The gastrointestinal tract can be deranged by ailments including sepsis, trauma and haemorrhage. Ischaemic injury provokes a common constellation of microscopic and macroscopic changes that, together with the paradoxical exacerbation of cellular dysfunction and death following restoration of blood flow, are collectively known as ischaemia-reperfusion injury (IRI). Although much of the gastrointestinal tract is normally hypoxemic, intestinal IRI results when there is inadequate oxygen availability due to poor supply (pathological hypoxia) or abnormal tissue oxygen use and metabolism (dysoxia).

Article Synopsis
  • The pathophysiology of pediatric hydrocephalus remains unclear, and valve-based shunt systems have been the primary treatment option since the 1950s, transitioning from ventricular-atrial systems to peritoneal systems in the 1970s.
  • The review highlights the comparison of valve types and emphasizes endoscopic third ventriculostomy (ETV) as the only surgical alternative today, which offers lower infection and re-operation rates, especially for children with previous shunt failures.
  • It also identifies key predictive factors for ETV success and attempts to determine optimal treatment strategies for different patient subgroups, aiming to improve long-term outcomes for pediatric hydrocephalus patients.

Article Synopsis
  • The immune system relies on the growth and specialization of specific lymphocytes to function effectively.
  • Telomeres, protective structures at the ends of chromosomes, vary in length among different lymphocyte types and can shorten with age.
  • Certain immune disorders, like dyskeratosis congenita and ICF syndrome, are linked to abnormal telomere shortening and issues with DNA repair and recombination.

Introduction: Intraoperative hemodynamic and metabolic optimization of both the high-risk surgical patients and critically ill patients remains challenging. Reductions in oxygen delivery or increases in oxygen consumption can initiate complex cellular processes precipitating oxygen debt (OXD).

Methods: This study tested the hypothesis that intraoperative changes in sublingual microcirculatory flow reflect clinically relevant transitions from aerobic to anaerobic metabolism (TRANAM).

Shwachman Diamond Syndrome (SDS) is a multi-system disease characterized by exocrine pancreatic insufficiency with malabsorption, infantile neutropenia and aplastic anemia. Life-threatening complications include progression to acute myeloid leukemia (AML) or myelodysplastic syndrome (MDS), critical deep-tissue infections and asphyxiating thoracic dystrophy. In most patients, SDS results from biallelic pathogenic variants in the gene, different combinations of which contribute to heterogenous clinical presentations.

  • A systematic review of randomized controlled trials from 2000 to 2023 identified 34 studies, with five focusing on Low Level Laser Therapy (LLLT) and honey, which were included in the meta-analysis.
  • The results showed that honey can reduce hospital stay duration for severe OM, while LLLT was ineffective in preventing or treating moderate to severe OM, indicating that honey may be a promising treatment option for managing OM in children.

Clinical Presentation of Patients with Eosinophilic Gastrointestinal Diseases beyond Eosinophilic Esophagitis.

Immunol Allergy Clin North Am

May 2024

Institute of Gastroenterology, Nutrition, and Liver Diseases, Schneider Children's Medical Center of Israel, Tel-Aviv University, 14 Kaplan Street, PO Box 559, Petach Tikvah 4920235, Israel.

The clinical presentation of eosinophilic gastrointestinal diseases beyond eosinophilic esophagitis (non-EoE EGIDs) varies depending on the gastrointestinal segments affected by the eosinophilic inflammation, the extent of eosinophilic inflammation within the gastrointestinal tract and its depth through the bowel wall. Non-EoE EGIDs with mucosal involvement tend to present with diarrhea, malabsorption, and sometimes bleeding, those with muscular involvement may present with symptoms of obstruction or pseudo-obstruction, intussusception, and even perforation, whereas those with serosal involvement may present with eosinophilic ascites. Here we describe the differences in symptoms experienced by children with non-EoE EGIDs with varying degrees of eosinophilic inflammation through the bowel wall.
