Department of Orthopaedic Surgery, Seoul National University College of Medicine, Seoul National University Hospital, Seoul, Republic of Korea.

Article Synopsis
  • The study investigates the effectiveness of supraspinatus muscle advancement (MA) in treating irreparable rotator cuff tears compared to traditional rotator cuff repair for reparable tears.
  • It included 62 patients, with 29 undergoing the MA procedure and 33 receiving standard treatment, assessing outcomes through imaging and clinical scores one year post-surgery.
  • Results showed similar retear rates and overall clinical outcomes between the MA and non-MA groups, but the MA group exhibited weaker muscle power during forward elevation and a slight decrease in supraspinatus atrophy.

Article Synopsis
  • Precision medicine requires reliable tools for personalized therapy, and this study tested the feasibility of the single patient classifier (SPC) test for stage II-III gastric cancer patients.
  • A multicenter study with 237 patients showed that 99.5% of SPC test results were delivered on time before choosing postoperative treatments, with a median delivery time of 4 days.
  • The results indicated significant differences in 3-year disease-free survival rates among the risk groups identified by the SPC test, supporting its potential for improving treatment decisions.

Article Synopsis
  • Chemoresistant-epithelial ovarian cancer (EOC) has a poor prognosis, prompting researchers to explore new treatments, particularly the anticancer potential of certain antipsychotic drugs from the diphenylbutylpiperidine (DPBP) class.
  • In vitro studies with various EOC cell lines and in vivo models showed that DPBP drugs, especially penfluridol, significantly reduced cell proliferation and increased apoptosis in both chemosensitive and chemoresistant cells.
  • The research demonstrated that penfluridol not only effectively reduced tumor weight in xenografts but also worked against paclitaxel-resistant models, suggesting its potential as a promising therapeutic option for EOC treatment.

Antithrombotic agents, including antiplatelet agents and anticoagulants, are widely used in Korea because of the increasing incidence of cardiocerebrovascular disease and the aging population. The management of patients using antithrombotic agents during endoscopic procedures is an important clinical challenge. The clinical practice guidelines for this issue, developed by the Korean Society of Gastrointestinal Endoscopy, were published in 2020.

Unlabelled: This population-based retrospective cohort study aimed to estimate the association between antibiotic exposure and osteoporotic fracture risk. Long-term antibiotic use was associated with the risk of osteoporotic fracture. An increase in the number of antibiotic classes prescribed may also be associated with an increased osteoporotic fracture risk.

Impact of Teriparatide and Denosumab on Clinical and Radiographic Outcomes in Osteoporotic Vertebral Compression Fractures.

Medicina (Kaunas)

August 2024

Spine Center and Department of Orthopaedic Surgery, Seoul National University College of Medicine, Seoul National University Bundang Hospital, Seongnam-si 13620, Republic of Korea.

Article Synopsis
  • * The retrospective study included 78 patients, assessing clinical outcomes like pain and quality of life at various time points, while evaluating bone mineral density (BMD) and fracture healing over a 12-month period.
  • * Both treatments significantly improved patient outcomes with no major differences found between them, suggesting that denosumab is a viable option for early treatment without affecting healing, although further research is needed for confirmation.

This systematic review and meta-analysis of randomized controlled trials (RCTs) with trial sequential analysis (TSA) aimed to comprehensively evaluate and compare the efficacy of the prophylactic administration of tropisetron in the prevention of the incidence of post-operative nausea and vomiting (PONV) in patients undergoing surgery under general anesthesia. This study was registered with PROSPERO (CRD42024372692). RCTs comparing the efficacy of the perioperative administration of tropisetron with that of a placebo, other anti-emetic agents, or a combination of anti-emetic injections were retrieved from the databases of Ovid-MEDLINE, Ovid-EMBASE, the Cochrane Central Register of Controlled Trials, and Google Scholar.

We evaluated the effectiveness of the predictive low-glucose suspend (PLGS) algorithm in the DIA:CONN G8. Forty people with type 1 diabetes mellitus (T1DM) who used a DIA:CONN G8 for at least 2 months with prior experience using pumps without and with PLGS were retrospectively analyzed. The objective was to assess the changes in time spent in hypoglycemia (percent of time below range [%TBR]) before and after using PLGS.

  • The Delphi consensus study, conducted by IHPBA-APHPBA, aimed to create global practice guidelines for managing gallbladder cancer (GBC).
  • Experts from 17 countries participated in a four-round consensus process, where 68 clinical questions were posed and a consensus was reached if more than 75% of participants agreed.
  • The study achieved consensus on 92.6% of the questions, covering important aspects of GBC management, but noted that further research is needed on unresolved issues such as the definitions of borderline resectable and locally advanced GBC.

Int J Cardiovasc Imaging

November 2024

Department of Cardiology, University of Ulsan College of Medicine, Asan Medical Center, 88, Olympic- ro 43-gil, Songpa-gu, Seoul, Korea.

This study was conducted to develop and validate a deep learning model for delineating intravascular ultrasound (IVUS) images of coronary arteries.Using a total of 1240 40-MHz IVUS pullbacks with 191,407 frames, the model for lumen and external elastic membrane (EEM) segmentation was developed. Both frame- and vessel-level performances and clinical impact of the model on 3-year cardiovascular events were evaluated in the independent data sets.

Article Synopsis
  • * Twenty patients were enrolled, and results after 8 months showed minimal in-stent late lumen loss (0.7 mm) and complete stent strut coverage, indicating successful healing.
  • * There were no adverse cardiovascular events reported within the same period, suggesting that the new stent design is safe and effective in treating coronary artery disease.

Article Synopsis
  • The study focused on analyzing changes in gut microbiota during anti-TNF-α therapy (adalimumab) in patients with ulcerative colitis (UC), noting that previous research had limited insights on long-term effects.
  • Results showed that the composition of gut microbiota varied significantly based on disease severity, with distinct microbial communities observed in UC patients compared to healthy individuals.
  • Clinical remission was linked to specific microbiota changes, including a reduction in some bacteria and an increase in beneficial ones, suggesting these microbial shifts could help predict treatment responses to adalimumab.

  • - The study explored if opioid-sparing anesthesia (OSA) can lower the rates of postoperative nausea and vomiting (PONV) in patients having laparoscopic gynecological surgery compared to conventional opioid-using anesthesia (OUA).
  • - 120 adult patients were randomly placed into either the OUA or OSA group, with OSA using minimal opioids and a multimodal non-opioid pain management approach, leading to a lower incidence of nausea and less need for opioid rescue analgesics in the PACU for the OSA group.
  • - Findings indicated that OSA not only reduced nausea and pain scores but also did not result in any significant changes in vital signs or length of hospital stay, suggesting it is a safer
