuum therapy improves tissue perfusion in superficial wounds, but this has not been shown for gastric conduits.

Article Synopsis
  • The study investigates the link between low levels of intact parathyroid hormone (iPTH) after total thyroidectomy and the risk of hypocalcemia (low calcium levels) post-surgery.
  • Data from 607 patients who underwent thyroidectomy from 2010 to 2022 were analyzed, showing that 32.5% had decreased iPTH levels, with 20.4% developing hypocalcemia on the first day after surgery.
  • Results indicate that a significant drop in iPTH, especially levels below 10 pg/ml, correlates with a higher likelihood of postoperative hypocalcemia, suggesting iPTH measurement can help predict this complication.
