Background: Despite accumulating evidence and recommendations for management of colonic diverticular bleeding (CDB), the changes in its clinical management and outcomes remain unknown.

Methods: We performed a retrospective tendency analysis on a biennial basis, a propensity score-matched cohort study between the first and latter half groups, and mediation analyses to compare the diagnostic and treatment methods between January 2010 and December 2019 (CODE BLUE-J Study).

Results: A total of 6575 patients with CDB were included.

Tralokinumab, an anti-IL-13 antibody, is an effective treatment for patients with atopic dermatitis (AD). However, predictive factors for responders to tralokinumab remain unclear in real-world practice. This study aimed to identify predictive factors for early and late responders to tralokinumab treatment.

Background: With the advent of effective chemotherapy, conversion surgery (CS) has been performed in patients who have responded to pretreatment, even for pancreatic cancer diagnosed as unresectable (UR) at the time of initial diagnosis. In CS, major arterial resection and reconstruction are necessary for complete radical resection.

Methods: We discuss the key points for safely performing pancreatectomy with celiac axis (CA) resection combined with reconstruction, divided into resection and arterial reconstruction.

Article Synopsis
  • * A study conducted from August 2021 to March 2024 analyzed 204 Japanese AD patients, comparing clinical and lab data between those who responded and those who did not respond to the treatment.
  • * Findings indicated primary nonresponders had higher levels of eczema severity and various inflammatory markers, while secondary nonresponders were more likely to have used previous systemic treatments like dupilumab or corticosteroids.

Background: The association between the clinical significance and prognostic impact of catheter-based micro-axial ventricular assist device Impella (Abiomed, Danvers, MA, USA) support and age has not been sufficiently investigated.

Methods And Results: A total of 5282 patients treated with Impella were analyzed from the Japanese registry for Percutaneous Ventricular Assist Device (J-PVAD). Patients were categorized into four age groups: <50 years old (Group A, n = 670), 50-69 years old (Group B, n = 2009), 70-79 years old (Group C, n = 1716), and ≥80 years old (Group D, n = 887).

Article Synopsis
  • Tralokinumab, a human monoclonal antibody, is an approved treatment for atopic dermatitis (AD) and was evaluated for its effectiveness and safety in Japanese patients between October 2023 and March 2024.
  • In a study involving 103 patients, significant improvements in eczema severity were observed at weeks 4 and 12, including notable reductions in the Eczema Area and Severity Index (EASI) scores and itch intensity.
  • The treatment was generally well-tolerated, with mild and manageable side effects reported in 14.8% of patients, including a low incidence of conjunctivitis that resolved on its own.

Background And Aims: We sought to validate the British Society of Gastroenterology (BSG) guidelines for acute lower gastrointestinal bleeding (ALGIB).

Methods: We analyzed 8,956 patients with ALGIB in CODE BLUE-J study and categorized them into four groups based on BSG guidelines. Outcomes included 30-day rebleeding, 30-day mortality, blood transfusion, therapeutic intervention, and severe bleeding.

  • A 46-year-old woman with headaches and left hemiplegia due to intracranial internal carotid artery (IICA) dissection underwent an emergency STA-MCA bypass after experiencing cerebral hypoperfusion.
  • Post-surgery, her symptoms improved, and contrast-enhanced MRI revealed better delineation of the carotid artery over time, supporting her recovery.
  • The study emphasizes the effectiveness of STA-MCA bypass in increasing blood flow to the brain and highlights the usefulness of MRI in monitoring disease progression in IICA dissections.

Determining optimal pretreatment in cardiac surgery: an experimental study.

Gen Thorac Cardiovasc Surg

November 2024

Department of Cardiovascular Surgery, Nippon Medical School Hospital, 1-1-5 Sendagi, Bunkyo, Tokyo, 113-8603, Japan.

Objectives: Heart failure patients with reduced ejection fraction are currently treated with four drug combinations: angiotensin receptor/neprilysin inhibitors, beta-blockers, mineralocorticoid receptor antagonists, and sodium-glucose cotransporter 2 inhibitors, resulting in improved survival outcomes. Herein, we examined whether myocardial protection by esaxerenone or sacubitril/valsartan may present a counter-effect to the harm caused by cardioplegic arrest.

Methods: Male Wistar rats fed a normal diet were orally administered esaxerenone (3 mg/kg; Esax) or sacubitril/valsartan (68 mg/kg; SaV) once a day for 2 weeks from 6 weeks of age.

The artery of Percheron (AOP), a common anatomic vascular variation of the P1 segment of the posterior cerebral artery, provides arterial blood supply to the paramedian thalami and rostral midbrain. Occlusion of the AOP can lead to infarction of the bilateral paramedian thalamus, with or without midbrain involvement, but is rare in children. Here, we describe a case involving a 14-year-old girl with sudden onset of disturbance of consciousness, hypersomnia, and global aphasia.

  • * Delayed diagnosis can result in issues like abscesses, prolonged infections, or chronic joint problems.
  • * MRI is an effective, noninvasive tool for diagnosis, as illustrated by a case of an 11-year-old girl diagnosed just 3 days after her symptoms began, highlighting its importance in detecting this condition early in children.

  • The study compares two techniques—Range-of-Motion (ROM) and Akagi's line—for aligning the tibial component in total knee arthroplasty (TKA) to see how differently they align the tibia.
  • Results showed that the ROM technique generally led to a tibial component aligned 2.5° more externally than Akagi's line, particularly in knees with a smaller preoperative medial proximal tibial angle.
  • Overall, while the techniques produced different alignment values, they didn’t significantly differ in terms of tibial rotational movement during the procedure.

Background: We aimed to investigate the contribution of the palatopharyngeal muscle (PP) as a speech muscle in adjusting the velar position.

Methods: X-ray kinematic analysis of the position of the palatopharyngeal arch and an electromyographic study of the PP during speech were performed in two healthy volunteers.

Results: X-ray kinematic analysis revealed that the palatopharyngeal arch was positioned lower during the production of the low-back vowel /a/.

  • * A study of 56 Japanese patients showed that after 52 weeks of bimekizumab treatment, a significant percentage achieved maximum improvement in their psoriasis severity (PASI 100).
  • * The findings suggest that younger patients and those with lower monocyte-to-lymphocyte ratios are more likely to have a successful long-term response to the treatment, indicating potential for tailored therapy approaches.

Objectives: To explore the predictive performance on the need for surgical intervention in patients with adhesive small bowel obstruction (ASBO) using machine-learning (ML) algorithms and investigate the optimal timing for transition to surgery.

Methods: One hundred and six patients with ASBO who initially underwent long transnasal intestinal tube (LT) decompression were enrolled in this retrospective study. Traditional logistic regression analysis and ML algorithms were used to evaluate the risk of need for surgical intervention.
