Fascia turnover procedure at breast reconstruction using free TRAM flap for decreasing umbilical migration.

J Plast Reconstr Aesthet Surg

October 2023

Department of Plastic, Reconstructive and Aesthetic Surgery, Chiba University, Faculty of Medicine, Japan.

Background: The ideal umbilical position is midway between the two iliac crests. Some patients complained that the umbilicus position shifted from the midline after the breast reconstruction with a free muscle-sparing transverse rectus abdominis musculocutaneous (MS-TRAM) flap. We considered that the fascia of the external oblique muscle could be applied to the rectus abdominis fascia defect.

Division of Molecular Pathology, National Cancer Center Research Institute, Tokyo, Japan; Department of Diagnostic Pathology, National Cancer Center Hospital, Tokyo, Japan. Electronic address:

Hyalinizing clear cell carcinoma (HCCC) is a rare indolent malignant tumor of minor salivary gland origin with EWSR1::ATF1 rearrangement. Pathologically, the tumor cells possess a clear cytoplasm in a background of hyalinized stroma. Generally, the tumor cells are positive for p63 and p40 and negative for s100 and α-smooth muscle actin, suggesting that they differentiate into squamous epithelium and not into myoepithelium.

Article Synopsis
  • Researchers studied 50 patients with solitary fibrous tumours (SFT) to see how things found before and during surgery were connected and how they affected recovery.
  • They looked at details like MRI scans and how long surgeries took, finding that more complicated tumours often took longer to remove and caused more bleeding.
  • The study concluded that some signs seen before surgery, like the type of tumour and blood vessels around it, can help predict how tricky the surgery will be and the chance of problems afterward.

Correlation between abdominal visceral volume measured by a simple procedure and bulging at the abdominal free flap donor site for breast reconstruction.

J Plast Reconstr Aesthet Surg

September 2023

Department of Plastic, Reconstructive and Aesthetic Surgery, Chiba University, Faculty of Medicine, 666-2 Nitona, Chuo-ku, Chiba City, Chiba 260-8717, Japan.

Background: Breast reconstructions using autologous abdominal tissue result in breasts with a natural shape and consistency. One of the major complications is abdominal bulging. Because of the increased abdominal wall tension, high visceral volume (not visceral fat alone) may increase the occurrence rate of abdominal bulging.

  • Circulating tumor DNA (ctDNA) genotyping using next-generation sequencing (NGS) may aid in targeted therapy for metastatic colorectal cancer (mCRC), but its effectiveness in assessing the V600E mutation and guiding therapies needs further validation.
  • A study compared NGS-based ctDNA testing to traditional tissue testing in 212 mCRC patients, showing high concordance, sensitivity, and specificity rates for detecting V600E mutations, particularly when ctDNA levels were ≥1.0%.
  • The results indicate that ctDNA genotyping is a reliable method for identifying V600E mutations and can inform treatment decisions for anti-EGFR and BRAF therapies, highlighting its potential role in improving patient outcomes.

This systematic review was performed to investigate the superiority of proton beam therapy (PBT) to photon-based radiotherapy (RT) in treating esophageal cancer patients, especially those with poor cardiopulmonary function. The MEDLINE (PubMed) and ICHUSHI (Japana Centra Revuo Medicina) databases were searched from January 2000 to August 2020 for studies evaluating one end point at least as follows; overall survival, progression-free survival, grade ≥ 3 cardiopulmonary toxicities, dose-volume histograms, or lymphopenia or absolute lymphocyte counts (ALCs) in esophageal cancer patients treated with PBT or photon-based RT. Of 286 selected studies, 23 including 1 randomized control study, 2 propensity matched analyses, and 20 cohort studies were eligible for qualitative review.

Utility of the intraflap perfusion procedure for abdominal free flap in unilateral breast reconstruction.

J Plast Reconstr Aesthet Surg

September 2023

Department of Plastic, Reconstructive and Aesthetic Surgery, Chiba University, Faculty of Medicine, Japan.

Background: Heparin prophylaxis for venous thromboembolism can be used in microsurgery. If vein anastomosis is performed before the artery, heparin irrigation into the artery can be performed locally without systematic effect. This study aimed to introduce this "intraflap perfusion procedure" in autologous breast reconstruction.

Article Synopsis
  • - This study evaluated the effectiveness of nivolumab monotherapy for metastatic renal cell carcinoma (mRCC) in Japanese patients over a 36-month period, focusing on overall survival, progression-free survival, and response rates.
  • - Out of 208 patients, about 12% remained on nivolumab treatment after 36 months, with a significant portion discontinuing due to disease progression; the overall survival rate at 36 months was 54.3%.
  • - The findings suggest that nivolumab offers a notable survival advantage, especially for patients with favorable risk, and highlights the benefits of continuing treatment after nivolumab with other therapies like VEGF receptor-TKIs.

  • A phase II trial (JCOG1106) compared two treatment approaches for locally advanced pancreatic cancer: chemoradiotherapy with induction chemotherapy (Arm B) vs. without it (Arm A), finding better long-term survival in Arm A.
  • The study analyzed 51 patients from the trial, using Cox regression to explore how the systemic inflammatory response impacted overall survival, focusing on factors like C-reactive protein and albumin levels.
  • Results indicated that patients with a higher Glasgow Prognostic Score and indicators of inflammation (high C-reactive protein, low albumin) experienced a survival benefit from the induction chemotherapy, suggesting the inflammatory response could guide treatment choices.

Identifying the vulnerability of altered DNA repair machinery that displays synthetic lethality with amplification is a therapeutic rationale in unfavourable neuroblastoma. However, none of the inhibitors for DNA repair proteins are established as standard therapy in neuroblastoma. Here, we investigated whether DNA-PK inhibitor (DNA-PKi) could inhibit the proliferation of spheroids derived from neuroblastomas of MYCN transgenic mice and -amplified neuroblastoma cell lines.

Background: Breast reconstruction using endoscopically assisted latissimus dorsi (LD) flap leaves no scar on the back; however, the small amount of tissue obtained makes this procedure less practical. This study aimed to propose a new technique of endoscopically assisted extended LD (eeLD) flap plus lipofilling, which could secure a large breast volume.

Methods: Lateral thoracic adipose tissues supplied by the thoracodorsal artery branches and the LD muscle were elevated as a single unit only through the mastectomy scar and three ports through the lateral chest.
