"Adipsic hypernatremia" is clinically characterized by chronic elevation of plasma [Na] with an inappropriate lack of thirst and upward resetting of the osmotic set point for arginine vasopressin (AVP) secretion, combined with a relative deficiency of AVP, thereby resulting in persistent hypernatremia. Many cases are accompanied by structural lesions in the hypothalamus, pituitary gland, and circumventricular organs (CVOs). On the other hand, cases without structural lesions have been reported since the 1970s, but the pathophysiology was unknown for a long time.

Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is an important therapeutic option for patients with hematologic malignancies. However, the development of graft-versus-host disease (GVHD) after allo-HSCT remains a challenge. Although systemic steroid therapy is the established first-line therapy for acute GVHD (aGVHD) and chronic GVHD (cGVHD), many patients are unresponsive or resistant to corticosteroid therapy, and the response is insufficient.

Sex differences exist in sepsis, but the commitment of neutrophils to these differences remains unclear. Neutrophil extracellular traps (NETs) function to remove pathogens, yet excessive NETs release can contribute to organ damage. This study explores effects of the gender hormones on endotoxin-induced NETs using neutrophils from both male and female sources.

Article Synopsis
  • Persistent inflammation in people living with HIV (PWH) may influence their immune response to SARS-CoV-2 infection differently than in HIV-uninfected individuals.
  • During a study, proinflammatory cytokine levels were measured in PWH, revealing no significant differences in cytokine levels across various COVID-19 severity levels, unlike in HIV-uninfected individuals where higher levels correlated with severe cases.
  • The findings indicate that PWH are in a heightened inflammatory state but exhibit distinct inflammatory responses to SARS-CoV-2 compared to the general population, possibly due to impairments in their immune cells and existing inflammation.

Chest tube insertion is a common and relatively safe procedure in an emergency setting. However, a potentially fatal complication, vasovagal reflex, may be under-recognized due to its generally mild severity. We present a case of pulseless electrical activity (PEA) requiring chest compression due to vasovagal reflex during chest tube insertion for spontaneous pneumothorax.

Background: There is no established standard 3 line treatment for patients with advanced non-small cell lung cancer (NSCLC). Although cytotoxic chemotherapeutic agents that are not used as 1 or 2 line treatment are administrated as 3 line treatment, their anti-tumor efficacy is insufficient. Anti-programmed death ligand-1 (PD-L1)/programmed death-1 (PD1) treatment is more effective and less toxic than chemotherapy in anti-PD-L1/PD-1 treatment-naïve patients with NSCLC.

Article Synopsis
  • The study aimed to evaluate the long-term outcomes, specifically overall survival (OS), of patients with T1 colorectal cancer (CRC) who underwent endoscopic resection (ER) followed by additional surgery (AS) compared to those who had primary surgery (PS) alone.
  • It involved analyzing data from 6,105 patients treated at various high-volume Japanese institutions and utilized propensity score matching to ensure comparability between the two groups.
  • The findings suggested that patients who underwent AS after ER had a 5-year OS rate of 97.1%, comparable to 96.0% for the PS group, indicating that ER before AS does not adversely affect long-term outcomes.

  • Hepatoid adenocarcinoma of the stomach (HAS) is a rare and aggressive type of gastric cancer, making up less than 1% of cases, often leading to liver metastasis and poor prognosis; treatment guidelines are currently lacking.
  • A study involving 19 male patients diagnosed with HAS aimed to analyze the outcomes of surgical interventions (gastrectomy and hepatectomy) and assess prognostic factors, revealing a 5-year survival rate of 67.7% post-gastrectomy and a 100% survival rate for 3 years after hepatectomy in patients with liver metastasis.
  • The findings suggest that the prognosis for HAS may be more optimistic than

  • Congenital renal arteriovenous malformations (AVMs) can lead to serious symptoms like recurrent gross hematuria, particularly in younger individuals, but may also present as acute abdominal pain in cases without a history of hematuria.
  • A case study involved a 36-year-old woman who initially showed symptoms suggestive of adnexal torsion, but advanced imaging revealed clot retention and signs of a renal AVM instead.
  • The successful treatment of the AVM via ethanol embolization underscores the need for awareness of atypical presentations of renal AVMs to enable early diagnosis and prevent severe complications like clot retention or rupture.

Background: Pancreatoduodenectomy and subtotal esophagectomy are widely considered the most invasive and difficult surgical procedures in gastrointestinal surgery. Subtotal esophagectomy after pancreatoduodenectomy is expected to be extremely difficult due to complicated anatomical changes, and selecting an appropriate intestinal reconstruction method will also be a difficult task. Therefore, perhaps because the method is considered impossible, there have been few reports of subtotal esophagectomy after pancreatoduodenectomy.

Article Synopsis
  • Immune-mediated necrotizing myopathy (IMNM) is an inflammatory muscle disease that typically doesn't present with skin rashes, but patients with the specific HMGCR antibody may exhibit atypical skin conditions.
  • A study of 100 IIM patients revealed that 34% had skin conditions, with 42% of those with HMGCR-IMNM showing atypical lesions, which were distinct from typical IIM-related rashes.
  • Skin biopsies from HMGCR-IMNM patients revealed similar microscopic features, specifically Bcl-2-positive lymphocyte infiltrations, suggesting that these atypical skin manifestations could be a clue for diagnosing HMGCR-IMNM.
