  • The study investigates bladder dysfunction in patients with multiple system atrophy (MSA), focusing on the relationship between bladder contractility, post-void residuals (PVR), and voided percentage (VOID%).
  • Researchers analyzed data from 133 MSA patients to determine the best value of "k" (a variable in the contractility formula) that correlates with PVR and VOID%.
  • The findings suggest that a "k" value of over 5.0 is suitable for males, but a higher value (over 6.0) may lead to overestimation in females, indicating a need for improved formulas for female patients.

Background: Although the percentage of the population with a high degree of obesity (body mass index [BMI] ≥ 35 kg/m) is low in Japan, the prevalence of obesity-related diseases in patients with high-degree obesity is greater than that in patients with a BMI < 35 kg/m. Therefore, treatment for high-degree obesity is important. However, clinical studies have reported that 20-50% of patients with obesity discontinue weight-loss treatment in other countries.

Article Synopsis
  • Pyrogens, including bacterial endotoxins and non-endotoxin pyrogens (NEPs), can cause fever or shock when they enter the bloodstream, prompting the need for effective detection methods in medical products.
  • A new monocyte-activation test (MAT) has been developed to identify these pyrogens, but established controls don't work uniformly for both endotoxins and NEPs, highlighting a need for specific positive controls for NEPs.
  • This study focused on creating heat-killed Staphylococcus aureus (HKSA) as a positive control for NEPs by optimizing a filtration method, finding that a double filtration process using specific glass filters produced a consistent bacterial suspension suitable for MAT testing.

Introduction: Cardio-ankle vascular index (CAVI) is an arterial stiffness index that correlates inversely with body mass index (BMI) and subcutaneous fat area. Lipoprotein lipase (LPL) that catalyzes the hydrolysis of serum triglycerides is produced mainly in adipocytes. Serum LPL mass reflects LPL expression in adipose tissue, and its changes correlate inversely with changes in CAVI.

  • The study examines if a standardized T1-weighted/T2-weighted (sT1w/T2w) ratio can effectively identify differences in brain degeneration between multiple system atrophy with predominant parkinsonism (MSA-P) and Parkinson's disease (PD).
  • Researchers analyzed MRI scans from 35 MSA-P patients, 32 PD patients, and 17 controls, finding that MSA-P patients had significantly lower sT1w/T2w ratios compared to both PD patients and controls.
  • The sT1w/T2w ratio demonstrated strong accuracy in distinguishing MSA-P and PD from controls, and showed substantial potential to differentiate MSA-P from PD, making it a promising tool for early diagnosis.

Challenges in the diagnosis of the enigmatic primary adrenal leiomyosarcoma: two case reports and review of the literature.

BMC Endocr Disord

December 2023

Department of Endocrinology, Hematology and Gerontology, Graduate School of Medicine, Chiba University, 1-8-1 Inohana, Chuo-ku, Chiba, 260-8670, Japan.

Background: Primary adrenal leiomyosarcoma is a rare and aggressive mesenchymal tumor derived from the smooth muscle wall of a central adrenal vein or its tributaries; therefore, tumors tend to invade the inferior vena cava and cause thrombosis. The great majority of tumors grow rapidly, which makes the disease difficult to diagnose in its early clinical stages and needs differentiation from adrenocortical carcinomas for the selection of chemotherapy including mitotane which causes adrenal insufficiency.

Case Presentation: We presented two patients with adrenal leiomyosarcoma who were referred to our hospital with abdominal pain and harboring large adrenal tumors and inferior vena cava thrombosis.
