  • Acute liver failure (ALF) in dengue patients is uncommon but highly fatal; identifying those at risk is crucial for improving survival rates.
  • A study involving 2532 dengue patients revealed that factors like international normalized ratio, bilirubin, albumin, and creatinine can predict the likelihood of ALF and in-hospital mortality in patients with dengue-induced severe hepatitis (DISH).
  • Several prognostic scores, including the MELD score and ALBI score, effectively predicted ALF and mortality, demonstrating high sensitivity and specificity in clinical assessments.

Closed-loop systems for deep brain stimulation to treat neuropsychiatric disorders.

Expert Rev Med Devices

December 2024

Edmond J. Safra Program in Parkinson's Disease and Morton and Gloria Shulman Movement Disorders Clinic, Toronto Western Hospital, UHN, Division of Neurology, University of Toronto, Toronto, ON, Canada.

Introduction: A closed-loop or feedback-control system is a process which considers the system's output in order to automatically adjust the input. Compared to a traditional open-loop system, a closed-loop system allows for a higher degree of accuracy with minimal human intervention. Novel methods of closed loop 'adaptive' deep brain stimulation DBS (aDBS) are being developed.

  • * A study examined HIF-1α levels in the bone marrow of 29 newly diagnosed NPM1FLT3-ITD normal karyotype AML patients and found that over half exhibited strong HIF-1α expression, correlating this with increased relapse rates and shorter relapse-free survival (RFS).
  • * High HIF-1α levels might serve as a useful prognostic biomarker for predicting poor RFS and resistance to cytarabine in NPM1FLT

Case detection of familial hypercholesterolemia using various criteria during an annual health examination in the workplace.

Int J Cardiol Cardiovasc Risk Prev

December 2024

Division of Endocrinology and Metabolism, Department of Medicine, Faculty of Medicine, Chulalongkorn University and Excellence Center in Diabetes, Hormones and Metabolism, King Chulalongkorn Memorial Hospital, Bangkok, Thailand.

Article Synopsis
  • A workplace screening identified potential cases of familial hypercholesterolemia (FH) in a notable percentage of participants, indicating a need for early diagnosis to prevent cardiovascular disease.
  • The study compared clinical characteristics between those diagnosed with potential FH and those unlikely to have the condition, revealing higher rates of premature cardiovascular issues and other health risks in the potential FH group.
  • Despite some individuals in the potential FH group using lipid-lowering medications, achieving LDL-C goals was significantly lower compared to those unlikely to have FH, suggesting a need for targeted lifestyle changes and medication adjustments.

November 2024

Department of Obstetrics and Gynecology, Faculty of Medicine, Chulalongkorn University and King Chulalongkorn Memorial Hospital, 1873 Rama IV Road, Patum Wan, Patum Wan, Bangkok, 10330, Thailand.

Article Synopsis
  • Etonogestrel and levonorgestrel contraceptive implants are effective for 3 and 5 years, respectively, but early removal before due date can lead to unintended pregnancies, particularly among adolescents.
  • A study conducted at King Chulalongkorn Memorial Hospital analyzed early implant discontinuation from 2014 to 2019, finding a 19.3% overall discontinuation rate with intolerance to side effects, like abnormal bleeding, being the main reason for removal.
  • Results indicated that adolescents had a lower likelihood of early discontinuation than adults, with factors such as free implants, continuous use, postpartum status, and having children contributing to reduced discontinuation rates.

IL-18 biology in severe asthma.

Front Med (Lausanne)

November 2024

Department of Medicine, McMAster University, Hamilton, ON, Canada.

The role of interleukin-18 (IL-18) and inflammasomes in chronic inflammatory airway diseases, such as asthma and chronic obstructive pulmonary disease (COPD), has garnered significant attention in recent years. This review aims to provide an overview of the current understanding of IL-18 biology, the associated signaling pathways, and the involvement of inflammasome complexes in airway diseases. We explore the multifaceted role of IL-18 in asthma pathophysiology, including its interactions with other cytokines and contributions to both T2 and non-T2 inflammation.

  • Pelvic floor dysfunction (PVD) can be assessed using magnetic resonance defecography (MRD) in either supine or lateral decubitus positions, impacting quality of life.
  • A study comparing these positions found that key measurements and grading of PVD parameters, such as M-line and uterine prolapse, were significantly higher in the lateral decubitus position.
  • Although the image quality was better in the supine position, the lateral decubitus provided more severe PVD assessments according to the HMO grading system.

Article Synopsis
  • Low vitamin D levels are linked to low muscle mass or function in dialysis patients, and this study explored how different supplementation methods could improve muscle health in those with end-stage kidney disease (ESKD).
  • A randomized controlled trial compared two vitamin D treatment regimens—one based on severity and another with a fixed dose of 20,000 IU/week—on muscle mass, strength, and function in dialysis patients.
  • Results showed both regimens significantly raised vitamin D levels, but the fixed-dose group experienced a notable increase in muscle mass compared to the severity-dependent group, especially among patients on peritoneal dialysis.

Most meningiomas are dural-based extra-axial tumors in close contact with the brain. Expression of genes and proteins at the tumor-brain interface in brain-invasive meningioma is basically unknown. Using the NanoString pan-cancer panel, differential expression of genes in the invasive edge versus main tumor body was determined in 12 invasive meningiomas (comprising the discovery cohort), and 6 candidate genes: DTX1, RASGRF1, GRIN1, TNR, IL6, and NR4A1, were identified.

  • Stent misdeployment (SMD) is a significant challenge in EUS-guided choledochoduodenostomy (EUS-CDS) for treating malignant distal biliary obstruction, with the study aiming to define its rate and outcomes, and propose a classification system.
  • In a review of data from two randomized controlled trials involving 152 patients, SMD was found in 7.2% of cases, with most types being misdeployments of the distal flange (type I) and a small number causing minor adverse events.
  • The analysis revealed that a smaller extrahepatic bile duct diameter (≤ 15 mm) increases the likelihood of SMD or technical failures, but most misdeployments can be successfully managed

Article Synopsis
  • - The study focused on how parieto-motor paired stimulation affects brain plasticity in patients with writer's cramp (WC) and cervical dystonia (CD) compared to healthy volunteers.
  • - Using paired transcranial magnetic stimulation, researchers found that the stimulation increased motor cortex excitability in WC patients but not in CD patients or healthy individuals.
  • - The results indicate that while the stimulation enhances brain function in WC, it doesn't seem to relate to changes in the connectivity between the posterior parietal cortex and the motor cortex.
