Despite the well-established significance of the CAC score as a cardiovascular risk marker, the timing of using CAC score in routine clinical practice remains unclear. We aim to develop a prediction model for patients visiting outpatient cardiology units, which can recommend whether CAC score screening is necessary. A prediction model using retrospective cross-sectional design was conducted.

Acute normovolemic hemodilution in cardiac surgery: Rationale and design of a multicenter randomized trial.

Contemp Clin Trials

August 2024

Department of Anesthesia and Intensive Care, IRCCS San Raffaele Scientific Institute, Milan, Italy; Vita-Salute San Raffaele University, Milan, Italy.

Article Synopsis
  • The study evaluates acute normovolemic hemodilution (ANH) as a method to decrease the need for red blood cell (RBC) transfusions in patients undergoing elective cardiac surgery, aiming to reduce associated risks and costs.
  • It is a randomized controlled trial conducted in various hospitals, where patients are assigned to either receive ANH before surgery or the best available alternative treatment.
  • The primary goal is to see if ANH lowers the percentage of patients requiring RBC transfusion from 35% to 28%, with secondary outcomes including mortality and complications related to kidney and bleeding issues.

Article Synopsis
  • This study looked at how standard medications for systemic lupus erythematosus (SLE) affect important health outcomes like disease activity, flare-ups, and damage over time, using a substantial patient data set from the Asia Pacific Lupus Collaboration (APLC).
  • Findings showed that a significant percentage of patients reached low disease activity levels, but many also had flares, with variations in medication use across different countries; specifically, some medications appeared to have a steroid-sparing effect.
  • Key results revealed that patients on specific medications like tacrolimus had better odds of achieving low disease activity, while those taking azathioprine and methotrexate were less likely to reach that outcome; however

Cost and effectiveness comparison of sirolimus versus standard treatment in Kasabach-Merritt phenomenon: a real-world evidence study in Thailand.

Pediatr Hematol Oncol

April 2024

Division of Hematology and Oncology, Department of Pediatrics, Faculty of Medicine King Chulalongkorn Memorial Hospital, Chulalongkorn University, Bangkok, Thailand.

Article Synopsis
  • The study compares the first-year costs and effectiveness of sirolimus versus conventional treatments (corticosteroids and vincristine/vinblastine) for Kasabach-Merritt Phenomenon (KMP) in children aged 9 months to 12 years, analyzed from research spanning 2000 to 2022 in Thailand.
  • Researchers found no significant differences in total treatment costs or therapeutic effectiveness when comparing sirolimus to conventional treatments in the overall patient group; costs were nearly similar at around $8,800 to $9,100.
  • However, in a smaller subgroup of patients receiving sirolimus as first-line treatment, costs were notably lower compared to conventional treatment, with a significant reduction in treatment duration

Background: The selection and categorisation of laboratory tests in disease activity measures used within systemic lupus erythematosus (SLE) trial endpoints lack strong evidence. We aimed to determine whether longitudinal improvements in routinely measured laboratory tests are associated with measures of clinical improvement in patients with baseline active SLE.

Methods: We included patients from a multicentre longitudinal cohort (recruited between May 1, 2013, and Dec 31, 2019) with active SLE (SLEDAI-2K ≥6) coinciding with an abnormality in at least one of 13 routine laboratory tests, at a visit designated as baseline.

Objective: In trials of systemic lupus erythematosus (SLE), the SLE Responder Index (SRI) is the most commonly used primary efficacy end point but has limited validation against long-term outcomes. We aimed to investigate associations of attainment of a modified version of the SRI (mSRI) with key clinical outcomes in SLE patients with up to 5 years of follow-up.

Methods: We used data from a large multicenter, longitudinal SLE cohort in which patients received standard of care.

Background: Care bundles for ventilator-associated pneumonia (VAP) have been shown to minimize the rate of VAP in critically ill patients. Standard care bundles may need to be modified in resource-constrained situations. The goal of this study was to see if our modified VAP-care bundles lowered the risk of VAP in neurosurgical patients.

Article Synopsis
  • The study examined the prevalence and factors associated with leucopenia (low white blood cell count) in patients with Systemic Lupus Erythematosus (SLE) using data from a multinational cohort from 2013 to 2018.
  • Out of 2330 patients, 43.7% experienced at least one episode of leucopenia, with 37.2% having lymphopenia and 12.5% exhibiting neutropenia.
  • The findings revealed that lymphopenia was linked to disease activity and specific medications, while neutropenia showed a different relationship with treatment, indicating they should be studied separately in SLE research.

November 2020

Department of Orthopaedics, Faculty of Medicine, Chiang Mai University Hospital, Chiang Mai, 50200, Thailand.

Background: Excess aldosterone has been shown to be associated with obesity; however, there is currently a lack of data regarding the relationship between percentage of body fat and primary aldosteronism (PA), particularly pertaining to Asian populations. Furthermore, essential hypertension may mimic the condition of PA and there needs to be differentiation between the two. This study aimed to assess the association between percentage of body fat and PA.
