Longitudinal Reliability of Milestones Learning Trajectories during Anesthesiology Residency.

Background: Longitudinal Milestones data reported to the Accreditation Council for Graduate Medical Education (ACGME) provide a structured framework for assessing the developmental progression of residents in key competencies and subcompetencies. This study aims to investigate the previously underexplored longitudinal reliability of Milestones data, with the goal of identifying patterns in learning trajectories that can inform targeted interventions for residents and programs.

Methods: We conducted a retrospective cohort study with national anesthesiology Milestones data collected from 2014 to 2020.

Detecting noncredible symptomology in ADHD evaluations using machine learning.

Introduction: Diagnostic evaluations for attention-deficit/hyperactivity disorder (ADHD) are becoming increasingly complicated by the number of adults who fabricate or exaggerate symptoms. Novel methods are needed to improve the assessment process required to detect these noncredible symptoms. The present study investigated whether unsupervised machine learning (ML) could serve as one such method, and detect noncredible symptom reporting in adults undergoing ADHD evaluations.

Breathing Easy: A Qualitative Analysis of School Nurse and Administrator Perspectives on Stock Inhalers and School-Based Asthma Management.

Stock inhalers are unassigned rescue inhalers available for students who experience asthma symptoms at school. This study sought to understand school nurse and administrator perspectives on school-based asthma management and stock inhaler policy implementation. Twenty-three semistructured interviews were conducted with professionals from five high asthma burdened counties in varied geographic settings.

Social Determinants, Mental Well-Being, and Disrupted Life Transitions Among Young Adults with Disabling Mental Health Conditions.

This study sought to understand how young adults (age 18-25) with histories of mental health disorders are coping with disrupted transitions to adulthood during the COVID-19 pandemic. A cross-sectional web survey was conducted in March-June 2021 of 967 US young adults with pre-pandemic psychiatric disability to assess their current psychiatric status, interrupted transitions, and associations with social determinants including income, community participation, and social context. Mental health was assessed with the Patient Health Questionnaire (PHQ-9), Generalized Anxiety Disorder Scale (GAD-7), and PTSD Checklist-Civilian Version.

Anxiety symptoms are distinctly related to working memory deficits in adults with ADHD.

Working memory (WM), the cognitive system that briefly stores and updates information during complex tasks, is one of the most consistently identified neurocognitive deficits in individuals with ADHD. WM deficits are linked to significant challenges in daily life. Adults with ADHD often experience co-occurring anxiety and mood disorders, which are associated with more severe clinical presentations and greater WM deficits.

The Perceived Impact of Global Surgery Engagement on Career Advancement among US Medical Students.

Introduction: There is growing interest among medical institutions to formalize global surgery training. Understanding medical students' perceptions of how global surgery engagement can enhance career advancement is essential for providing appropriate guidance and support for individuals with aspirations in academic global surgery.

Methods: A cross-sectional survey study of US-based medical students from 38 participating schools was performed.

Proximity to traffic is associated with worse symptom severity in patients with chronic rhinosinusitis with nasal polyps.

Background: Environmental exposures may be associated with increased severity of chronic rhinosinusitis (CRS). However, research examining associations of traffic related air pollution with CRS is limited. The purpose of this study was to determine the association between residential traffic proximity and CRS with nasal polyposis (CRSwNP) severity in an existing database of adults in the United States.

This study investigated whether unsupervised machine learning (ML) could serve as one such method. Participants were 359 adult outpatients who underwent a neuropsychological evaluation for various referral reasons.

Sex moderates diet quality differences in integrated collaborative care for comorbid obesity and depression: Post-hoc analysis of the RAINBOW RCT.

Objectives: To investigate (1) whether an evidence-based behavioral weight loss intervention was associated with improved diet quality in adults with obesity and depression compared with usual care; and (2) whether the associations were modified by sex.

Design: In the RAINBOW RCT, 409 participants were randomized in 1:1 ratio to receive a 12-month intervention integrating a Diabetes Prevention Program-based behavioral weight loss treatment with problem-solving therapy for depression (n = 204) or usual care (n = 205). Participants completed 24-h dietary recalls at baseline, 6, 12, 18, and 24 months.

Endocrine pathology in young rabbits with cystic fibrosis.

Background: Cystic fibrosis (CF) is an autosomal recessive genetic disorder caused by loss-of-function mutations in the transmembrane conductance regulator gene. CF-related pancreatic lesions are known to cause exocrine dysfunctions such as pancreatic insufficiency, and endocrine dysfunctions, including CF related diabetes. In a previous study, we generated CF rabbits using CRISPR/Cas9-mediated gene editing.

Changing behavior and promoting clinical empathy through a patient experience curriculum for health profession students.

Objectives: The authors sought to explore how a curriculum that uses a patient experience simulation followed by reflection can lead to clinical empathy in learners and whether this experience leads to behavioral change. Further, in response to critiques of common pragmatic approaches to clinical empathy teaching in which empathy is operationalized and taught through formal trainings and checklists, the study aimed to contribute insights regarding how clinical empathy may best be taught to health profession students.

Postoperative management of pediatric patients undergoing single-stage laryngotracheal reconstruction in the United States: A survey of ASPO members.

Introduction: The postoperative management of single-stage laryngotracheal reconstruction (ssLTR) plays a significant role in the surgery's outcomes. The relatively prolonged period in which the child remains intubated and sedated to allow graft healing may be complicated by pulmonary sequelae, airway obstruction, withdrawal symptoms, and eventually failed extubation. This study aims to assess post-ssLTR practices among pediatric otolaryngologists.

Effects of genetic knockdown of the serotonin transporter on established L-DOPA-induced dyskinesia and gene expression in hemiparkinsonian rats.

Parkinson's disease (PD) is a neurodegenerative disorder typified by the loss of dopamine (DA) neurons in the substantia nigra pars compacta (SNpc) leading to motor symptoms including resting tremor, rigidity, akinesia, and postural instability. DA replacement therapy with levodopa (L-DOPA) remains the gold-standard treatment for the motor symptoms of PD. Unfortunately, chronic use of L-DOPA leads to the development of side effects known as L-DOPA-induced dyskinesia (LID).

Transradial versus transfemoral access for mechanical thrombectomy: A single institution experience.

Background: There has been debate in the literature regarding the adoption of a "radial-first" approach for mechanical thrombectomy (MT) in acute ischemic stroke (AIS). Conflicting reports suggest that transradial access (TRA) may allow for shorter times to reperfusion while others conclude that long-term functional outcomes may favor transfemoral access (TFA). Here, we report a single-institution experience with the adoption of TRA as the primary route for acute stroke intervention.

Comparing a Model of Augmented Postpartum Primary Care to Usual Care in an Urban Medical Center.

Background: The US faces a maternal health crisis and struggles to deliver recommended postpartum care. In some populations, less than half of mothers attend a postpartum visit.

Objective: To determine if a two-generation (Two-Gen) model of interdisciplinary, postpartum primary care was associated with increased visit attendance for postpartum care, primary care, and behavioral health.

Physiological Modeling of the Vascularized Human Lung Organoid.

Studies using human lung organoids (hLO) have focused on differentiation of lung epithelial subtypes into distal alveolar unit. A major question has been whether introducing endothelial cells (EC) and resultant vascularization alter development of hLO. We describe herein a method for vessel infiltration of hLO in which we determined differences of these hLOs with standard avascular hLOs.
