November 2024

Department of Research, U. N. Mehta Institute of Cardiology and Research Centre (UNMICRC), Civil Hospital Campus, Gujarat - Índia.

Background: The optimal treatment for ischemic mitral regurgitation (IMR) in patients of non-ST elevation myocardial infarction (NSTEMI) is a debated topic.

Objective: To evaluate the long term outcome on patients with NSTEMI and IMR, particularly emphasizing the comparison of treatments in those with moderate to severe MR.

Methods: We enrolled patients with NSTEMI and classified non/trivial to mild regurgitation as insignificant IMR and moderate to severe regurgitation as significant IMR.

The midterm outcomes of coronary artery bypass grafting for adult anomalous origin of the left coronary artery from the pulmonary artery.

Indian J Thorac Cardiovasc Surg

January 2025

Department of Cardiovascular & Thoracic Surgery, U. N. Mehta Institute of Cardiology and Research Center, Civil Hospital Campus, Asarwa, Ahmedabad, 380016 Gujarat India.

Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital heart disease. It presents as myocardial infarction in children; however, the presentation may vary between asymptomatic to progressive heart failure and also death. As a result, diagnosis in living adults is extremely rare.

Introduction Fixed-dose combinations (FDCs) have the potential in glaucoma management to improve efficacy due to the complementary mechanism of action of the drugs as well as compliance while reducing adverse effects by minimizing exposure to preservatives and the financial burden on the patients. FDC of brinzolamide/timolol has demonstrated efficacy and safety in multinational phase 3 studies in primary open-angle glaucoma (POAG) and ocular hypertension. However, efficacy and safety in the Indian population are not known.

Background: While recommending a human papillomavirus (HPV) single-dose vaccination schedule in 2022, the World Health Organization highlighted the need for long-term follow-up studies to monitor waning of protection. We report on vaccine efficacy against HPV infections in 1-, 2-, and 3-dose schedules and protection against cervical precancers at a median follow-up of 12 years postvaccination.

Methods: This randomized multicenter study in India was originally designed to vaccinate unmarried girls aged 10-18 years with either 2 or 3 doses of quadrivalent HPV vaccine.

Article Synopsis
  • Restoring lateral lip defects while maintaining mouth function and aesthetics has been a significant challenge, but a study from 2006 to 2022 explored a modified McGregor fan flap technique for reconstruction.
  • The study involved 126 flap procedures on 122 patients with lower lip defects between 30-100% in size, with a notable success in vermilion reconstruction and no complications reported.
  • All patients achieved normal mouth opening within three months, and the technique is considered reliable for moderate to large defects, though different methods may be needed for cases with buccal mucosa involvement.

Eur Heart J Case Rep

August 2024

Department of Cardiology, U. N. Mehta Institute of Cardiology and Research Centre (UNMICRC), Civil Hospital Campus, Asarwa, Ahmedabad 380016, Gujarat, India.

Article Synopsis
  • Ruptured sinus of Valsalva (RSOV) is a rare heart condition mainly affecting the right coronary sinus or the non-coronary sinus, typically resulting in ruptures into right-sided heart chambers.
  • A 24-year-old male patient exhibited symptoms like fatigue and chest pain, with diagnostic imaging revealing multiple sinus of Valsalva aneurysms affecting both the right and left coronary sinuses, necessitating successful surgical repair.
  • The involvement of multiple sinuses and ruptures into the interventricular septum are very rare, with significant risks like complete heart block and sudden death, highlighting the importance of thorough diagnostic methods and timely treatment.

Permacath: wrong pathway leading to a correct destination.

Indian J Thorac Cardiovasc Surg

September 2024

Department of Cardiovascular & Thoracic Surgery, U. N. Mehta Institute of Cardiology and Research Center, Civil Hospital Campus, Asarwa, Ahmedabad, Gujarat 380016 India.

Permacath insertion using ultrasound has drastically reduced complications than those associated with blind puncture. We hereby report a case of entry of the permacath into the right femoral vein after through and through puncture of the right femoral artery. Use of real-time ultrasound, early clinical suspicion, timely diagnosis, and emergency surgery can avoid grave complications.

Department of Research, U. N. Mehta Institute of Cardiology and Research Centre (UNMICRC), Civil Hospital Campus, Asarwa, Ahmedabad, 380016, Gujarat, India. Electronic address:

Background: Electrocardiographic diagnosis of causes of supraventricular tachycardia (SVT) is sometimes difficult and application of routine algorithms can lead to misdiagnosis in as many as 37 % of patients. ST segment depression may be useful in diagnosing the nature of SVT.

Methods: We reviewed surface electrocardiogram (ECG) characteristics of 300 patients having SVT with 1:1 AV relationship and correlated findings with electrophysiology study (EPS) findings.

The convergence of takotsubo and Kounis syndromes, collectively referred to as the "ATAK complex" (short for adrenaline, takotsubo, anaphylaxis, and Kounis syndrome), poses a unique and challenging clinical scenario, especially in the context of chemotherapy-related anaphylaxis. We present a case report involving a 63-year-old woman undergoing chemotherapy for endometrial adenocarcinoma who experienced anaphylactic symptoms during treatment. Immediate administration of epinephrine was followed by the emergence of ST elevation, a reduced left ventricular ejection fraction, and wall motion abnormalities indicative of stress-induced cardiomyopathy.
