Multiple endocrine neoplasia type 1 (MEN1) is a rare autosomal dominant disease with an estimated prevalence of 2 per 100,000. This disease is caused by a mutation in the tumor suppressor gene MEN1, which is located on chromosome 11 and codifies the menin protein. It is characterized by a predisposition of parathyroids, enteropancreatic, and anterior pituitary tumors, affecting the quality of life and lifespan of those who have the disease.

Autologous Stem-Cell Transplantation (ASTC) has proven efficacy in several hematological malignancies. The greatest antineoplastic effect achieved with intensified chemotherapy is associated with severe myelotoxicity. This report describes the case of a 64-year-old female patient, with multiple comorbidities which presented an ecthyma gangrenosum due to Klebsiella pneumoniae confirmed by blood and tissue cultures. It finally ended in the amputation of the limb. The case is presented given the low prevalence of positive cultures for this microorganism in this pathology associated to devastating consequences for the patient.
